1.Effect of antibiotics combined with surgical treatment and psychological intervention on congenital heart disease complicated with infective endocarditis

Objective To investigate the clinical treatment of congenital heart disease complicated with infective endocarditis effective programs, joint analysis of the clinical efficacy of the use of antibiotics in surgery on the basis of providing a basis for clinical treatment.MethodsAnalysis of January 2009 to December 2016 in our hospital with congenital heart disease complicated with infective endocarditis clinical data of 78 cases of patients were randomly divided into observation group and control group of 39 cases, The control group was given appropriate antimicrobial treatment, patients in the observation group according to the samples of culture and drug sensitivity test choose appropriate broad-spectrum antibiotics for treatment, observation after treatment and the clinical efficacy of the two groups were analyzed.According to the medical staff were given psychological psychological counseling for patients to explain the importance of disease related knowledge and related treatment, timely eliminate / alleviate the negative emotion of patients with, to ensure that patients with a good attitude to face treatment.Results78 cases of congenital heart disease complicated with infective endocarditis clinical infection germs mainly Staphylococcus aureus and Streptococcus viridans, in addition to a small number of patients infected with Enterococcus faecalis, Staphylococcus epidermidis, gram-negative bacteria and mixed bacteria.Two groups of patients after treatment statistical clinical efficacy results show 56.41% cure rate in patients in the observation group, the effective rate was 97.43% clinical cure rate was 35.90% in the control group of patients, clinical effective rate was 79.49%, a statistically significant difference between groups (P<0.05).Statistical quality of life of patients after two scores, statistics of daily living, psychological feelings, general health, drug dependence and pain and discomfort five indicators, pain and discomfort, and two indicators of the overall health of patients in the observation group was significantly better than the control group (P<0.05), no significant difference.ConclusionThe clinical administration congenital heart infection endocarditis surgical treatment of broad-spectrum antibiotics combined with substantial clinical efficacy, safety, reliability, patients were recovering well, can be clinical.
2. Application of 3D printing technology in fracture and deformity of foot and ankle

BACKGROUND: The anatomical structure of the foot and ankle is complex. There are many extensor and flexor tendons passing through the foot and ankle, which is adjacent to many blood vessels and nerves. The foot and ankle play an important role in human walking, jumping and squatting. As the ankle joint itself is the most distal load-bearing joint of the limb, it is difficult to achieve satisfactory therapeutic effect by traditional methods to treat complex periankle fractures (such as severe comminuted trimalleolar fractures and Pilon fractures), or various severe ankle deformities (such as talipes equinovarus, hallux valgus, and flatfoot). Fortunately, in recent years, 3D printing technology has significantly improved the success rate and patient satisfaction of the above diseases. OBJECTIVE: To summarize the current situation of 3D printing technology in the treatment of ankle fracture and deformity. METHODS: The computer was used to search CNKI, PubMed database. Chinese search terms were “3D printing technology, ankle surgery, ankle fracture, ankle deformity, orthopedics”. The English search terms were “3D printing, foot and ankle surgery, ankle fracture, foot and ankle deformity, orthopedic surgery”. The retrieval period was from January 2000 to February 2020. A total of 105 articles were retrieved. According to the inclusion and exclusion criteria, 55 articles were finally included for review. RESULTS AND CONCLUSION: (1) Through the simulation operation of foot and ankle surgery on 3D physical model, the operator’s proficiency in the operation can be improved. Simultaneously, according to the needs of the operation, individual surgical instruments can be made to improve the success rate of the operation. (2) Compared with the traditional foot orthosis, 3D printing technology can make foot orthosis more suitable for human body structure.
3.Golph3 expression and apoptosis in colorectal cancer cells

Objective:To investigate the correlation between the expression of Golgi phosphoprotein 3 (GOLPH3) and the occurrence of apoptosis in colorectal cancer cells (CRC). Methods:Immunohistochemical assays of GOLPH3 and caspase-3 were performed on the paraffin-embedded sections of 62 CRC samples using the standard streptavidin-peroxidase technique. The apoptotic index of the CRCs was examined using the in situ terminal deoxynucleotidyl transferase nick-end labeling technique. The relationship of the GOLPH3 expression, the cell apoptosis, and the clinicopathologic parameters was analyzed. Results:The positive rates of GOLPH3 expression were significantly higher in the CRC tissues (53.2%) than in the normal colorectal mucosa (37.2%;P<0.05). Likewise, GOLPH3 expression was significantly higher in poorly differentiated cancer tissue, tissue outside the serous membrane, metastatic lymph node tissue, and the stage III CRCs, as compared with those of the moderately to well differentiated tissue, tissue inside the serous membrane, lymph node tissue without metastasis, and the stage I to II CRCs (P<0.05). However, GOLPH3 expression was not significantly correlated with the other clinicopathologic pa-rameters, namely, the age and sex of the patients as well as the site, depth, and length of the invasive tumor (P>0.05). The caspase-3 expression and apoptotic index were significantly lower in the GOLPH3-positive CRC tissue than in the GOLPH3-negative tissue (P<0.05). GOLPH3 expression was negatively correlated with the apoptotic index of CRCs based on the Spearman correlation (r=-0.320, P<0.05). Conclusion:GOLPH3 overexpression in CRC tissue is negatively correlated with apoptotic index.

