2.CT,MR and MRS Diagnosis of Basal Ganglia Infarction

Objective To discuss imaging differentiating diagnosis of basal ganglia infarction by summarizing the features of 17 lesions on CT and MRI.Methods Among 17 cases, 6 were children occurred after their injury, and ranged in age from 5 to 11 years old with average 8 years old. Others were adults ranged in age from 53 to 78 years old with average 63 years old. All patients were examined with CT plain scans. 11 cases were underwent MRI and 5 were underwent 1 H MRS.Results 6 lesions in children were all single lateral cases. Among 11 adult lesions, 10 were single lateral and 1 was bilateral case. Basal ganglia infarctions demonstrated hyposignal on T_1WI, hypersignal on T_2WI and hypodense on CT. The lateral ventricles' anterior horns of the same side were pressed and narrow. On 1 H MRS, Lac was obviously observed in all lesions. NAA decreased markedly in 4 lesions and slightly in 1 lesion. 2 basal ganglia infarctions were together with hemorrhage.Conclusion Basal ganglia infarctions were mainly observed in the elder with arteriosclerosis and injured children. Its features on CT and MRI are characteristic, but it is in need of differentiating from basal ganglia lesions caused by other reasons. Marked Lac on 1 H MRS is its character.
3.Effect of erythropoietin on expression of myocardial NADPH oxidase in pressure overload rats

AIM:To explore the effect of erythropoietin ( EPO) on the expression of myocardial NADPH oxi-dase (Nox) in the pressure overload rats.METHODS:Male SD rats (n=36) were used to establish a pressure overload myocardial hypertrophy model by abdominal aorta ligation.The animals were divided into model group, control group ( sham, without narrowing abdominal aorta, the rest of the operation was the same as the model) and recombinant human erythropoietin ( rhEPO) treatment group ( intraperitoneal injection of rhEPO postoperatively, 4 000 U/kg, twice a week) . After 8 weeks, the cardiac ultrasound imaging and hemodynamic evaluation were conducted to determine the cardiac func-tions.Masson staining was used to observe the degree of myocardial fibrosis.The expression of Nox2 and Nox4 at mRNA and protein levels was detected by real-time quantitative PCR and Western blotting.The protein levels of myocardial inflam-matory factors CD45, F4/80 and TGF-βwere determined by Western blotting.RESULTS:Compared with model group, the left ventricular ejection fraction (LVEF), left ventricular fractional shortening (LVFS), left ventricular end-systolic pressure (LVESP) and left ventricular pressure maximum rising and falling rates ( ±dp/dtmax) increased significantly in EPO treatment group (P<0.01).At the same time, left ventricular end-systolic diameter (LVESD), left ventricular end-diastolic diameter ( LVEDD) and left ventricular end-diastolic pressure ( LVEDP) were decreased in EPO treatment group (P<0.01).EPO reduced the degree of myocardial fibrosis caused by pressure overload (P<0.01) and decreased the ex-pression of Nox2 and Nox4 at mRNA and protein levels in the myocardium (P<0.05 or P<0.01), and reduced the pro-tein expression of myocardial inflammatory factors CD45, F4/80 and TGF-β.CONCLUSION: EPO inhibits rat myocar-dial fibrosis induced by pressure overload, improves heart functions by decreasing NADPH oxidase activity and inhibiting myocardial oxidative stress levels and myocardial inflammatory reaction.

7.Effects of Different Doses of Oxycodone Hydrochloride on Spontaneous Breathing and Consciousness Level of Patients

