1.Analysis of Detection Indicators of Blood Donor’s Transfusion Infection in Wuxi Area from 2 0 0 5 to 2 0 1 2

Objective To understand the Wuxi area index detection of infectious blood donors in the blood situation,in order to take targeted measures to block the blood-borne diseases.Methods From 2005 to 2012,Wuxi area (not including county, district)blood donation blood samples 329 254 (person)copies of the test results were retrospectively analyzed.Results 8 years,Wuxi unpaid donors HBsAg,anti-HCV,anti-HIV,and anti-TP four tested positive rates were 0.587%,0.452%, 0.212% and 0.509%,the total positive rate was 1.76%.Different years of anti-HCV,anti-HIV and anti-TP positive rate difference was statistically significant (χ2= 40.74~138.24,P<0.01).From 2011 to 2012 HBsAg positive rate was signifi-cantly increased,and compared with the previous year,difference was statistically significant (χ2=47.56,52.34,P<0.01), while four the number of positive indicators of infection in different sex,age and occupation with different proportions of dis-tribution.Conclusion The positive rate of most indicators declined year by year,but there was still an upward trend in indi-vidual proj ects.Should further improve the different groups of scientific blood donation advocacy work to ensure blood safe-ty.
2.The analysis of risk factors in gastrointestinal graft versus host disease following allogeneic hematopoietic stem cell transplantation

Objective To investigate the risk factors of gastrointestinal graft versus host disease(GVHD) following allogeneic hematopoietic stem cell transplantation (allo-HSCT).Method 214 patients receiving allo-HSCT from 2003 to 2012 were enrolled in this study,and assessed on the degree of gastrointestinal GVHD.The effects of the primary diseases status,gender,age,conditioning regimen intensity,donor type,the number of cells positive for the CD34 expression,and the use of anti-lymphocyte immunoglobulin (ALG) in the pretreatment scheme on the occurrence of gastrointestinal GVHD was studied.The responses of different degrees of GVHD to immunotherapy were evaluated.Result Univariate and multivariate analyses revealed that the graft type and the conditioning regimen intensity were the risk factors of gastrointestinal GVHD (P<0.05).Conclusion Donor type and conditioningregimen intensity may be the main risk factors of gastrointestinal GVHD.
3.Study on effect of PluronicF-127 and vascular endothelial growth factor composite delivery system for improving the survival of grafted fat.

To explore the effect of PluronicF-127 and Vascular endthelial growth factor(VEGF) delivery system on the survival of the grafted fat, we divided fat harvest under the same condition into 4 groups. One group served as blank control; the other 3 groups served for experiments with respective to DMEM containing 20 ng/ml VEGF; DMEM containing 30% Pluronic F-127; DMEM containing 20 ng/ml VEGF and 30% Pluronic F-127, and then we transplanted the 4 groups of fat, subcutaneously, on the back of 3 groups of BALB/c nude mice (8 mice per group; injecting 4 points per mouse; 0.2 ml per point). At 3 weeks, 6 weeks and 12 weeks, we dissected the fat grafts, measured their weight retention, and put them in histopathologic examination with the use of HE and CD34 staining. And we compared the weight retention and microvessel density (MVD) of each experiment group versus those of control group. The relation between adipose cell and PluronicF-127 was observed through electron microscope. The results reveal that the MVD and weight of pluronicF-127 and VEGF of the experiment groups are significantly greater than those of other groups. The PluronicF-127 and VEGF composite delivery system can significantly improve the blood circulation for fat transplantation, and increase the survival rate of grafted fat.

4.Preparation and osteoinduction capacity of collagen/bone morphogenetic protein composites.

Collagen/bone morphogenetic protein composites were prepared with collagen type I sponge and bone morphogenetic protein-2 (BMP-2). The composites were implanted into Latissimus dorsi muscles pouches of rabbits. Samples were studied with ALP staining, Von Kossa staining, HE staining, toluidine blue staining and CD31 histochemical labeling of microvessel. Bony samples were then used to repair mandibular defect. The effects were evaluated by X-ray, compressive strength, economycin fluorescence labeling, HE staining, toluidine blue staining and bone quantity analysis. Bone formation induced by collagen/BMP composites was found as woven bone between 4 and 6 weeks; cartilaginous osteogenesis was the main type of bone formation; microvessels could be seen in the bony tissues; and the bone defects were healed completely 6 weeks after operation. Bone formation induced by collagen/BMP composites in the muscles can be used as a donor to repair the bone defect.

