1.Clinical observation of traumatic ankle arthritis with orthopedics lotion on 60 cases.

Observing the clinical curative effect of orthopedics lotion to treat traumatic ankle arthritis, the outpatient department of orthopedics in Guang'anmen Hospital collected 60 cases who were diagnosed as traumatic ankle arthritis. The cases who already met the inclusion criteria, were randomly divided into the treatment group (30 cases) and control group (30 cases). Thirty patients in treatment group were received fumigation treatment with orthopedics lotion; 30 patients in control group were treated by intra-articular injection of sodium hyaluronate. After 5 weeks treatment, the effects on the both groups would be observed and analysed. Baird-Jackson scoring system was used to assess the overall curative effect. Visual analogue scales (VAS) was used to assess analgesic effect. BJ scores of pre-treatment and post-treatment in both the treatment group and the control group were compared, P <0O. 01, significantly different; and the BJ scores after treatment of two groups were compared by paired t-test, the t value is 1. 76, there was no significant difference between two groups. Both of the treatments were effective, but there was no significant difference in overall curative effect between the two kinds of treatment. The VAS scores after treatment of two groups were compared by paired t-test, the t value was 2. 282, and there was a significant difference between two groups (P <0. 05). The analgesic effect shown in treatment group was better than the control group. The results suggest that: orthopedics lotion is a effective way to treat traumatic ankle arthritis, and it has significant effect on analgesic.

2.Effects of platelet-derived growth factor, basic fibroblast growth factor and epidermal growth factor on the proliferation, migration and phenotypic modulation of airway smooth muscle cells

Objective To investigate the effects of platelet-derived growth factor(POGF),basic fibroblast growth factor(bFGF)and epidermal growth factor(EGF)on the proliferation,migration and phenotypie modulation of airway smooth muscle cells(ASMCs)in vitro.Methods Cultured human ASMCs were stimulated with PDGF.bFGF,EGF or DMEM(control).Cell pmliferation was detected by BrdU incorporation and cell count.Cell migration was measured using Boyden's chamber.Reverse transcription-poly-merase chain reaction(RT-PCR)and Western blot analysis were used for the detection of sm-α-actin and sm-MHC mRNA and protein expression in ASMCs.Resuits The mitotic index(%BrdU-positive per total number of cells)of ASMCs stimulated with PDGF.bFGF and EGF were significantly higher than those in control(P<0.001 or P<0.05).And the cell number of ASMCs stimulated with PDGF,bFGF and EGFwere also significantly higher than those in control(P<0.05).The number of migrating cells treated with PDGF,bFGF and EGF were higher than that in eontrol(P<0.05).The expressions of sm-α-actin mRNA treated with bFGF and EGF were significantly lower than those in control.but it was higher than control when treated with PDGF.The expressions of sm-MHC mRNA treated with bFGF and EGF were significantly lower than those in control,but it was higher than control when treated with PDGF(P<0.01 orP<0.05).Theexpressions of sm-α-actin protein treated with bFGF and EGF were significantly lower than those in control,but it was higher than control when treated with PDGF(P<0.05).The expressions of sm-MHC protein treated with bFGF and EGF were significantly lower than those in control.but it was higher than control when treated with PDGF(P<0.05).Conclusion PDGF,bFGF and EGF can directly induce ASMCs,proliferation and migration in vitro;and meanwhile,the expressions of cells,contractive phenotype increased treated with PDGF and decreased treated with bFGF and EGF.

