3.Combined recombinant human growth hormoneand fibrin glue accelerate wound healing of intestinal anastomosis in abdominal sepsis:A dynamic study

Objectives: To determine whether administration of growth hormone and fibrin glue could attenuate the inhibitory effects of sepsis on wound healing of intestinal anastomosis in rats. Methods: Antimesenteric transverse enterotomy of 4 mm was performed, a second laparotomy was subsequently performed after 24 hours, 6 cm of the intestine in the middle of the fistula segment was resected. An end-to-end single layer anastomosis was constructed using 4 inverted interrupted 6/0 polypropylene sutures. After anastomosis, the animals were divided into three groups.In control group, the rats had a sutured anastomosis;in fibrin glue(FG) group, the rats had a sutured anastomosis and fibrin glue; and in the FG/GH group, the rats had a sutured anastomosis,fibrin glueand growth hormone.Growth hormone of 3.0 U/(kg?d) was injected subcutaneously every 24 hours for 14 days. The anastomotic bursting pressureand breaking strength, hydroxyproline contentand histologywere measured as indicators of wound healing. Results: The breaking strengthand bursting pressureof intestinal anastomosis were increased at 12 th postoperatively and lasted for 5 days in FG group, and they were increased and lasted for 14 days in FG/GH group. Conclusions: Both fibrin glueand growth hormone can improve wound healing of intestinal anastomosis, and they had the effect of congruential sequence.
4.Treatment of velopharyngeal insufficiency with a modified pharyngngeal flap

Objective:To study the a feasibility of modified pharyngoplasty in the treatment of velopharyngeal insufficiency.Methods:30 cases of velopharyngeal insufficiency were treated with a modified pharyngoplasty followed by speech training.The effects of the treatment were evaluated by blowing test (BT),chinese articulation inteligibility test (CIT) and hypernasality observation(HO).Results:Before and after treatment BT was 1~26 and 8~40,CIT(%) 20~60 and 47~82,HO(cases) 30 and 3,respectively.Conclusions:The modified pharyngoplastic surgery is feasible in the treatment of velophayngeal insufficiency.
5.Generation of the monoclonal antibody against Ciprofloxacin and analysis for its immunological traits

Objective:To generate monoclonal antibodies against ciprofloxacin (CIP) and to analyze its immunological traits for establishing a determination method for ciprofloxacin residues in food tissues.Methods:BALB/c mice were immunized by the conjugation of CIP-BSA successfully.The splenic cells of BALB/c mice and the oncocyte were fused and screened in HAT culture medium.Cell strains of 1C9,3F6,6H2,6A7,6G11 and 8F5 were cloned,which could secret the monoclonal antibody (Mab) for ciprofloxacin.Results:By ELISA method,the haplotype of Mabs were identified.The antibody secreted by 1C9,3F6 and 6A7 were classified to the IgG2a.Those of 6H2 and 8F5 were classified to the IgG1 while 6G11 to IgG3.The results of SDS-PAGE showed that the Mab protein was made up of the weight chain and the light chain whose molecular weight were 50 kD and 25 kD,respectively.All of those Mabs had fine specificity and sensibility.The indirect ELISA titer of 1C9 was 1:6.4?102 in supernatant and 1:5.6?105 in ascites.The affinity of 1C9 was 2.85?109 L/mol and the value of IC50 reached to 245.86 ng/ml.The protein secreted by 1C9 was screened for establishing the ELISA method and the detective limitation reached to 45.25 ng/ml.There were no cross reactions to Ofloxacin,Difloxacin,Sarafloxacin,Malachite green,Chloramphenicol and Furacilin while the rate of cross reaction to Enrofloxacin could reach to 84.6%.Conclusion:The Mabs are sensitive and specific,which are suitable to be applied in establishing immunoassay of CIP residues.
6.Change of serum vascular endothelial growth factor level in patients with bladder transitional cell carcinoma

AIM: To investigate serum vascular endothelial growth factor (VEGF) level in patients with bladder transitional cell carcinoma(BTCC) and its clinical significance. METHODS: ELISA method was used to examine the serum VEGF level in 42 cases of bladder transitional cell carcinoma and in 10 cases of normal people as control. The change of VEGF in blood of the pre-operation and post-operation patients with BTCC was also compared. RESULTS: The VEGF level in blood of the patients was higher than that of the normal people, in spite of pre-operation, post-chemotherapy, and post-operation, but VEGF level decreased obviously after chemotherapy or operation. In addition, the plasma VEGF level was related to the grade and invasion of tumor. CONCLUSION: Detecting serum VEGF level can help us to assess the change of tumor and therapeutic effect.

