3.Changes of islet ? cell function in patients with diabetic ketosis and ketoacidosis
Chinese Journal of Diabetes 1995;0(04):-
The islet secretion function in 42 clinically diagnosed diabetic ketosis and ketoacidosis pa- tients, aged 25~39 year-old,were studied. The results showed that the acute insulin responses to L-ARG (AIRarg) was significantly reduced in patients compared to control. As disease prolonged,AIRarg de- creased gradually. In the patients less than three months, the acute insulin responses to glucose(AIRglu) were markedly reduced, although the AIRarg were still sustained to some extent. The fasting blood insulin levels,in the patients more than three months were not different from controls, but C-peptide levels were significantly lower than those of controls. However, the C-peptide responses to L-ARG stimulation were not as evident as the insulin responses. The results suggested that impairment of the insulin secretion exist of in the early stage of DM,and exaggerated as the disease prolonged.Terapy of this type of diabetes mellitus de- pended mainly on insulin.

China Pharmacist 2015;18(10):1760-1762
Objective:To discuss the therapeutic effect of pharmaceutical care for old patients with chronic obstructive pulmonary disease ( COPD) . Methods:Totally 192 patients diagnosed as COPD were randomly divided into 2 groups according to a random num-ber table, 100 cases in the experimental group and 92 cases in the control group. The control group was treated with the conditional therapy, and the experimental group was treated with pharmaceutical care additionally. The studieds on COPD assessment test ( CAT) , modified British medical research council ( mMRC) and the value of pulmonary function index ( FEV1% index) were carried out and compared between the two groups. Results:There was no statistically significant difference in the CAT score, mMRC classification and FEV1% index between the two groups on admission (P>0. 05), however, after the treatment, the CAT score and mMRC classifica-tion were decreased and FEV1% index was increased in the two groups, and there was statistical significance between them ( P <0. 05). The CAT score and mMRC classification in the experimental group were lower and FEV1% index was higher than that in the control group(P<0.05). Compared with the medication errors(n=13, 14.13%) and incidence of adverse drug reactions(n=5, 5. 43%) in the control group, the medication errors(n=2, 2. 0%) and incidence of adverse drug reactions (0%) in the experimental group were significantly decreased. Conclusion:Clinical pharmacists provide effectively pharmaceutical care, which can significantly improve the clinical therapeutic efficacy in old patients with COPD.

OBJECTIVE To analyze 1000 Hz probe tone tympanometry of middle ear with different volume of fluid, and to evaluate the role of 1000 Hz probe tone tympanometry in the diagnosis of presence of fluid in tympanic cavity. METHODS Tympanometries with 1000 Hz or 226 Hz probe tone were obtained from all the guinea pigs with different volume of fluid in tympanic cavity using GSI33 analyzer and tympanometry curves were analyzed. RESULTS There was a significant difference of the peak pressure of 1000 Hz tympanometry among all test groups and control group. The peak pressure decreased with the increasing of fluid volume in tympanic cavity. CONCLUSION Tympanometry with 1000 Hz probe tone can be used in the diagnosis of fluid in tympanic cavity, and it can be an indicator of volume of fluid in tympanic cavity.

Objective To observe the expression of both type 1 (AT1) and type 2 (AT2) receptors of angiotensin Ⅱ (AngⅡ) in human hypertrophic scars, and to explore theirs role in collagen synthesis of fibroblasts in human hypertrophic scars. Methods The expression of both AT1 and AT2 receptors in fibroblasts of hypertrophic scars was detected with immunohistochemical staining and radioligand receptor binding assay. Collagen synthesis was examined in cultured in vitro fibroblasts of hypertrophic scars by measuring [~3H]proline incorporation into collagenous proteins. Results Positive staining signals of both AT1 and AT2 receptors were found in fibroblasts of hypertrophic scars. Similar results were also got in cultured in vitro fibroblasts of hypertrophic scars, expression level of AT1 and AT2 receptors were 10.69?2.15fmol/10~6cells and 4.9?1.05fmol/10~6cells respectively. In cultured in vitro fibroblasts, AngⅡ may accelerate the collagen synthesis significantly (P

Chinese Journal of Postgraduates of Medicine 2016;39(6):561-564
Objective To study the optimal dose of esmolol for maintaining cardiovascular stability in patients with hypertension during tracheal extubation. Methods In post-anestheisa care unit, hypertensive patients after general anesthesia meeting the extubation criteria were included. Patients were divided into 2 groups according the age: group Ⅰ (>65 years old for the elderly hypertensive, 21 cases), and groupⅡ(≤65 years old for the non-elderly hypertensive, 22 cases). All the patients received esmolol bolus before sputum suction and tube extraction, and the tracheal extubation were extubated 2 minutes after esmolol bolus. The systolic blood pressure, diastolic blood pressure and heart rate were was recorded before tracheal extubation, 2 min after esmolol bolus, at the time of sputum suction extubation, 1 min after tracheal extubation, 3 min after tracheal extubation and 5 min after tracheal extubation. Esmolol dose was determined by the up and down method. Initial dose was 0.5 mg/kg, in accordance with the arithmetic dose (0.2 mg/kg) increasing or decreasing progressively. In negative results (the systolic blood pressure at extubation or 5 min after extubation ≥ 20% of the base, or the systolic blood pressure at sputum suction extubation>180 mmHg, 1 mmHg=0.133 kPa) esmolol dose increased progressively, and in positive results (the systolic blood pressure at extubation or 5 min after extubation<20%of the base) esmolol dose decreased progressively. When the crosspoint (from positive to negative result) reached 6, the study was terminated. Results The median effective doses of esmolol for maintaining cardiovascular stability in groupⅠand groupⅡwere (0.6 ± 0.1) and (0.8 ± 0.1) mg/kg. Conclusions Esmolol can maintain cardiovascular stability in patients with hypertension during tracheal extubation. Median effective dose decreases in older hypertensive patients.