5.Influence of mixing parameters in STAR automatic sample processing instruments on the detection results of internal controls

【Objective】 To explore the significance of blending internal controls by automatic sample processing instruments in the enzyme linked immunosorbent assay (ELISA). 【Methods】 The internal controls were vortexed and mixed before the test, and then were added to the same ELISA plate by the STAR automatic sample processing instruments under the same detection conditions. The difference of S/CO value of internal controls with and without sufficient blending via the sampling needle and their frequency distribution were compared. Internal controls that were greatly affected by mixing parameters were submitted to the same test with different batches of reagents from the same manufacturer, and the results were analyzed for consistency. 【Results】 The S/CO value of anti-HCV internal controls without blending using adding sample needle was significantly lower than that of quality control samples with sufficient blending (P<0.000 1). The S/CO values of unmixed internal controls concerning anti-TP and anti-HIV detection given by some detection systems were also different from the values of mixed internal controls (P<0.05). Some of the S/CO values of the anti-HCV internal controls without mixing were distributed within the interval of less than 2. Different batches of reagents from the same manufacturer were used to detect anti-HCV internal controls, and there were differences in the partial detection values between the mixed and unmixed internal controls (P<0.05). 【Conclusion】 Although the internal controls were mixed by vortex shock before the test, the detection results of some internal controls will still be affected when the STAR automatic sample processing instruments does not set the mixing parameters for internal controls.
6. Predict response to decitabine in patients with myelodysplastic syndrome and related neoplasms

Objective:
To identify clinical and molecular signatures for predicting response to decitabine (DAC) in patients with myelodysplastic syndrome (MDS) and related neoplasms.
Methods:
The clinical characteristics of 109 patients with MDS and related neoplasms who were treated with DAC were analyzed retrospectively and the next target sequencing was performed to define recurrently mutated genes in these disease samples, to examine the association of the clinical and molecular signatures with response to DAC treatment.
Results:
Of 109 MDS and related neoplasms patients, there were 70 males and 39 females, the median age was 61 years old (ranges: 17-85 years old) . According to the international prognostic scoring system (IPSS) , 46 cases were included in the relatively low risk group (low risk and intermediate-1 risk) , 63 in the relative high risk group (intermediate-2 and high risk) . There were 21 cases with complex karyotype, 17 chromosome 7 abnormality and 17 monosomal karyotype. The median courses of DAC treatment was 4 (2-11) . A total of 74 patients achieved response (67.9%) and 30 (27.5%) achieved complete response (CR) . Univariate analysis found that CR was higher in patients with high risk of IPSS, complex karyotypes, monosomal karyotypes, chromosome 7 abnormality, and platelet doubling after one cycle of DAC treatment. Patients with TP53 gene mutation were more likely to receive CR, 10 of 15 patients with TP53 mutations achieved CR. 7.The grey area of HBsAg enzyme-linked immunosorbent assay (ELISA) in blood screening laboratory

【Objective】 To evaluate the necessity and rationality of setting 0.8 CO of the gray area of hepatitis B surface antigen(ELISA) reagent in our laboratory. 【Methods】 1) 792 samples of serum plates from the Clinical Laboratory Centre, Ministry of Health (NCCL) were tested by two HBsAg ELISA reagents. The true positive rate, and confirmed positive rate of gray area samples revealed by 2 reagents were calculated. ROC curve was drawn to determine the best CO value of 2 reagents. The changes in sensitivity and specificity under different CO values were compared. 2) Based on previous data, the HBV-DNA-yield rate among HBsAg gray area samples was analyzed, and the relationship between the distribution of ELISA results of solo HBV-DNA positive samples and gray area was analyzed. 【Results】 Of the 792 samples that form NCCL, 587 were positive, 197 negative and 8 indeterminate. The true positive detection rates of reagents A and B were 82.45% and 71.89%. The confirmed positive rate of gray area samples given by 2 reagents were 94.74%(18/19)and 93.10%(27/29). The best CO values of reagents A and B are 0.49 and 0.27, which are both lower than the 0.8. The specificity corresponding to the best CO values of the two reagents decreased slightly, but the sensitivity increased greatly. From January 2015 to January 2019, 183 551 samples were tested. Of the 13 cases of HBsAg gray area samples, 7 were revealed by reagent A and 1 was positive for NAT; 6 were revealed byreagent B and all negative for NAT. Out of 134 cases of solo HBV-DNA positive samples, 96.27% (129/134) samples had S/CO values below 0.4, overlapped with negative samples, and were far from 0.8. 【Conclusion】 It is necessary to set gray area for these two HBsAg ELISA reagents. The gray area value setting to 0.8 CO corresponds to poor reagent sensitivity. The best cutoff value would be selected according to the ROC curve: 0.49 CO in Reagent A and 0.27 CO in Reagent B. Gray area has no obvious effect on screening of single-virus NAT-yield sample.
8.Characteristics of HBcrAg in HBsAg-/HBV DNA+ blood donors in Wuxi area

