1.Clinical and Biological Features of Interval Colorectal Cancer.

Interval colorectal cancer (I-CRC) is defined as a CRC diagnosed within 60 months after a negative colonoscopy, taking into account that 5 years is the “mean sojourn time.” It is important to prevent the development of interval cancer. The development of interval colon cancer is associated with female sex, old age, family history of CRC, comorbidities, diverticulosis, and the skill of the endoscopist. During carcinogenesis, sessile serrated adenomas/polyps (SSA/Ps) share many genomic and colonic site characteristics with I-CRCs. The clinical and biological features of I-CRC should be elucidated to prevent the development of interval colon cancer.

2.Codon Usage Bias of Human Cytomegalovirus Genes with Different Evolutionary Conservancy.

Human cytomegalovirus (HCMV) is a member of beta-herpesvirus and contains a double-stranded genome with longer than 230 Kbp. HCMV infection of human is mostly asymptomatic, but often causes fatal diseases in immunocompromised people. In this study, codon usages of HCMV genes were analyzed and attempted to correlate with evolutionary conservancy. Core genes are the most conserved genes common among herpesvirus family, beta-herpes genes are common to beta-herpesviruses, and CMV genes are the least conserved found only in CMVs. Core genes had higher codon adaptation index (CAI) and GC content of silent 3rd codon position (GC3s) values and lower effective number of codons (Nc) and Nc/GC3s values than CMV genes. The average length of core genes was statistically longer than CMV genes, and core genes were found to be less varied than CMV genes. beta-herpes genes could be placed between core and CMV genes. Higher CAI and GC3s values along with lower Nc and Nc/GC3s values are suggestive of higher codon usage bias and more adaptation to host cells. Thus it is concluded that core genes of HCMV are more biased in codon usage and adapted to host cells compared to CMV genes.

3.The Role of Noncoding Region in Hantaan Viral S Genome for Expression of Nucleocapsid Protein.

The genome of Hantaan virus, the prototype of the hantavirus genus, is composed of three segmented, single stranded negative sense RNA genome. The 5' and 3' termini of the Hantaan virus RNA genome contain noncoding regions (NCRs) that are highly conserved and complementary to form panhandle stuctures. There are some reports that these NCRs seems to control gene expression and viral replication in influenza virus and vesicular stomatitis virus. In this study, we examined whether NCRs in Hantaan virus play a role in expression of the viral nucleocapsid protein (Np) and foreign (luciferase) gene. The 5' and/or 3' NCR-deleted mutants were constructed and analysed. The Np expression of 5' NCR-deleted clone, it showed 40% reduction. To investigate the role of NCR in foreign gene expression, the clones which are replaced ORF of Hantaan viral Np gene with that of luciferase gene were constructed. The results were similar to those of the experiments using Np gene. These results suggest that 3' NCR is more important than 5' NCR in protein expression. To find out a critical region of 3' NCR in more important than 5' NCR in protein expression. To find out a critical region of 3' NCR in protein expression, several clones with a deleted part of 3' NCR were constructed and analyzed. The deletion of the conserved region in 3' NCR showed 20~30% decrease in Np expression. However there were no change in luciferase activities between clones with or without non-conserved region of 3' NCR. These results suggest that the 3' NCR of Hantaan virus S genome, especially conserved region in 3' NCR, plays and important role in the expression of Hantaan viral Np and foreign genes.

4.Transverse uterine incision closure: One versus Two layers.

OBJECTIVE: Closure of a low transverse cesarean incision with one layer and two layer, we compared operative time, hemostasis, blood loss and postoperative sonohysteroperitoneographic findings. Study design: At our hospital 103 women were randomized to closure of a low transverse cesarean incision with either one continuous layer of a locking No. 1 chromic suture or two continuous of No. 1 chromic suture with the first layer locked between Mar. 1, 1998 and Dec. 31, 1998. After three months later, sonhysteroperitoneography was taken and then we evaluated uterus indirectly. RESULTS: A one layer closure required less operative time, 16 versus 20.8 minutes (p<0.01), less hemoglobin change, 0.44 versus 1.3 (p<0.01) and less suture materials (p<0.01). But postoperative follow up sonohysteroperitoneography, a one layer closure shows slightly thinning of isthmic layer of myometrium, two layer closure shows more adhesional band and both are similar other findings. CONCLUSION: A one layer does not significantly affect the clinical course than traditional two layer closure. So we recommended a one layer closure when its use is anatomically feasible.

5.A Case Report of Fatal Mediastinal Abscess Secondary to Odontogenic Infection.

Modern antibiotic therapy and rapid surgical intervention have greately reduced the complication from the spread of odontogenic infections. Nevertheless, fatalities from dental infections continue to occur. One of the most threaded, and probably lethal form is the mediastinal abscess. This is a case of Ludwig's angina dissected along deep cervical planes into the mediastinum caused a virulent mediastinitis and abscess, computed tomography precisely delineated the extent of the infection. Incomplete debridement resulted in a residual abscess and persistent systemic sepsis that culminated in the patient's death. Aggressive antibiotic treatment of the orofacial cellulitis along with complete mediastinal drainage are recommended for optimal outcome.