Objective To assess the effects of different doses of oxycodone hydrochloride on spontaneous breathing and consciousness level of patients,so as to provide theoretical basis for its clinical application.Sixty patients undergoing elective surgery were randomly divided into 3 groups:0.05 mg · kg-1 oxycodone group (group P1),0.1 mg· kg-1 oxycodone group (group P2),0.2 mg· kg-1 oxycodone group (group P3).Changes of respiratory rate (RR),end tidal carbon dioxide partial pressure (PETCO2),saturation of blood oxygen (SpO2) and bispectral index (BIS) were recorded in patients before injection (t0) and 1-15 min after injection (once per min);the Observer's Assessment of Alertness/Sedation Scale (OAA/S) were recorded.At the same time,the adverse reactions were observed after drug injection in each group.Results In 10 min after injection there were no significant differences in the RR,SpO2,PETCO2,BIS and OAA/S in group P1 as compared with those before injection (P>0.05).The patients had no respiratory depression in group P1.In group P2,RR had a significant decrease (P<0.05),BIS had decreased but were greater than 85;there were no significant differences in the SpO2,PETCO2 and OAA/S (P>0.05).The P2 group had 3 cases with respiratory frequency<10 per min,but SpO2 were all greater than 94%.In group P3,There were significant differences in the RR,SpO2,PETCO2,BIS and OAA/S (P<0.05).The P3 group had 12 cases of respiratory frequency <10 per min,at the same time there were 8 patients with SpO2 less than 94%.With the increasing dose,the frequency of respiratory inhibition increased,and there were statistical differences (P< 0.05).In 15 min after injection,RR,SpO2,PETCO2,BIS and OAA/S were not significantly different in group P1 and P2 as compared with those before injection (P>0.05).In group P3,RR was significantly different after injection (P<0.05).There were no significant differences in the SpO2,PETCO2,BIS and OAA/S in group P3 after injection (P>0.05).No patients complained with chest wall stiffness,nausea and vomiting,cough and other adverse reactions in group P 1 and P2.In group P3,three patients had nausea 5 min after injection,two patients complained of chest skin itching but no skin flushing.Conclusion With the increasing dose,effect of oxycodone hydrochloride on breathing and consciousness level of patients gradually increased.Injection of oxycodone hydrochloride 0.05 mg · kg-1 had no obvious effect on breathing and consciousness.After injection of oxycodone hydrochloride 0.1 and 0.2 mg · kg-1 for 5 to 10 min,respiration inhibition and sedative effect were the most obvious.Fifteen min after injection,the 0.1 mg · kg-1 dose group recovered to the level before,the respiratory rate of the 0.2 mg · kg-1 dose group was still lower than that before the injection.
8.Research status of radiomics in cardiovascular diseases diagnosis using nuclear medicine

Radiomics has become an important means of medical imaging diagnosis. Cardiovascular diseases seriously threaten human life, and nuclear medicine has shown its unique advantages in the diagnosis of cardiovascular diseases. In this field, relevant studies on the combination of nuclear medicine and radiomics have achieved some results. The combination of two technologies has great application prospects, but there are also some challenges. This paper reviews the basic principles of radiomics, the advantages and clinical applications of nuclear medicine in the diagnosis of cardiovascular diseases, the current research status, challenges and foreground of radiomics combined with nuclear medicine in the diagnosis of cardiovascular diseases, in order to provide some useful thoughts for further research.
9.Application of Case-based Learning in Ward-round for Residents Training

Objective To observe the effect of case-based learning (CBL) applied in ward-round on residents training. Methods A total of 33 residents trained with routine ward-round and CBL ward-round in the neurological department were investigated with questionnaire. Results 30 effective answers were recalled. More of them reported that CBL ward-round was very significant in improving clinical thinking, mastering the specialized knowledge, developing autonomous learning and improving the communication with patients and their family members (P<0.05). Conclusion CBL ward-round is effective on improving the comprehensive professional abilities of residents.
10.Histological pathological changes in articular cartilages after firearm injury

Objective To investigate the pathological change in articular cartilages after firearm injury.Methods Four rabbits from 28 New Zealand healthy rabbits were chosen as control group and subjected to joint capsule incision only. Another 24 rabbits were equally divided into 6 experimental groups( groups B to G) and subjected to medial femoral condyle cartilage surface damage by the nail gun.After the operation, their specimens were collected after 6 h,3 d,7 d,14 d,28 d and 56 d, respectively.Tissue sections were observed and stained by HE staining and toluidine blue staining.The histolopathological changes in articular cartilage after firearm injury were detected.Results The color of articular cartilages in experimental groups became lighter, the cell number increased but then decreased, the articular cartilage layer disappeared, the cell shape became uneven, cells began to cluster and the Mankin score increased, and the statistical differences between experimental groups and control group were significant.Conclusion The histological pathological changes in articular cartilages after fiream injury seem to follow some pattern.The degeneration seems obvious after 7 days and then becomes heavier.