In this paper,the development trend of medical monitor system is analyzed.The portable trend and network function become more and more popular among all kinds of medical monitor devices.The architecture of medical network monitor system is provided: pocket medical monitor devices based on embedded platform is developed to interact with center database on networks;the telecommunication interface based on 802.11b/g and CDMA is designed to rebuild and extend the functions of traditional imaging devices(ultrasonic?CT?MRI) and the implementation details of terminal,monitor center software,distributed database and two kinds of medical information terminal are especially discussed;distributed medical database is designed for hospital center according to DICOM information model and HL7 standard;hand terminal based on WINCE is developed for nurse's routine care and doctor's auxiliary diagnosis.
5.Research of Automatic QT Interval Detection Based on Morphological Method

Objective To study and design a new algorithm of QT interval measurement based on multi-scale morphological derivative transform(MMDT) according to the demands of mobile monitoring platform.Methods After the MMDT of ECG signals by inducing a triangle and a short line as a pair of structure elements,they were classified into four types by their morphological characteristics and the optimal strategy for each type was described to detect the onset of Q-wave.By introducing the "wing" function,two referenced points of T peak were picked out,which was helpful to locate the peak and offset of T wave,and improve the precision and recognition rate of MMDT method when detecting the bifid T wave(90.9%)and biphasic T wave(86.7%).Results Eighty data records from CSE database were used to evaluate the availability.By contrast with the wavelet transform method,the statistical results showed that the proposed algorithm had generally less error and smaller standard deviation especially for abnormal-phase T wave.Conclusion Compared with those algorithms based on wavelet transform and self-adaptive threshold techniques,our algorithm needs less empirical parameters and calculation.It is also suitable for mobile monitoring and HOLTER system,and has a wide prospect of application.

Pulmonary edema frequently occurs after severe burn. It not only impairs pulmonary function directly, but also can induce or exacerbate other pulmonary complications such as lung infection, acute lung injury ( ALI), and ARDS. Extravascular lung water (EVLW) is closely related to the pulmonary edema. Dynamical monitor of EVLW has been used to predict and quantify the degree of pulmonary edema clinically. This review focuses on the recent progresses at home and abroad on the formation mechanism, monitoring approach, and prevention and treatment of EVLW after severe burn injury.

9.Relationship between different degrees of asymptomatic carotid artery stenosis and cognitive impairment

Objective To investigate relationship between different degrees of asymptomatic carotid artery stenosis and cognitive impairment. Methods The cognitive function were evaluated with the montreal cognitive assessment scale (MoCA) in 80 patients with asymptomatic carotid artery stenosis and 56 controls, the total MoCA scores and subprojects scores were compared between the patients with different degrees of ACS and controls. Results The total MoCA score, visual space and executive function, attention and delayed recall had significant difference among the mild, moderate, severe ACS patients and control group (all P < 0.05). There was no statistical significance of the total MoCA score and the scores of all subprojects between the patients of mild ACS patients and the control group (all P > 0.05). The total MoCA score and the scores of visual space and executive function , attention and delayed recall in moderate and severe ACS patients were significantly lower than the control group and mild ACS patients (P < 0.05). Furthermore, the severe ACS patients, compared with the moderate ACS patients , performed worse on the cognitive tasks which involved visual space and executive function, attention and delayed recall (P < 0.05). Conclusions Cognitive function is impaired in the patients with moderate and severe ACS,the neuropsychological characteristics are mainly impaired in visual space and executive function, attention and delayed recall.
10.Short-term curative effect of vacuum sealing drainage (VSD) combined with platelet rich plasma (PRP) for the treatment of the refractory wounds.

OBJECTIVETo observe the efficacy of the treatment of refractory wound by VSD combined with platelet-rich plasma.
METHODSFrom April 2010 to June 2012,15 patients with refractory wound were treated including 11 males and 4 females with an average age of 35.2 years old ranging from 18 to 45 years old. The formation time of wound was from 6 to 24 months, which was unhealed after long-term medication or repeated treatment. The VSD combined with autologous platelet-rich plasma was applied to treat the wound. The wound healing was an indicator and treatment and clinical features were summarized.
RESULTSAll the patients were followed up for 3 to 8 months (means 5 months). The wound of all patients were healed without recurrence.
CONCLUSIONThe curative effect of VSD combined with platelet rich plasma for treatment of refractory wounds is obvious. It could reduce the treatment course and the treatment cost.