Objective To study the effects of liver graft on the immune tolerance of intestinal allograft in auxiliary en-bloc liver-small bowel transplantation in pigs.Methods Seventy outbreed Landrace pigs were divided into 4 groups.Ten auxiliary liver-small bowel allotransplantations were performed in group A,B,C,respectively,and 5 segmental small bowel allotransplantatiom were performed in group D.Pigs were administered with routine and lower dose of cyclosporine and methylprednisolone in group B and C,respectively. No immunosuppressive agent was administered to pigs in group A and D.Results The initial time of acute rejection was obviously prolonged in group A than group D.and the acute rejection was milder in group A than group D(P<0.05).There was no significant difference upon postoperative survival time,initial time of acute rejection and degree of acute rejection between group B and C(P>0.05).Conclusions The immune tolerance of intestinal allograft Can be induced by liver graft in auxiliary en-bloc liver-small howel transplantation.

Objective To explore the Schistosoma japonicum cercariae collected method and the quantity of cercariae obtained from a great quantity artificial infected snails. Methods In laboratory condition, Oncomelania snails were infected with schistosome miracidia. Sixty days post-infection all snails were divided into 7 shares. Cercariae shed from 1 share snails every day and the number of all shedding days were 40. Cercariae shed from snails were collected with low-velocity centrifuge and the cercariae sediment were weighted. Results One thousand and nine hundred g snails bred for 120 days post-infection, the infection and survival rates were 36. 00% and 51. 58%. Cercariae col-lected were 10. 5 g from 40 days collection. Cercariae quantity of shedding from 1 000 positive snails per day was 0. 257 4 g.
9.Nephrectomy and removal of inferior vena cava tumor thrombus under profound hypothermia and arrested circulation

Objective To study the feasibility and safety of performing nephrectomy together with the removal of complicated inferior vena cava tumor thrombus under profound hypothermia and arrested circulation. Methods After made the median thoraco-abdominal incision, the exploration of the abdominal organs was done. The right kidney, inferior vena cava and renal pedicle were well exposed then. After the whole body heparinization, cannulas were put into ascending aorta, superior vena cava, aortic root and right superior pulmonary vein. The body temperature was reduced to 20℃ with cardiopulmonary bypass unit and the extracorporeal circulation was stopped then. Cut open the inferior vena cava at vena renalis dextra ingress and the F16 urinary catheter was inserted into atrum dextra through inferior vena cava and inflated. The tumor thrombus was pulled out and the right kidney was removed. The inferior vena cava incision was sutured to close and the extracorporeal circulation was resumed and patient was re-warmed.Results The operation time was 330 min and the extracorporeal circulation time was 90 min, while the profound hypothermia with circulatory arrest time was 20 min. The estimated blood loss during operation was 400 ml and 6 unit red cells and 600 ml blood plasm were transfused. The patient was awaked 2.5 h after the operation, food intake resumed 4 days after operation and the patient was discharged on day 10 post-operatively. After 6 months'follow-up, there were no local recurrence and metastasis occurred. Conclusion The technique of profound hypothermia and circulation arrest could improve the safety and efficacy in the treatment of renal cell carcinoma with suprahepatic (level Ⅲ) caval tumor thrombus.

Objective To approach the clinical presentation,treatment and diagnosis of primary seminal vesicle carcinoma.Methods The records of 4 patients who diagnosed seminal vesicle carcinoma were retrospectively reviewed,including the symptoms signs and examination results as well as operation program,postoperative therapy.Considered to the literature reports.Bilateral seminal vesicles,bladder, prostate and rectum were totally removed in one case.Seminal vosiculectomy and partial cystoprostotectomy were performed in 2 cases,and the another one,bilateral lower ureterectomy and ileum substitute bladder was be done.Results Followed up for 3 months to 5 years,and no recurrence had been observed so far, one died of colon cancer after 2 years.Conclusions Early symptoms of primary seminal vesicle carcinoma are unobvious,so that early diagnosis of seminal vesicle carcinoma is difficult and the misdiagnosis is so usually.Thus,accurate recognition is important for early diagnosis.Radical surgery appears to offer the best chance and different approaches can be selected according to tumor stage and invasive condition of the circumambient organ.Comprehensive treatment like hormonal therapy,along with the 5-fluorouracil,paclitaxel,and oxaliplatin chemotherapy regimen appears to be effective against adenocarcinoma.
