1.Basic Rehabilitation Service Needs of People with Disability at Zhabei District in Shanghai

Objective To investigate the factors influencing basic rehabilitation service needs of people with disability at Zhabei District in Shanghai .Methods Primary rehabilitation demands and services of 10 311 disabled persons in Zhabei District were surveyed.Results The screening rate of persons with disability having primary rehabilitation demands was 83.5%. And the gender, age, type of disability, disability level, education, employment might be the factors affecting the primary rehabilitation demands of people with disability.Conclusion The primary rehabilitation service demands vary in deferent types of people with disability. The various services based on the extensive rehabilitation services should be provided to deferent categories of people with disability.
2.Current Status of Special Rehabilitation Service Needs of People with Disability at Zhabei District in Shanghai

Objective To understand the status of special rehabilitation service demands of people with disability at Zhabei District in Shanghai and to provide the basis for the policy development.Methods Screen rate (SR) was used to indicate the level of demand of rehabilitation service, the data were analyzed with SPSS 13.0 statistical software.Results There were 4 349 persons with disability who demanded the special rehabilitation service; the SR of “on-site rehabilitation service” demands was the highest among the whole persons with disability; the SR of “personalized allotment of supplies and appliances” demands of the intellectual disability and mental disability was lower than the other types; the SR of special rehabilitation service demands of the female, the elderly, the low cultural level, the unemployed and the people with severe disability were relatively higher.Conclusion The form of “on-site rehabilitation services” needs to improve based on the demand of the disabilities; “personalized supplies and appliances” for the intellectual disability and mental disability needs to further develop; the rehabilitation services targeted to the female, the elderly, the low cultural level, the unemployed and the people with severe disability need to improve.
3.Multivariate Analysis of Rehabilitation Services Needs of People with Disability at Zhabei District in Shanghai

Objective To provide the advice on how to optimize the allocation of resources and make other policy through the analysis factors affecting rehabilitation services needs of people with disability at Zhabei district in Shanghai.Methods The factors affecting rehabilitation services needs such as types of disability, disability levels, education, employment, marital status, age and sex were analyzed with Logistic analysis.Results The factors affecting primary rehabilitation services were types of disability, education, employment situation, marital status and sex. And people with disability had the characters of female, hearing disability, retired returned to more primary rehabilitation services, with OR values being 1.219, 1.544 and 1.533 respectively compared with itself controls. And the high educational disable people' will, returning to primary rehabilitation services, was weaker compared with low educational disable people. The factors affecting special rehabilitation services were types of disability and employment situation. And people with disability had the characters of hearing disability, unemployment returned to more special rehabilitation services, with OR values being 2.380 and 1.310 respectively compared with itself controls.Conclusion The rehabilitation needs are different among different groups people with disability, so the factors affecting rehabilitation services needs such as types of disability, education, employment situation, marital status and sex should be taken into consideration while activating primary rehabilitation services, while the types of disability and employment situation should be taken into more consideration while activating special rehabilitation services.

5.Quality of Life for 280 Persons with Disablity in Zhabei District, Shanghai

Objective To investigate the quality of life and analyze related factors of all the disabled at the entry of the Home Service Program in Zhabei district, Shanghai. Methods All the disabled were surveyed using the Chinese version of Short-Form 36 Health Survey(SF-36). Results 280 disabled people finished the survey, the total score was (59.72±20.03). There were significant differences in the scores among different ages, types of disabilities and status of subsistence allowances (P<0.01). The age was negatively correlated with quality of life (P<0.01). The education degree and the status of subsistence allowances were positively correlated with quality of life (P<0.05, P<0.01).Conclusion People with disability have poor quality of life in Zhabei district, Shanghai.
6.Intestinal Function Changes in Mice after Spinal Cord Injury in Early Stage

Objective To investigate the influence of spinal cord injury on intestinal function in mice after a short period. Methods 105 Kunming mice were randomly assigned to normal group (group A, n=30), sham group (group B, n=30) and model group (group C, n=45). Group A received no treatment, group B was only exposed the spinal cord, while group C was induced by a constant compression with a modified arterial forcep at T10 to establish spinal cord injury model. 12 hours, 24 hours and 48 hours after modeling, myoelectric slow wave activity and tension activity of the ileum were tested, and HE staining was used. Results Compared with groups A and B, the slow wave activity was significantly weaker in group C at every time point (P<0.05), so was the amplitude of tension activity (P<0.05). Frequency of tension activity was obviously higher in group C than in groups A and B 24 hours and 48 hours after modeling (P<0.05). The injury scores were higher in group C than in groups A and B (P<0.05). Conclusion There is reduce in myoelectric slow wave activity, tension activity of the ileum and mild injury in intestinal mucosa in mice after spinal cord injury in the early stage.
7.Clinical Observations on Wheat-grain Size Cone Moxibustion plus Acupuncture for Mild to Moderate Carpal Tunnel Syndrome

