Cardiac size of 291 new-born infants was measured using the method illustated on Fig 1. Among the 291 infants,53 were asphyxiated, and asphxia was only regared from Apgar score below 6 on 1 min. and 5 min. Remaining 238infants were normal, and classified to group with lung abnormalities and without lung abnormalities on chest A-Pfilm. The results are as follows; 1. The average CTR. of normal group was 52.37(C/T1;54.89, C/T2;49.43, C/T3;49.15, C/T;55.97) 2. The average CTR. of asphxiated group was 54.91 (C/T1;57.13, C/T2;51.69, C/T3;51.94,C/T4;58.25) 3. Consequently, asphyxiated infants revealed larger cardiac size than normal infant group.

4.Congenital coronary arteriovenous fistulae

Six cases of congenital coronary arteriovenous fistulae were diagnosed radiographically and confirmed afteroperation at Seoul National University Hospital in recent three and a quarter years since 1979. The clinical andradiological findings with the emphasis on the cinecardiographic findings were analysed. The summaries of theanalysis are as follows; 1. Among 6 cases, 2 cases were male and 4 cases were female. Age dsitribution was from 3months to 19 years. 2. In chest x-ray findings, 5 cases had cardiomegaly. Among these 5 cases, increase ofpulmonary vascularity was in 3 cases. Chest X-ray finding of the rest 1 case was within normal limit. 3. Thecoronary arteriovenous fistula orginated from the right coronary artery in 5 cases and from the single left coronary artery in 1 case. In 5 cases, a coronary arteriovenous fistula drained into the right ventricle and in 1case, a fistula from a SA node branch of the right coronary artery into the right atrium. 4. In 5 cases, a mainright coronary artery was dilated. Among these 5 cases, aneurysmal dilatation of main right coronary artery was in2 cases, saccular aneurysmal dilatation of distal portion of right coronary artery was in 2 cases and in 1 case,diffuse dilatation without aneurysm formation of a main right coronary artery. In 1 case, SA node branch of theright coronary artery was dilated with aneurysm formation. 5. For accurate diagnosis of congenital coronaryarteriovenous fistula, biplane cinecardiography is an essential procedure.

5.Comparison of Cotinine Excretion in Schizophrenics and Normal Controls.

OBJECTIVES: The prevalence of smoking in schizophrenic patients (74-92%) is higher than that of all psychiatric patients (34-54%) or general population (30-35%). This higher smoking Prevalence is demonstrated even after controlling for known confounders, such as marital status, alcohol use, and socioeconomic status. This study was conducted to determine whether there would be any difference in nicotine intake and metabolism between schizophrenics and normal controls. METHODS: Sixteen schizophrenic patients and sixteen normal controls were collected. All subjects were supplied with a pack of cigarette a day. Urinary cotinine excretion was measured by using gas chromatographic mass spectrometric method. RESULTS: Cotinine excretion was significantly increased in schizophrenic patients compared to normal controls (p<0.05). None of variables such as age at initial smoking, the average number of cigarettes at initial smoking, pack year (packs daily smoked x smoking year), abstinence history were found to influence cotinine levels when examined via the ANOVA, even when the interaction with diagnosis was considered. CONCLUSION: This result suggests that nicotine intake and consumption are increased in schizophrenic patients compared to normal controls, which can be an attempt to improve sensory inhibition and counteract neuronal effect of antipsychotic medications.

6.A Case Report of a 63 Year Old Lady With Coronary Arteriovenous Fistula Involving Left Coronary Artery and Draining Into Pulmonary Artery.

A 63 Year-old-lady has had substernal chest pain on exertion for 8 years. The chest pain has been increased over the last 3 years. A continuous murmur was heard at the left second to third inercostal spaces along the left sternal border. The electrocardiogram showed the inverted T-waves in the precordial leads. The right heart catheterization revealed 5% oxygen step up between RV and PA. The right sided pressures were normal. The coronary arteriography revealed markedly tortuous vessels starting shortly after the left main stem coronary artery was normally originated. One of the large vessel was shown to be drained into the pulmonary artery. In the operation room, without using heart-lung machine this abnormally drained vessel was simply ligated at it's draining site into pulmonary artery. After the operation the patient is feeling well with little symptoms and the continuous murmur is no longer heard.

Fourteen cases of double chambered right ventricle were diagnosed angiographically and of these nine caseswere confirmed after operation and autopsy at Seoul National University Hospital in recent four years since 1979.The clinical and radiological findings with the emphasis on the cinecardiographic findings were analysed. Thesummaries of the analysis are as follows; 1. Among 14 cases, 6 cases were male and 8 cases were female. Agedistribution was from 4 years to 36 years. 2. In chest X-ray findings, pulmonary vascularity was increased in 8cases, decreased in 4 cases, and nomral in 2 cases. Cardiomegaly was observed in 8 cases and other showed normalheart size. 3. In cinecardiography, 11 cases had interventricular septal defect. Among these 11 cases, VSD locatedin proximal high pressusre chamber was in 2 cases and located in distal low presssure chamber was in 9 cases. 4.The location of aberrant muscle bundle in sinus portion of right ventricule was in 8 cases. In the rest cases, andaberrant muscle bundle was located below the infundibulum of right ventricle. 5. For accurate diagnosis anddifferential diagnosis with other congenital cardiac anomalies such as Tetralogy of Fallot or isolated pulmonicstenosis, biplane cineangiography and catheterization is an essential procedure.

