1.Application of nutritional risk screening in inpatients of digestive department

Objective: To investigate the prevalence of nutritional risk,malnutrition and application of nutritional support in department of digestive trad in our hospital in Ningbo.Methods: The information of 205 patients were collected in our hospital.Nutrition status was assessed according to Nutrition Risk Screening(NRS) published by ESPEN in 2002.Results: A total of 205 patients were enrolled,and 189 patients(92.2%)underwent NRS 2002.Prevalence of malnutrition was 20.0% and nutritional risk was 32.7%.Totally 18 patients(8.8%) receiving nutritional support had a NRS2002≥3.The PN∶EN ratio was 2.6 ∶1. Conclusion: A large proportion of inpatients were at nutritional risk or malnutrition in our hospital.The application of PN and EN is inappropriate in inpatients.Evidence-based guidelines are required to improve this situation.
2.MICROVASCULAR ARCHITECTURE IN THE LEFT VENTRICULAR WALL OF CHILDREN HEART AS STUDIED BY LIGHT MICROSCOPY AND SCANNING ELECTRON MICROSCOPY

The microvascular architecture of the three layers in left ventricular wall of25 normal children hearts was studied by scanning electron microscopy of the vascu-lar casts,histochemical method(alkaline phosphatase stain)and light microscopy ofIndia-ink injection.A true three dimensional spatial configuration of the microcir-culation unit was obtained.The arteriolar configuration had very different characteristics from one anotherin the three layers examined.The arterioles in the epicardium presented a similarmesh distribution;tree-like branches were distributed mainly in the myocardium;thearterioles in subendocardium appeared as a plexiform distribution.The spatial struc-tures of capillary had several different characteristics:a close capillary network inthe epicardium;the capillary mesh in the myocardium showed the appearance of anarcade-type and vertex-like arrangement and parallelly arranged to the myofibers;the one in the subendocardium seems to be sheet-like.Arteriole casts from the three areas were essentially the same in the fact thatthey frequently showed prominent endothelial nuclear impressions and ring-shapeimpressions.The unusual appearance of venules called“turnip root”and“ginsengroot”was found in the three regions.The diameter of capillaries are 7.2?1.9?m,4.3?1.0?m and 5.4?1.6?m in theepicardium,myocardium and endocardium respectively.The difference between thediameter of capillaries in three layers is highly significant(P0.05).
3.Methylation status of DLEC1 promoter in colorectal cancer patients and its clinical relevance

Objective To detect the methylation status of DLEC1 promoter in the tissue and serum of colorectal cancer(CRC) patients and to evaluate its clinical relevance.Methods Genomic DNA was extracted from the tissues(cancer tissue and corresponding adjacent normal tissue) and sera of 71 CRC patients;serum genomic DNA was also obtained from 20 patients with benign gastrointestinal diseases and 20 healthy donors.Promoter methylation status of DLEC1 gene was detected by methylation-specific polymerase chain reaction(MSP).Results The incidence of aberrant methylation of DLEC1 promoter was 45.1%(32/71) in CRC tissues,which was significantly higher than that in the adjacent normal tissues(7.1%,4/56)(P
4.A SEM OBSERVATION OF THE THREE-DIMENSIONAL STRUCTURE OF THE MICROVASCULATURE OF SKELETAL MUSCLE

The authors used a microvascular corrosion cast/scanning electron microscopy (SEM) technique to study the microvascular bed of the muscles of the medial pterygoid and masseter in two normal babies. The cast were prepared by perfusing the microvascular vessels with methyl methacrylate. Under SEM, the patterns of branches of the arteriole are: 1. tree like ramifications; 2. symmetrical branching; 3. fine plexiform branching. Moreover, the casts of arterial and venous vessels showed on their surfaces the typical imprint of the endothelial lining. Therefore, the venous vessels could be easily identified by the round nuclei of the endothelial cells of the venous vessels. On the other side, arteries and arterioles displayed spindle shaped nuclear imprints, oriented along the direction of the vessel. The morphological and functional characterization of the imprint of the precapillary sphincter and the two-grade anastomoses of the arcade arteries. The diameter of the capillary casts (mean?SE) was 5.6?1.9?m. Two or three capillaries join together to form a postcapillary venules, sometimes a single capillary reaches a major venous trunk was observed. These structural features were considered to play an important physiological role in the microcirculation in skeletal muscle.
5.Chemical constituents of Paeonia veitchii

