2.A Study on Parents' Satisfaction with School Foodservice by Foodservice Monitoring Participation.

This study was carried out to compare parents' satisfaction with school foodservice between participants and non-participants of foodservice monitoring. Surveys were conducted in six schools that implemented foodservice monitoring. In these schools, 126 participants and 167 non-participants were surveyed. The results were as follows: The participant group was more satisfied with taste (participation 3.63, non-participation 3.20), quantity (participation 3.69, non-participation 3.20), and menu composition (participation 3.67, non-participation 3.16) than the non-participant group. In addition, the satisfaction level of the participant group (3.74) was significantly higher than that of the non-participant group (3.24) (p<0.01). It was also indicated that the overall level of trust toward the foodservice operation was significantly higher in the participant group (3.90) than in the non-participant group (3.32) (p<0.001). After participating in monitoring, 74.8% of the parents changed their perception on school foodservice and 84.7% of them became to have more positive views. In conclusion, we found that parents' participation in school foodservice monitoring had positive effects on their satisfaction and perceptions on school foodservice.

3.A Case of Vulvar Leiomyoma.

Vulvar leiomyoma is a rare benign solid tumor of the vulva. Their incidence is not known as most reports are of a single case or small series, and their origin is not known exactly. Most cases are asymptomatic and local excision is recommended as initial therapy and also for recurrences. We have experienced vulvar leiomyoma (the labia majora and vestibule of the vagina) in a 51 years old woman and present it with a brief review of the literatures.

4.Two Case of Acute Pulmonary Edema due to Local Epinephrine Injection during Tonsillectomy.

Epinephrine is used to control bleeding and clear operative field during tonsillectomy procedure by local and general anesthesia. But epinephrine also has many adverse effects, such as palpitation, headache, hypertension, arrhythmia, tachycardia, anxiety and some significantly more dangerous conditions including intracranial hemorrhage, pulmonary edema, cardiac arrest, etc. We have experienced two acute pulmonary edema owing to infiltration of epinephrine during tonsillectomy, where the condition of patients rapidly became more serious. We performed intensive care and ventilation therapy. In all cases, patients were recovered after intensive care and additional postoperative complication has not been detected until now. We report these cases with literature.

5.A Case of Oculocardiac Reflex During Endoscopic Sinus Surgery.

Endoscopic sinus surgery has been used popularly for treatment of chronic paranasal sinusitis. Sometimes endoscopic sinus surgery is complicated by narrow visual field and anatomical variations. Oculocardiac reflex is developed by surgical or nonsurgical procedures to the eyeball. It occurs because of decreasing of heart rate and other arrhythmia including atrioventricular block, ventricular premature beat and cardiac arrest. Oculocardiac reflex during endoscopic sinus surgery is unusual and it is potentially a life threatening complication. The authors report the first case of oculocardiac reflex experienced during endoscopic sinus surgery in Korea with a review of the literature.

6.Current Status and Trends in Inflammatory Bowel Disease Surgery in Korea: Analysis of Data in a Nationwide Registry.

PURPOSE: Inflammatory bowel disease (IBD) in Korea has been increasing in recent years, but accurate statistics about operations for IBD are lacking. The purpose of this study was to investigate the trends and current status of IBD surgeries in Korea. METHODS: Using a national database from the Korea Health Insurance Review and Assessment Service, we analyzed data from patients who underwent surgery for Crohn disease and ulcerative colitis from January 2009 to October 2016. RESULTS: The mean number of patients who underwent surgery for Crohn disease was 791.8 per year. Colorectal surgery, small bowel surgery, and anal surgery were performed fairly often (31.2%, 29.4%, 39.4%, respectively), and laparoscopic surgery continued to increase, recently exceeding 30%. About 50% of Crohn patients used biologics before and after surgery, and those patients also underwent a relatively high rate of anal surgeries (44.2%). The mean number of patients who underwent surgery for ulcerative colitis was 247.6 per year. Colorectal surgery accounted for more than half of all operations, and laparoscopic surgery has been increasing rapidly, having been performed in about 60% of patients in recent years. The incidence of colorectal cancer in patients with ulcerative colitis was very high and increased rapidly during the study period, reaching about 80%. CONCLUSION: The number of patients undergoing laparoscopic surgery for IBD in Korea has increased significantly. Biologics are actively used by patients with Crohn disease, with a high proportion of anal surgeries required. Many of the surgical indications for ulcerative colitis have shifted into colorectal cancer.

7.Follow-up examination of atypical squamous cells of undetermined significance and low grade squamous intraepithelial lesion.