5.Effect evaluation of nursing intervention on coping with fatigue in patients with cervical spine fracture and high paraplegia

Objective To study on the effect of nttrsing intervention on coping with fatigue in patients with cervical spine fracture and high paraplegia.Methods 46 patients who suffered cervical spine fracture with paraplegia were divided into the control group and the intervention group with 23 patients in each group randomly.The control group received routine care in general,while the intervention group was given additional systematic nursing intervention by full-time nurses.The brief fatigue inventory(FSI),as fatigue assessment tools,was used to assess the fatigue of patients in the two groups before invention and 20 days after invention respectively.Results As a result,we found that the difference of fatigue between the two groups had no prominent significance before intervention,but after invention the difference had prominent significance.Conclusions The nursing intervention is very important for patients with cervical spine fracture and paraplegia and has significant advantages to alleviate or eliminate patients' fatigue and improve their quality of life.
6.Clinical study of early enteral nutrition on prevention of stress ulcer in patients with severe craniocerebral brain injury

Objective To study the effect of early enteral nutrition on stress ulcer in patients with severe craniocerebral injury.Methods 134 patients with severe craniocerebral injury were randomly divided into two groups,the early enteral nutrition group (the EEN group,69 cases) was given nasogastric intubation feeding after hospitalization or within 24 hours postoperation,the delayed enteral nutrition group (the DEN group,65 cases) was given nasogastric intubation feeding 24 hours postoperation.The incidence of upper gastrointestinal bleeding in two groups were compared.Results The incidence of upper gastrointestinal bleeding in the EEN group was significantly lower than that of the DEN group.Conclusions Patients with severe brain injury should be given enteral nutrition as soon as possible to reduce the occurrence of stress ulcer and prevent upper gastrointestinal bleeding.
7.Research on mechanism of energy metabolism disorders of rat's hepatoxicity induced by saikosaponins.

OBJECTIVETo study the influence of saikosaponins on function of rats' liver mitochondria, its liver damage mechanism was discussed.
METHODAdministrating alcohol eluent of saikosaponins of different dose for 15 days to rats, and the high, middle and low lose-group are separately 300, 150, 50 mg x kg(-1) caculated by total saikosaponins. The liver index in serum, the respiratory function of liver mitochondria,the content of ATP and the activity of ATP enzyme were detected. The weight of heart, liver, spleen, lung, renal of rats were precisionly weighed, and the ratio of organ to body were calculated. The histopathologic examination of hepatic tissue were examined.
RESULTAlcohol eluent of saikosaponins of different dose can induce apparent decrease of PCR, P/O value, respiratory oxygen consumption and the activity of ATP enzyme; the level of ALT, AST and ALB in serum increased; the liver weight and the ratio of liver to body increaseed, and the hepatic tissue damage is obvious in the histopathologic examination of hepatic tissue. The above-mentioned changes gradually aggravates with dose increasing, and it is obviously discrepancy compared with control group.
CONCLUSIONAlcohol eluent of saikosaponins can induce liver damage by restraining the respiratory function of mitochondria and effecting liver's energy metabolism. Other hepatoxicity mechanism still need to be discussed.

8.Standardized management of acupuncture-moxibustion clinic in Singapore General Hospital.

The standardized management of acupuncture-moxibustion in Singapore General Hospital is introduced. With gradual improvement of outpatient infrastructure, re-training of medical staff, strict disinfection of manipulation, periodical inspection of medical instruments, unified management of writing, saving and processing in medical records and public education of TCM knowledge, a standardized management system in accordance with modernized hospital is gradually established. As a result, efficiency and quality of clinical treatment is continuously increasing. From April of 1998 to December of 2012, a total of 74 654 times of treatment were performed, and treatment amount per day is gradually increased. The unusual condition of acupuncture is avoided. Periodical strict inspection of joint committee authenticated by domestic and overseas medical health organization is repeatedly passed and accepted. Additionally, three clinical researches funded by Singapore Health-care Company are still in progress in acupuncture-moxibustion department.

9.Prognostic value and predictive threshold of tumor volume for patients with locally advanced nasopharyngeal carcinoma receiving intensity-modulated radiotherapy

Background:Gross target volume of primary tumor (GTV?P) is very important for the prognosis prediction of patients with nasopharyngeal carcinoma (NPC), but it is unknown whether the same is true for locally advanced NPC patients treated with intensity?modulated radiotherapy (IMRT). This study aimed to clarify the prognostic value of tumor volume for patient with locally advanced NPC receiving IMRT and to ifnd a suitable cut?off value of GTV?P for prognosis prediction. Methods:Clinical data of 358 patients with locally advanced NPC who received IMRT were reviewed. Receiver oper?ating characteristic (ROC) curves were used to identify the cut?off values of GTV?P for the prediction of different end?points [overall survival (OS), local relapse?free survival (LRFS), distant metastasis?free survival (DMFS), and disease?free survival (DFS)] and to test the prognostic value of GTV?P when compared with that of the American Joint Committee on Cancer T staging system. Results:The 358 patients with locally advanced NPC were divided into two groups by the cut?off value of GTV?P as determined using ROC curves: 219 (61.2%) patients with GTV?P≤46.4mL and 139 (38.8%) with GTV?P>46.4mL. The 3?year OS, LRFS, DMFS, and DFS rates were all higher in patients with GTV?P≤46.4mL than in those with GTV?P>46.4mL (allP<0.05). Multivariate analysis indicated that GTV?P>46.4mL was an independent unfavorable prognostic factor for patient survival. The ROC curve veriifed that the predictive ability of GTV?P was superior to that of T category (P<0.001). The cut?off values of GTV?P for the prediction of OS, LRFS, DMFS, and DFS were 46.4, 57.9, 75.4 and 46.4mL, respectively. Conclusion:In patients with locally advanced NPC, GTV?P>46.4mL is an independent unfavorable prognostic indi?cator for survival after IMRT, with a prognostic value superior to that of T category.