8.A radiologic analysis and comparative evaluation of hysterosalpingographic and laparoscopic findings ininfertile patients

Hysterosalpingograms of 375 patients with infertility were reviewed with reference to radiological features of each pathology and a comparative study of hysterosalpingography and laparoscopy in 36 patients was presented. The results were as follows; 1. In 375 cases infertility, 191 cases (50.9%) were primary infertility and 181 cases(49.1%) were secondary infertility. 2. The peak age distribution was 26 to 30 year old group in 211 cases (56.3%),and the peak duration of infertility was less than 3 years in 256 cases (68.3%). 3. Abnormal uterine findings were 115 cases (30.7%) and there were 73 cases of filling defects, 13 cases of irregularity , 12 cases of uterineflexion, 11 cases of congenital anomaly, and 9 cases of hypoplasia among them. 4. Abnormal tubal findings were 196 cases (52.3%) and there were 103 cases of simple obstruction, 69 cases of hydrosalpinx, and 50 cases of peritubal adhesion among them. 5. Intravasation of contrast media was noted in 42 cases (11.2%). 6. In 36 cases of laparoscopy, 23 cases (63.7%) was agreed to hysterosalpingography. 7. Aothough laparoscopy was better than hysterosalpinography in external anatomy of salpinx and relationship to overay, hysterosalpingography should be important for fundamental study of infertile women because of lower failure rate, lower risk of complications,therapeutic effect, and lower cost.

9.Milk Transfer and Toxicokinetics of Valproic Acid in Lactating Cynomolgus Monkeys.

Studies on milk transfer of drugs in non-human primates (NHPs) are among the crucial components in the assessment of peri- and postnatal toxicity because of the similarity between NHPs and humans. To evaluate the milk transfer of valproic acid (VPA) in NHPs, the toxicokinetics of VPA, an antiepileptic drug, were studied in pregnant cynomolgus monkeys. VPA was administered once daily to pregnant cynomolgus monkeys at doses of 0, 30, 90, and 270 mg/kg by oral gavage from Day 100 of gestation (GD 100) to Day 31 of lactation (LD 31). Concentrations of VPA and its metabolite, 4-ene-VPA, in the maternal plasma on GD 100, GD 140, and LD 30, and concentrations of VPA and 4-ene-VPA in the offspring plasma and milk on LDs 30 and 31, respectively, were quantified using liquid chromatography tandem mass spectrometry (LC/MS/MS). After administration of a single oral dose of VPA to pregnant monkeys on GD 100, the concentrations of VPA and 4-ene-VPA were generally quantifiable in the plasma of all treatment groups up to 24 hr after administration, which showed that VPA was absorbed and that the monkeys were systemically exposed to VPA and 4-ene-VPA. After administration of multiple doses of VPA to the monkeys, VPA was detected in the pup's plasma and in milk taken on LD 30 and LD 31, respectively, which showed that VPA was transferred via milk, and the pup was exposed to VPA. Further, the concentration of VPA in the milk increased with an increase in the dose. Extremely low concentrations of 4-ene VPA were detected in the milk and in the pup plasma. In conclusion, pregnant monkeys were exposed to VPA and 4-ene-VPA after oral administration of VPA at doses of 30, 90, and 270 mg/kg/day from GD 100 to LD 31. VPA was transferred via milk, and the VPA exposure to the pup increased with an increase in the dose of VPA. The metabolite, 4-ene VPA, was present in extremely low concentrations (< 0.5 microg/ml) in the milk and in the pup plasma. In this study, we established methods to confirm milk transfer in NHPs, such as mating and diagnosis of pregnancy by examining gestational sac with ultrasonography, collection of milk and pup plasma and determination of toxicokinetics, using cynomolgus monkeys.

10.Pathologic Scoring and Postoperative Liver Function in Canine Partial Liver Transplantation: after Desferrioxamine Exposure.

PURPOSE: Liver biopsy plays an important role in the histopathological evaluation of the transplanted liver, but till now pretransplant graft biopsy has limited role in predicting primary non function of the graft. Desferrioxamine (DFO), the iron chelating agent, has been known to be effective in reducing rat liver ischemia-reperfusion injury. We tried desferrioxamine in canine partial liver transplantation, and pathologic scores were compared. METHODS: ~70% partial liver was harvested and reimplanted in same mongrel dog weighing about 25 kg. Desferrioxamine (20 mg/kg) was infused via splenic vein just from the beginning of reperfusion of the partial liver graft (n=5). Serum aspartate aminotransferase (AST) Alkaline phosphatase (ALP), Lactate dehydrogenase (LDH) were checked and compared with the control group (n=5). Morphological liver injury score were compared to the control group. Statistical analysis was done with independent T-test. RESULTS: Total ischemic time was 4 hours and 42 minutes in average. AST level was significantly lower in Desferrioxamine group at 1 hour and 48 hours after reperfusion, (P=0.4) ALP level was significantly lower in desferrioxamine group at 48 hours after reperfusion (P=0.4). LDH level in desferrioxamine group was lower than that of control group but without statistical significance. The pathologic score at 1 hour after reperfusion showed a reduced degree of sinusoidal injury among the DFO group but the difference was not statistically significant. The pathologic score just before harvest of the graft showed no correlation with serum AST, ALP, LDH levels at that time or at 1 hour or 48 hours after reperfusion. Only the pathologic score at 1 hour after reperfusion had significant correlation with the serum LDH levels at 48 hours after reperfusion. CONCLUSION: In canine live donor partial liver transplantation, desferrioxamine infusion just before reperfusion might be an effective way of reducing ischemia-reperfusion injury. And the pathologic grading on samples obtained at 1 hour after reperfusion showed a significant correlation with subsequent liver function