BACKGROUND: The New Bethesda System terminology has opened a series of problems about the atypical squamous cells of undetermined significance and low grade squamous intraepithelial lesion categories, particularly on their follow up. METHODS: We observe 12300 Pap smear examination, from jan 1995 to Dec 1999, in the Health promotion Center in a university hospital. Subjects were defined a the Health Promotion Center in a university hospital by electrical record and chart review. RESULTS: We find 48 cases of ASCUS and 33 cases of LSIL. When it is followed up by Pap smear, 34 cases of ASCUS are confirmed normal 19 cases(55.9%), benign cellular change 6 cases(17.6%), ASCUS 5 cases(14.7%), HSIL 4 cases(11.8%) and 27 cases of LSIL are confirmed normal 11 cases(40.7%), benign cellular change 3 cases(11.1%), ASCUS 3 cases(11.1%), LSIL 8 cases(29.6%), HSIL 2 cases(7.4%). 14 cases of ASCUS and 23 cases of LSIL are diagnosed by biopsy. Hostologic results of 14 cases of ASCUS are confirmed cervicitis 10 cases(71.4%), Moderate dysplasia 2 cases(14.3%), carcinoma in sute 2 cases(14.3%) and histologic results of 23 cases of LSIL are confirmed cervicitis 15 cases(65.2%), mild dysplasia 3 cases(13%), moderated dysplasia 3 cases(13%), sever dysplasia 1 cases(4.3%), carcinoma in situ 1 case(4.3%). Pap smear only was used for follow-up at family medicine clinic and 14 cases(29.2%), 4 cases(12.1%) are follow-up loss. CONCLUSION: After it is diagnosed ASCUS or LSIL Lesion of Pap smear in healthy care visitors, Follw-up loss is high in only follow-up Pap smear examination and follow-up results are presented more we severe lesions. To reduce follow-up loss, aggressive diagnosis and managements may be needed more than Pap smear follow-up examination.

9.Visual Analogue Scale in Acute Pain measurement: Its Usefulness as a Pain Measurement Tool in an Emergency Setting.

PURPOSE: The Visual Analogue Scale (VAS), as a pain measurement tool, has been shown to be a reliable measurement for chronic pain. However, the reliability and the validity of the VAS have not been demonstrated in an acute setting where pain fluctuations might be greater than they would be for chronic pain. This study assessed the usefulness of the VAS in an emergency setting. METHODS: Patients 16 years of age or older who presented with acute pain resulting from trauma or non-traumatic diseases were enrolled in this prospective, observational study. A 100-mm non-hatched, horizontal visual analogue scale was used to measure the pain severity. VAS measurements were obtained 1 minute apart at admission, 30 minutes after admission, and 1 hour after treatment. Intraclass correlation coefficients (ICCs) with 95% confidence intervals (95% CIs) and a Bland-Altman analysis were used to assess the reliability and the validity of the VAS measurements. RESULTS: The ICCs for paired VAS scores at admission, 30 minutes after admission, and 1 hour after treatment were 0.988 (95% CI=0.98 to 0.99), 0.968 (95% CI=0.95 to 0.98), and 0.989 (95% CI=0.98 to 0.99), respectively. The Bland-Altman analysis showed that 95% of the paired measurements at admission, 30 minutes after admission, and 1 hour after treatment were within 7 mm. CONCLUSION: The VAS appears to be a highly reproducible instrument for measurement of acute pain in the emergency department. This study suggests that the VAS is sufficiently reliable to be used to assess acute pain.

10.Hearing Outcomes and Prognostic Factors in Idiopathic Sudden Sensorineural Hearing Loss Patients with Combined Intratympanic and Systemic Steroid Therapy.

BACKGROUND AND OBJECTIVES: Hearing outcomes and prognostic factors of idiopathic sudden sensorineural hearing loss (SSNHL) were investigated in patients who underwent combined intratympanic and systemic steroid therapy. SUBJECTS AND METHOD: This study was performed by retrospective chart review. Clinical outcomes and prognostic factors were evaluated in 147 patients who received intratympanic steroid plus systemic steroid therapy. RESULTS: Complete hearing recovery was achieved in 36.7% (n=54) of the patients, partial recovery in 12.9% (n=19), slight recovery in 12.3% (n=18), and total recovery in 61.9% (n=91). Age was identified as an independent, negative prognostic factor for hearing recovery. The recovery rates of the down sloping and profound types were poorer than those of the up sloping type as determined by audiography. CONCLUSION: The results of this study suggest that the combined treatment of intratympanic and systemic steroids for idiopathic SSNHL results in high hearing recovery rates, and that the down-sloping and profound types of audiogram patterns and age are negative prognostic factors.
