2.Prediction of response by FDG PET early during concurrent chemoradiotherapy for locally advanced non-small cell lung cancer.

PURPOSE: To evaluate the predictive value of the early response of 18F-flurodeoxyglucose positron emission tomography (FDG PET) during concurrent chemoradiotherapy (CCRT) for locally advanced non-small cell lung cancer (NSCLC). MATERIALS AND METHODS: FDG PET was performed before and during CCRT for 13 NSCLC patients. Maximum standardized uptake value (SUVmax), mean standardized uptake value (SUVmean), metabolic tumor volume (MTV), and total lesion glycolysis (TLG) were measured and the changes were calculated. These early metabolic changes were compared with the standard tumor response by computed tomograms (CT) one month after CCRT. RESULTS: One month after the completion of CCRT, 9 patients had partial response (PR) of tumor and 4 patients had stable disease. The percent changes of SUVmax (%DeltaSUVmax) were larger in responder group than in non-responder group (55.7% +/- 15.6% vs. 23.1% +/- 19.0%, p = 0.01). The percent changes of SUVmean (%DeltaSUVmean) were also larger in responder group than in non-responder group (54.4% +/- 15.9% vs. 22.3% +/- 23.0%, p = 0.01). The percent changes of MTV (%DeltaMTV) or TLG (%DeltaTLG) had no correlation with the tumor response after treatment. All the 7 patients (100%) with %DeltaSUVmax > or = 50% had PR, but only 2 out of 6 patients (33%) with %DeltaSUVmax < 50% had PR after CCRT (p = 0.009). Likewise, all the 6 patients (100%) with %DeltaSUVmean > or = 50% had PR, but only 3 out of 7 patients (43%) with %DeltaSUVmean < 50% had PR after CCRT (p = 0.026). CONCLUSION: The degree of metabolic changes measured by PET-CT during CCRT was predictive for NSCLC tumor response after CCRT.

3.Differntiation between Endobronchial Tuberculosis and Bronchogenic Carcinoma Associated with Atelectasis or Obstructive Pneumonitis: CT Evaluation.

PURPOSE: Endobronchial tuberculosis and bronchogenic cancer are common causes of atelectasis or obstructive pneumonitis in Korea. Differntiation between endobronchial tuberculosis and bronchogenic carcinoma is important for the treatment and prognosis but it is sometimes difficult to differentiate these two lesions with radiologic examinations. The purpose of this study was to find the differential points between endobronchial tuberculosis and bronchogenic carcinoma associated with atelectasis or obstructive pneumonitis. MATERIALS AND METHODS: Forty patients in whom atelectasis or obstructive pneumonitis was detected on chest radiographs comprised the study. A definite mass opacity was not observed on chest radiographs in all patients. In these patients, the causes of obstruction were endobronchial tuberculosis (n=20) and bronchogenic cancer (n=20) which were microbiologically or pathologically confirmed. RESULTS: Double obstructive lesions were more frequently found in endobronchial tuberculosis (8/20) than in bronchogenic cancer (1/20). Multiple calcifications along the bronchial wall and severe distortion of bronchi were observed only in endobronchial tuberculosis (4/20) and associated low density mass at obstruction site was only observed in bronchogenic cancer (6/20). Bronchial dilatation (11/20) and parenchymal calcifications (14/20) distal to obstruction site, air containing bronchogram at post obstructive bronchus (14/20) were more frequently found in endobronchial tuberculosis. Contour bulging at obstruction site (14/20), and only mucus bronchogram at post obstructive bronchus (14/20) were more frequently found in bronchogenic carcinoma. CONCLUSION: In patients with atelectasis or obstructive pneumonitis, endobronchial tuberculosis is characterized by double obstructive lesion, multiple calcifications at the bronchial wall, and severe distortion of the bronchi. Endobronchial carcinoma is characterized by a low density mass at the obstructive site.

5.The Effects of Community-Based Self-Help Management Program by Strengthening Self-Efficacy of Post Stroke Elderly Patients.

The purpose of this study was to determine the effects of self-help management program which strengthens self-efficacy for post stroke elders who attend day care center in Chongju. One group pretest-posttest design was used, and the subjects were 7 post stroke elders with hemiparesis. The self-help management program consisted of 5 sessions and each session included group education on stroke. ROM exercise. recreation and self-care strategies. Those strategies for encouraging and reinforcing self-efficacy provided verbal persuasion. performance accomplishment and vicarious experience. All subjects were questioned before and after the program about ADLs, IADLs. self-efficacy. self-care behavior and depression. The analysis of data revealed that the program was effective to increase the IADLs. self-efficacy and self-care behavior. However. there was no significant difference between pre and post ADLs and depression. The results indicated that this program may be adopted to improve the physical and psychological function of post stroke elderly patients in community settings.

The malignant fibrous histiocytoma (MFH) was first introduced in 1963 to refer to a group of soft-issue tumors characterized by a storiform or cartwheel-ike growth pattern and predominantly fibroblastic appearance. It was postulated that they were derived from histiocytes that could assume the appearance and function of fibroblasts. MFH has been recognized to be the most common sarcoma in the soft tissue of late adult life which involves the deep fascia or skeletal muscles of the extremities or retroperitoneum. It has been noted in other organs, however, it rarely arises in the liver. We report two cases of primary malignant fibrous histiocytoma of the liver histopathologically confirmed after surgical resection-storiform pleomorphic and myxoid subtypes in the 55 year old male and 49 year old female patients, respectively, and describes the clinical courses, characteristics and pathologic aspects with review of the related literatures.

10.Efficacy of Ciprofloxacin-soaked Collagen Shields for Treatment of Experimental Staphylococcus aureus Keratitis in Rabbits.

In order to study the efficacy of a 0.2% ciprofloxacin-soaked collagen shield for the treatment of Staphylococcus aureus keratitis, an experimental study was performed on 40 eyes of 20 rabbits. Twenty hours after intrastromal injection with S. aureus, the rabbits were divided into four groups: Group 1(10 eyes) was treated with a 0.2% ciprofloxacin-soaked collagen shield with an additional instillation of 0.2% ciprofloxacin drops every 30 minutes for 4 hours; Group 2 (10 eyes) was treated with a 0.2% ciprofloxacin-soaked collagen shield for 4 hours; Group 3 (10 eyes) was treated with 0.2% ciprofloxacin drops every 30 minutes for 4 hours; Group 4 (10 eyes) received BSS every 30 minutes for 4 hours as a control. Bacterial killing was quantitated by culturing corneal homogenates and calculating the number of viable bacteria (colony-forming units) per cornea. Groups 1, 2, and 3 showed significantly reduced numbers of bacteria compared with the control (p<0.01). The order of efficacy in the treatment for S. aureus keratritis were groups 1, 2 and 3, respectively, but the statistical difference was not significant among groups 1, 2, and 3(p>0.05). These results suggested that a 0.2% ciprofloxacin-soaked collagen shield may be an effective and convenient mode of therapy for S. aureus keratitis.