Objective To investigate the effect of wheat-grain size cone moxibustion plus acupuncture on related high-frequency ultrasonographic indicators before and after its treatment of mild to moderate carpal tunnel syndrome and explore its mechanism of action. Method Sixty patients were randomized to an acupuncture group (20 cases) and a wheat-grain size cone moxibustion groups (40 cases). The acupuncture group received acupuncture at affected-side points Daling (PC7), Hegu (LI4), Neiguan (PC6) and Shousanli (LI10). The wheat-grain size cone moxibustion group received wheat-grain size cone moxibustion on point Daling and acupuncture at the other points. Treatment was given once every other day for a total of 10 times. The thickness of the transverse carpal ligament, the cross-sectional area of the median nerve at the level of the pisiform bone and the flattening ratio at the level of the hamate hook were measured by high frequency ultrasonography before and after treatment. The clinical therapeutic effect was evaluated using the SSS score. Result After treatment, the SSS score decreased in both groups compared with before (P<0.05) and was lower in the wheat-grain size cone moxibustion group than in the acupuncture group (P<0.05). The cure and marked efficacy rate was 71.1% in the wheat-grain size cone moxibustion groupwhich was higher than 42.1% in the acupuncture group (P<0.05). There were no statistically significant pre-/post-treatment differences in the thickness of the transverse carpal ligament and the flattening ratio at the level of the hamate hook in the two groups (P>0.05). After treatment, the cross-sectional area of the median nerve at the level of the pisiform bone decreased in both groups compared with before (P<0.05) and was smaller in the wheat-grain size cone moxibustion group than in the acupuncture group (P<0.05). Conclusion Wheat-grain size cone moxibustion plus acupuncture can markedly improve the clinical symptoms of carpal tunnel syndrome. Its mechanism of action is related to reducing the cross-sectional area and edema of the median nerve.
8.Development of time resolved fluorescence microparticle-conjugated anti-human procalcitonin antibody for homogeneous immunoassay

Objective To develop europium (Ⅲ) [Eu (Ⅲ)] chelated microparticles for homogeneous immunoassay.Methods Anti-human PCT antibodies were labeled with Eu (Ⅲ) chelated nanoscale microparticles as the detection antibody,and another anti-human PCT antibody was labeled with biotin as the solid-phase antibody.Magnetic microspheres labeled with streptavidin were used to separate the complexes of Eu-IgM-PCT-IgM-Biotin.Results In the homogeneous immunoassay,the standard curve fit was not linear.The quadratic curve was Y =19170.12 + 75493.74X-26.00X2(r =0.9986).According to the standard curve,the limit of detection for PCT was 0.04 ng/ml.Conclusion The homogeneous immunoassay which uses Eu (Ⅲ) chelated microparticles is highly sensitive for detection of PCT recombinant antigens and may serve as a promising method to measure serum PCT levels in the future.
9.The effects of genistein on epidermal growth factor receptor mediated signal transduction pathway in human ovarian carcinoma cells lines SKOV3 and its xenograft in nude mice.

OBJECTIVEIt was reported previously that genistein could inhibit proliferation of human ovarian carcinoma cell line SKOV(3), but mechanism was not clear. There is a close relationship between EGFR mediated signal transduction pathway and the development of ovarian tumor. This study aimed to investigate the effects of genistein on the EGFR mediated signal transduction pathway and to clarify its mechanisms of proliferation inhibition on human ovarian carcinoma cell line SKOV(3) and its xenograft in nude mice.
METHODSThe expression of c-erbB-2 protein was determined using immunocytochemistry. The expressions of c-jun and c-fos protein were determined using Western blotting. The expression of c-erbB-2, c-raf-1, c-jun and c-fos mRNA were tested by reverse transcription-polymerase chain reaction (RT-PCR).
RESULTSThe expression of c-erbB-2, c-raf-1 and its downstream gene c-jun and c-fos were decreased at mRNA level in the 20 micromol/L genistein group. The expression of c-erbB-2 protein were decreased, its average absorbency (A) were decreased after treatment of SKOV(3) with 20 micromol/L genistein for 48 h, reached at 0.42 +/- 0.02 (P < 0.05). Western blotting demonstrated that the expressions of c-jun and c-fos protein were decreased gradually after being treated with 20 micromol/L genistein for 12 - 72 h.
CONCLUSIONSGenistein could down-regulate the expression of two key genes, c-erbB-2 and c-raf-1 at mRNA and protein level in the EGFR mediated signal transduction pathway, and down-regulate the expression of its downstream nuclear transcription factors c-jun and c-fos at mRNA and protein level. It is suggested that interfering the expressions of some key signal molecules in EGFR mediated signal transduction system by genistein may account for its molecular foundation of proliferation inhibition in ovarian carcinoma.

10.Evaluation of lymph node metastasis in the contralateral mediastinum or scalene through mediastinoscopy and para-mediastinal small incision in potentially operable non-small cell lung cancer.

OBJECTIVEThe purpose of this study was to investigate the clinical characteristics of lymph node metastasis in the contralateral mediastinum and scalene in patients with potentially operable nonsmall cell lung cancer (NSCLC).
METHODSCervical mediastinoscopy was performed for 89 patients with clinical stage I-III A non-small cell lung cancer prior to thoracotomy. Of these, 12 underwent cervical medistinoscopy combined with right scalene lymph node biopsy and 10 with anterior para-mediastinal small incision.
RESULTSA total of 9 patients were found have N3 disease on mediastinosopy, with cancer-cell-positive lymph nodes in the contralateral mediastinum in 6 and 3 in the right scalene. Statistical analysis revealed that the incidence of N3 disease in adenocarcinoma group was higher than that in patients with nonadenocarcinoma (P < 0.05), which was also higher in the patients with serum CEA >5 ng/ml than that in the patients with CEA <5 ng/ml (P < 0.05), and it was higher in the patients with ipsilateral mediastinal multi-station lymph node metastasis than that in the patients with uni-station lymph node metastasis (P < 0.05).
CONCLUSIONBiopsy of contralateral mediastinal lymph nodes or scalene lymph node should be performed in order to exclude N3 disease for potentially operable NSCLC patients with adenocarcinoma, serum CEA >5 ng/ml or ipsilateral multi-station mediastinal lymph node metastasis.