Objective To study the chemical constituents of Paeonia veitchii. Methods Isolation and purification were carried out on repeated silica gel column chromatography. The structures of the compounds were identified by physico-chemical properties and spectral analyses. Results Nine compounds were isolated and identified as paeoniflorin (Ⅰ), hydroxypaeoniflorin (Ⅱ), benzoylpaeoniflorin (Ⅲ), benzoylhydroxypaeoniflorin (Ⅳ), albiflorin (Ⅴ), paeonisothujone (Ⅵ), mudanpinoic acid A (Ⅶ), 2-hydroxybenzyl alcohol (Ⅷ), bis (2-hydroxybenzyl) ether (Ⅸ). Conclusion Compound Ⅸ is obtained from natural products for the first time. Compounds Ⅳ, Ⅵ-Ⅸ are isolated from this plant for the first time.
6.In vitro and in vivo study on the regulation of microRNA-101 in human gastric cancer proliferation, migration and invasion

Objective To investigate the expression of microRNA‐101 (miRNA‐101 ) in human gastric cancer ,and to explore its effects on proliferation ,migration and invasion of gastric cancer cell . Methods The expression of miRNA‐101 in 28 human gastric cancer tissues ,human gastric cell lines BGC‐823 , SGC‐7901 , MKN‐45 , AGS and human normal gastric epithelial cell line GES‐1 were determined by real time polymerase chain reaction (PCR) .Recombinant miRNA‐101 adenovirus vector was constructed . The effects of miRNA‐101 on gastric cancer proliferation was detected with cell proliferation assay .The ability of gastric cancer cell migration and invasion was assessed with Transwell assay .Gastric xenograft cancer model was established in BALB /c nude mice and the tumor size was compared .The t test was used for the statistical analysis .Results The expression of miRNA‐101 in gastric cancer tissues was 0 .661 ± 0 .396 ,which was lower than that of corresponding para carcinoma tissues (1 .128 ± 0 .697) ,and the difference was statistically significant (t = 10 .091 , P < 0 .01) .The expression of miRNA‐101 in normal gastric epithelial cell line GES‐1 was higher than those of gastric cancer cell lines BGC‐823 ,SGC‐7901 , MKN‐45 and AGS . There was significant suppression role of miRNA‐101 on MKN‐45 cells proliferation , and which also had inhibition role on cell migration and invasion of gastric cancer cell lines BGC‐823 ,SGC‐7901 ,MKN‐45 and AGS .At five weeks after MKN‐45 gastric xenograft cancer nude mice model established ,the tumor size of Ad‐miRNA‐101 group ((333 .56 ± 46 .71) mm3 ) was smaller than that of Ad‐enhanced green fluorescent protein (EGFP) group (806 .41 ± 51 .83) mm3 ,and the difference was statistically significant (t = 21 .431 , P < 0 .01 ) .Conclusion In gastric tissues and cells ,miRNA‐101 is a tumor suppressive miRNA and its downregulated expression involved in the genesis and development of gastric cancer ,which may be a new target of biological target therapy in gastric cancer .
7.Effect of ginsenosides from Panax ginseng on proliferation of human osteosarcoma cell U_2OS

Object To find out which of the 27 ginsenosides isolated from Panax ginseng C A Mey that may inhibit the proliferation of human osteosarcoma cell line U 2OS Methods Effects of each individual ginsenoside on the proliferation of U 2OS cell were studied by determining the viability of cancer cells during culture with or without the presence of the test compound DNA assay was determined by flow cytometry Results Ginsonosides Ro, Rh 1, Rh 2, F 1 and L 8 at concentrations of 5 ?mol/L could obviously suppress the proliferation of U 2OS cells while ginsenosides Rg 1, F 3, Rf, PPT and PT significantly inhibited the cancer cells Flow cytometry revealed that ginsenosides Ro, Rg 1, Rf, F 1, Rh 2 ,PPT and PT induced cell cycle arrest at G 0/G 1 phase with obvious decrease of cell count at S and G 2+M phase Moreover, ginsenosides Rf 1, Rg 1, F 1 and PPT induced significantly high rates of cell death as compared with the control Conclusion These data suggested that ginsenosides inhibited U 2OS proliferation via cell cycle arrest or induction of cell death
8.Early mortality and risk analysis in adult patients with maintenance hemodialysis