【Objective】 To analyze the detection characteristics of a novel serum marker, hepatitis B core-associated antigen (HBcrAg), in the HBsAg-/HBV DNA+ blood donors in Wuxi. 【Methods】 A total of 37 previous HBsAg-/HBV DNA+ blood donors were followed up by telephone and their serum was obtained, and the serum of 22 HBsAg-/HBV DNA+ blood donors was detected by electrochemiluminescence and real-time PCR nucleic acid screening as the OBI group for HBcrAg enzyme-linked immunosorbent assay(ELISA). The serum of 20 healthy blood donors who underwent dual ELISA and one nucleic acid testing(NAT) was selected as the healthy control group, and the serum of 20 patients with chronic hepatitis B who were clinically diagnosed by Wuxi Fifth People's Hospital was selected as the experimental CHB group, and HBcrAg ELISA was detected respectively. The correlation analysis between HBcrAg and HBeAb, HBcAb, ALT and HBV DNA in the OBI group was performed. 【Results】 Thirty-seven blood samples were detected by chemiluminescence for HBsAg and NAT, and 22 HBsAg-/HBV DNA+ samples were detected in the OBI group, with a detection rate of 59.46%. The serum HBcrAg expression content (ng/mL) between the OBI group, the healthy control group and the CHB group were (0.92±0.13), (0.47±0.09) and (1.14±0.23), respectively, and the differences were statistically significant (P<0.05), the expression of HBcrAg in the OBI group and CHB group was higher than that in the healthy control group (P<0.05). There was no correlation between HBcrAg and HBeAb, HBcAb, ALT and HBV DNA indexes in the OBI group (P>0.05). 【Conclusion】 The expression of HBcrAg in the OBI group and CHB group was higher than that in the healthy control group, and the serum HBcrAg was not correlated with HBeAb, HBcAb, ALT and HBV DNA to a certain extent. HBcrAg has a good application prospect in screening HBsAg-/HBV DNA+ blood donors.
9.Analysis of blood component transfusion and some adverse reactions in myelodysplastic syndroms patients

【Objective】 To analyze the blood transfusion and adverse reactions in myelodysplastic syndroms (MDS) patients, so as to improve transfusion management in MDS patients. 【Methods】 The diagnosis and treatment information of MDS patients with blood transfusion in our hospital from January 2003 to December 2022 were collected, and the component transfusion and adverse reactions were investigated. 【Results】 The average infusion volume of red blood cells(RBCs) and platelets were respectively (27.46±43.11 ) and (16.41±24.81 ) in 799 MDS patients, which had no correlation with gender and blood type. The incidence of adverse reactions was 18.27% (146/799), with the most common adverse reactions as delayed serologic transfusion reaction (DSTR) (9.01%, 72/799), followed by non hemolytic fever reaction (4.76%, 38/799) and allergic reaction (4.38%, 35/799). Compared with all patients with transfusion, DSTR was more common in females (P<0.05), with elder age and had more RBCs consumption (all P<0.01). 86.11%(62/72) were Rh system, and 40.28% (29/72) had 2 or more antibodies. The occurrence time of DSTR in some patients was not related to the volume of RBCs trans infusion. 【Conclusion】 MDS patients, with more average transfusion volume and higher incidence of adverse reactions especially DSTR, were recommended a strictly limited transfusion schedule and Rh phenotype matching RBC products. The investigation of immune status of MDS patients at different periods is helpful to provide new aspects and therapeutic measures for the pathogenesis of DSTR, and the antibody screening time may adjusted appropriately.

Objective:To summarize and analyze the clinical data of Chinese patients with colony-stimulating factor 1 receptor (CSF1R)-related leukoencephalopathy, and clarify the phenotypic and genetic characteristics of Chinese patients.Methods:Medical history of patients with CSF1R-related leukoencephalopathy diagnosed from April 1, 2018 to January 31, 2021 in the department of neurology of 22 hospitals in China was collected, and scores of Mini-Mental State Examination (MMSE), Montreal Cognitive Assessment Scale (MoCA), magnetic resonance severity scale were evaluated. Group comparison was performed between male and female patients.Results:A total of 62 patients were included, and the male-female ratio was 1∶1.95. The age of onset was (40.35±8.42) years. Cognitive impairment (82.3%, 51/62) and motor symptoms (77.4%,48/62) were the most common symptoms. The MMSE and MoCA scores were 18.79±7.16 and 13.96±7.23, respectively, and the scores of two scales in male patients (22.06±5.31 and 18.08±5.60) were significantly higher than those in females (15.53±7.41 , t=2.954, P=0.006; 10.15±6.26, t=3.328 , P=0.003). The most common radiographic feature was bilateral asymmetric white matter changes (100.0%), and the magnetic resonance imaging severity scale score was 27.42±11.40, while the white matter lesion score of females (22.94±8.39) was significantly higher than that of males (17.62±8.74 , t=-2.221, P<0.05). A total of 36 CSF1R gene mutations were found in this study, among which c.2381T>C/p.I794T was the hotspot mutation that carried by 17.9% (10/56) of the probands. Conclusions:The core phenotypic characteristics of CSF1R-related leukoencephalopathy in China are progressive motor and cognitive impairment, with bilateral asymmetrical white matter changes. In addition, there exist gender differences clinically, with severer cognitive impairment and imaging changes in female patients. Thirty-six CSF1R gene mutations were found in this study, and c.2381T>C/p. I794T was the hotspot mutation.
