1.Influencing factors for pediatric nurses' reporting of adverse events

Objective To identify the influencing factors for pediatric nursese' reporting of adverse events at 3A hospitals in Beijing. Methods Adopting the random sampling of ten 3A hospitals with pediatric wards in Beijing and conducting an questionnaire survey of some nurses in those hospitals. Results The possibility of adverse events reporting increased with the awareness of adverse events which was related with their severity. Except for title, educational background, length of service, position and past experiences were not associated with the barriers. The major perceived barrier was fear of colleague relationship brokenup and some other undesirable consequence. Conclusions Better training of awareness of adverse events and changes of the punitive culture by the roots as well as some regulation or policy were the major solutions to improve incident reporting.
3.The influence of labor analgesia on maternal T-lymphocyte subsets

Objective To observe the subarachnoid space the epidural space joint block labor analgesia (CSEA) changes of T lymphocyte subsets.Methods 100 cases of full-term mothers were randomly divided into two groups,analgesia group and the control group.The analgesic group row CSEA childbirth analgesia.Two groups of maternal cervix large 2 to 3cm (T1),when the baby was delivered (T2),24h (T3),blood was colleoted after childbirth,detected by radioimmunoassay of cortisol.Flow cytometry assay was wsed to test T lymphocyte subsets CD3+,CD4+,CD8+,CD4+/CD8+,And the birth process,Apgar score,Labor pain visual analogue score (VAS) were recoroded.Results (1) Active phase of the first stage of childbirth analgesia group was significantly shorter than the control group (t =2.182,P < 0.05).The second and third stage of Childbirth and neonatal Apgar score difference between the two groups was not statistically significant.(2)Serum cortisol concentration at T2 analgesia group was significantly lower that of than the control group (P < 0.05).(3) The two groups in maternal blood of CD3+,CD4+,CD4+ / CD8+ were reduced to varying degrees,T3 was reduced significantly (P < 0.05),Analgesia group was significantly lower than that of the control group (all P < 0.05).Conclusion CSEA analgesia in Childbirth can shorten the first stage of Childbirth active period,has no significant effect on the Apgar score,can alleviate the pain and stress reactions such as the suppression of the maternal immune function.
4.The clinical observation of neoadjuvant chemotherapy with TAC scheme in breast cancer

Objective To investigate clinical effects of neoadjuvant chemotherapy of TAC scheme in breast cancer.Methods 82 patients with locally advanced breast cancer receiving neoadjuvant TAC chemotherapy program were retrospectively analyzed.The total efficiency,toxicity,and recent surgical cases were evaluated.Results There were 21 cases of CR in 82 cases with breast cancer,accounting for 25.6%,47 PR(57.3 %),14 SD(17.1%),so the clinical total effective rate was 82.9%.The major toxic reactions was bone marrow suppression and neutropenia occurrence rate was 81.7%.Other adverse effects include gastrointestinal symptoms and liver dysfunction,and alopecia.All patients after symptomatic treatment can be tolerated.Conclusion TAC program neoadjuvant chemotherapy for locally advanced breast cancer patients can improve the cure rate,increase breast-conserving opportunities.
6.Research on the Treatment of Depression with Sleep Disorders by Paroxetine Combined with Olanzapine

Objective:To observe the clinical efficacy and adverse reactions of paroxetine combined with olanzapine in the treat-ment of depression complicated with sleep disorders. Methods: Totally 150 cases of sleep disorder patients were selected, and then randomly divided into the control group and the observation group with 75 ones in each. The control group was given paroxetine treat-ment, the observation group was given paroxetine combined with olanzapine treatment, and the treatment course was four weeks. The sleep latency, total sleep time, wake-up time and the number of awakening in the two groups were recorded, and the HAMD scale score and treatment efficacy were compared. Results:Compared with those in the control group, the sleep latency, total sleep time, wake-up time and the number of awakening in the observation group were obviously improved (P<0. 05). The clinical cure rate, the HAMD scale score and the incidence of adverse reactions between the two groups all had significant differences (P<0. 05). Conclu-sion:Paroxetine combined with olanzapine in the treatment of depression with sleep disorders shows fast efficacy, which can improve the sleep disorder symptoms and reduce the incidence of adverse reactions significantly.
7.Regulatory study of LBP on the expression of T cells apoptosis-related genes in aged rats

Objective:To investigate the regulatory mechanism of LBP on T lymphocytes excessive apoptosis and the mRNA expression of apoptotic genes in aged rats Methods:T lymphocyte apoptosis and the expression of pre apoptotic and anti apoptotic genes (Fas, FasL, TNFR1, Bax, Bcl2 and TNFR2) from young rats and old rats treated with or without LBP were studied using flow cytometry and fluorescence real time quantitative RT PCR Results:LBP could effectively downregulate the percentage of T cell apoptosis in aged rats At the same time LBP also downregulated the transcription level of TNFR1 gene and upregulated the transcription level of Bcl 2 in T cells from old rats Conclusion:The T cell excessive apoptosis in old rats could be effectively downregulated with LBP, by the by of decreasing the expression of pro apoptotic gene and enhancing the expression of anti apoptotic gene

9.Clinical significance of increased plasma IGF-1 in type 2 diabetic patients with malignant tumor

Objective To test plasma insulin‐like growth factor 1(IGF‐1) level in type 2 diabetic patients with malignant tumor , and make comparison with diabetic patients and normal patients .To discuss the significance of detection of plasma IGF‐1 level in type 2 diabetes in screening early malignant tumor .Methods Plasma IGF‐1 level were determined in type 2 diabetes among malig‐nant tumor group ,type 2 diabetes ,and normal control group ,and statistical comparison was made between the three groups .Results the plasma levels of IGF‐1 of type 2 diabetes mellitus ,type 2 diabetes mellitus with malignant tumor were significantly lower than normal group(P<0 .05) .And the plasma level of IGF‐1 in type 2 diabetes mellitus with malignant tumor group was obviously high‐er than that of type 2 diabetic group(P<0 .05) .Plasma IGF‐1 ,FBG ,HbA1c and 2 HBG level are risk factors for tumor .The level of FIns is a protective factor for tumor .Conclusion Type 2 diabetes is closely related to the malignant tumor ,and the plasma levels of IGF‐1 is a risk factor for tumor .
10.The study of using minimally invasive surgery(MIS) to treat the Ribs-tumor under the scapula

Objective The purpose of this study is to investigate the surgical procedures and curative effect through a small incision on both sides of the scapula for surgical resection the ribs tumor which located un -der ventral surface of the scapula .Methods The group included 11 patients,6 males and 5 females,aged 18 to 49 years.Lesions were bilateral scapula deep square 2~4 single rib bone tumors .Among them there were five ca-ses of fibrous dysplasia ,case of bone cyst ,2 cases of plasma cell tumors and metastatic tumors in 3 cases.Surgery incision through cross-scapular ,to determine which lesions ribs ,cutting the lesions on both sides of the stripping and junction.Results The group obtained a satisfactory excision extension ,one case appeared hemopneumotho-rax postoperatively.Secondly,none of patients occured local recurrence .Thirdly,two patients with metastatic carci-noma died after one year because of multiple metastases .Conclusion Straddling scapular notch is the ideal way to cut off the rib tumor of ventral surface of the scapula .Possessing advantages of a little damage and effect on the function,and rapid recovery .