Objective:To retrospectively analyze the early mortality and related risk factors in adult patients with maintenance hemodialysis (MHD).Methods:Adult MHD patients from 2008 to 2018 were enrolled and divided into training data group and validation data group. In training data group, multivariate logistic regression was used to analyze the risk factors of early death within 120 days after hemodialysis and establish a prediction model. The receiver operating characteristic (ROC) curve was applied to evaluate the prediction ability of the model.Results:A total of 4 885 patients were included. The cumulative mortality within 120 days was 20.97/100 person years, and that within 365 days was 12.25/100 person years. A total of 3 603 patients in the training data group were analyzed. The following risk factors were correlated with early mortality (all P<0.05), including age at start of dialysis over 60 years old ( OR=1.792), non-chronic glomerulonephritis ( OR=2.214), cardio-cerebrovascular disease ( OR=2.695), plasma albumin less than 35 g/L ( OR=1.358), platelet count less than 120×10 9/L ( OR=2.194), serum creatinine less than 600 μmol/L ( OR=1.652), blood urea nitrogen over 30 mmol/L ( OR=1.887), blood phosphorus less than 1.13 mmol/L ( OR=1.783), pulse pressure over 55 mmHg(1 mmHg=0.133 kPa) ( OR=1.656), low density lipoprotein less than 1.5 mmol/L ( OR=1.873), and blood calcium over 2.5 mmol/L ( OR=1.876). Risk prediction model was established. The other 1 282 cases in the validation data group were verified. The area under ROC curve was 0.810, with sensitivity 85.7%, and specificity 62.5%. Conclusion:The mortality rate of adult MHD patients within 120 days after dialysis is high. The established prediction model can effectively predict the risk of early death.
9.Clinical application and efficacy of lag screw in condylar intracapsular sagittal fracture

Objective To discuss the applicability and application skills of lag screw in condylar intracapsular sagittal fracture and also to observe its therapeutic effect .Methods We conducted surgical reduction and rigid internal fixation to 21 patients with 27 sides of condylar intracapsular sagittal fracture in department of oral and maxillofacial surgery of the first affiliated hospital of chongqing medical university .Fixation of 24 sides were carried out with lag screw and and the rest sides had condylar bone fragment removal surgeries .Imaging data and clinical symptoms of each side were analyzed before and after the treatment .Results After postoperative follow‐ups in average 17 .4 months ,there were no facial paralysis ,salivary fistula and loose screws in 21 patients .Al‐so ,the recoveries of mouth opening degree and occlusal relationship were satisfactory .There were different degrees of temporoman‐dibular joint disturbances syndrome for the 3 patients with just 1 side done with condylar bone fragment removal surgeries and screw internal fixation before .Seven sides of patients had malocclusion and slight bone absorption in image inspection after surgery . However ,the functions were basically normal .Conclusion Lag screw could be used in condylar intracapsular sagittal fracture and the effect in recovery of the height of ramus and the function of joints is exact .
10.Biomechanical study on three-dimensional internal fixation for unstable posterior pelvic ring fracture

Objective To compare the biomechanical difference of two internal fixation approaches for the posterior pelvic ring fracture dislocation.Methods Six fresh adult cadaver pelvic specimens were tested biomechanically.Then,the specimens were subjected to Denis-I sacral pelvic fracture.The specimen was first fixed with improved Galveston fixation and then with improved Galveston fixation plus sacral rod.Biomechanical tests were performed from vertical and reversed directions on the fixed pelvis.Results Under vertical compression,the biomeehanical value of the improved Galveston fixation plus sacral rod was lower than that of the improved Galveston fixation,with no statistical difference between two methods(P > 0.05).Under the reversed compression,the biomeehanieal value of the improved Galveston fixation plus sacral rod was lower than that of the improved Galveston fixation,with statistical difference between two groups(P < 0.05).Conclusion The improved Galveston fixation combined with sacral rod can increase the anti-reverse stability of the posterior pelvic ring.