It is very important for oral and maxillofacial surgeon to understand the attitude of maxillofacial deformity patients toward their deformity. The patients' expectations from surgery, motivations, and the psychological meanings of maxillofacial deformity are considerd by surgeon. The patient's satisfaction and self-assessment on his or her apperance and malocclusion are influenced by realistic condition more and less. There are some differences in points of view on malocclusion between specialist and lay-persons. Some differences from social and cultural base may exist as well. The study of psychological implications of malocclusion with general public will be a foundation in understanding of malocclusion patients. The authers studied the relation between malocclusion and body image, self-perception of malocclusion, social meanings of malocclusion with Korean men around early twenties The results were as follows: 1. The malocclusion played important role in body image. 2. There was difference in the weight of teeth in body image distortion according to the malocclusion. 3. Dissatisfaction aginst his malocclusion was salient in the groups of mandibular prognathism, anterior both crowding, deep overjet . The gruops of deep overjet, anterior both crowding weresusceptibe to the teasing in adolescence period.

7.Intradural Extramedullary Cysticercosis Involving the Thoracolumbar Spinal Canal in a Patient with Cerebral Cysticercosis.

Neurocysticercosis (NCC) by Taenia solium is the most common parasitic infection of the central nervous system involving the cerebrum. However, spinal involvement of NCC is rare. Spinal NCC can cause radiculopathy, myelopathy, cauda equina syndrome, and even paraparesis, depending on its location and size. Spinal NCC may require surgical treatment as a first-line treatment because medical therapy can further aggravate the inflammation due to dead cysts, resulting in clinical deterioration. The current standard therapy for spinal NCC is surgical decompression followed by medical therapy. We experienced a case of widespread thoracolumbar intradural extramedullary cysticercosis involving the spinal canal with cerebral cysticercosis. We report this rare case with literature review.

8.A Masked Marjolin Ulcer despite of Multiple Biopsies Performed at the Outpatient Clinic

Marjolin ulcer (MU) is a rare occurrence of squamous cell carcinoma (SCC) in the location of a previous scar. Old thermal burn scars are the most common site of MU development. Due to the aggressive behavior causing metastasis more frequently, precise diagnose is essential. Treatment of choice is wide excision with a wider margin. Here in, we present a case, where there was a risk of misdiagnosing the carcinoma as benign, since multiple punch biopsies did not reveal the SCC. A high suspicion of cancer according to clinical correlation, patient history, and radiologic studies are recommended to reduce the risk of neglecting cancer. Even if no cancerous lesion is reported in the biopsy, treatment equivalent to that administered for cancers should be initiated in consideration of gross findings and the mechanism of injury. Though it is revealed to be benign lesion through postoperative biopsy, it must be resected due to its precancerous characteristic.
9.The Efficacy of Enhanced Magnetic Resonance Imaging and Computed Tomography in Extensive Infected Deep Burn with Pus Collection

Purpose:
In the case of a chronically unhealed 3rd or 4th degree burn, the lesion may involve the deep structures, and may be deeper and wider than what is visible on gross examination. Deep burns with severe inflammation may result in pus collection.Surgical unroofing and drainage are the treatments of choice for improving the patient's general condition. Before surgery, accurately identifying the range of the lesion is important for planning. Preoperative imaging scans such as magnetic resonance imaging (MRI) and computed tomography (CT) help identify involved deep structures and check the range and presence of pus collections.
Methods:
A retrospective review was performed from 2021 to 2023 on a total of nine cases with severely infected deep burns who were checked with preoperative MRI or CT to confirm the range of lesions and pus collection in our hospital. The patients were chosen based on whether or not there was pus discharge, the inflammation levels, and a fever above 38 degrees.
Results:
Preoperative imaging scans were crucial for determining the timing of surgery and establishing its scope and plan. All nine patients were able to unroof the entire abscess in one surgery without trial and error. After surgery, the inflammation was reduced, and a clean wound bed was observed.
Conclusion
In this report, we discuss the efficacy of preoperative MRI and CT examinations for identifying deep burns and extensive pus collection findings that are difficult to identify on the gross examination when planning surgical management.
10.Effects of Aircraft Noise on Children's Mental Health: Data from the Health Survey of Inhabitants in the Vicinity of Gunsan Airport.

OBJECTIVES: This study was conducted to investigate the effects of aircraft noise exposure on children's mental health problems such as depression and personality. METHODS: We obtained data from the health examinations of school children in the vicinity of Gunsan airport. This survey was conducted in the rural area, Gunsan during 2009. The study population included 195 children attending 4th to 6th grade from 6 elementary schools near the airport. Based on 60 Weighted Equivalent Continuous Perceived Noise Level (WECPNL) measures of aircraft noise, participants were divided into 2 groups; the exposure group consisted of 106 children from 4 schools, and the reference group included 89 children from the 2 schools in Gunsan. All children completed questions on general characteristics, the Korean Children's Depression Inventory (KCDI) for depression, and the Korea Human Nature Inspection (KHNI) for personality. RESULTS: Children in the exposure group were at higher risk of depression (5.62, 95% CI 1.12~28.25), and personality changes (2.08, 95% CI 1.07~4.05) than those of the reference group. In particular, children's diligence, cooperation, and concentration, subgroups of personality measure, were affected significantly and negatively, by aircraft noise level. CONCLUSIONS: Our results suggest that exposure to aircraft noise at school is associated with mental health problems in children. Requirements should be implemented to manage aircraft noise of its affect on children's health.
