1.Correlation between endoscopic classification and antibiotic resistance of Helicobacter pylori in 5 441 patients with benign gastric diseases

Objective:To analyze the relationship between the gastric diseases classification under endoscopy and infection and antibiotic resistance of Helicobacter pylori ( H. pylori) based on the data of gastric diseases classification under endoscopy, H. pylori isolation and antibiotic resistance. Methods:From January 1, 2015 to December 31, 2019, endoscopic diagnosis and the data of drug susceptibility of H. pylori of 5 441 patients with benign gastric diseases along with gastrointestinal symptoms who visited The First People′s Hospital of Putuo District of Zhoushan were retrospectively collected. The H. pylori infection in patients with different genders and different types of gastric diseases and the drug resistance rates of H. pylori among different antibiotic types were analyzed. Chi-square test was used for statistical analysis. Results:The positive rate of H. pylori in 5 441 patients with benign gastric diseases was 51.37% (2 795/5 441). The drug resistance rates of clarithromycin, levofloxacin, metronidazole, amoxicillin, tetracycline and furazolidone were 26.73% (747/2 795), 43.22% (1 208/2 795), 98.68% (2 758/2 795), 0.32% (9/2 795), 0 and 0, respectively. Among the 2 795 cases of H. pylori-positive patients, only 23 cases (0.82%) were sensitive to all six antibiotics; 1 263 cases (45.19%) were resistant to single antibiotic; 1 072 cases (38.35%) were resistant to combination of two antibiotics, and 433 cases (15.49%) were resistant to combination of three antibiotics. The drug resistance rate to three antibiotics, clarithromycin, levofloxacin and metronidazole simultaneously was the highest (15.31%, 428/2 795). And only four patients (0.14%) were simultaneously resistant to four antibiotics clarithromycin, levofloxacin, metronidazole and amoxicillin. H. pylori infection rate in men was higher than that in women (55.23%, 1 615/2 924 vs. 46.88%, 1 180/2 517), and the difference was statistically significant ( χ2=37.76, P<0.01). However there were no statistically significant differences in the drug resistance rates of six antibiotics between different gender among H. pylori-positive patients (all P>0.05). There was statistically significant difference in the positive rate of H. pylori among patients with different types of gastric diseases ( χ2=909.07, P<0.01). The positive rate of H. pylori was the highest in patients with duodenal ulcer (97.53%, 79/81), and the lowest in patients with bile reflux gastritis (19.79%, 37/187). There were statistically significant differences in the drug resistance rates of metronidazole, clarithromycin, levofloxacin and amoxicillin among 9 types of gastric diseases ( χ2=24.75, 38.62, 80.54 and 7.01, all P<0.01). Conclusions:The positive rate of H. pylori and antibiotic resistance rate are different in patients with different types of gastric diseases. It is necessary to pay attention to the selection of drugs for ulcerative diseases in the process of clinical treatment. Individualized treatment is recommended according to the results of antibiotic susceptibility test.
2.Protective Effects of Opioid Preconditioning on Rabbits Donor Heart Preservation

Objective To study the effects of D-Ala2-Leu5-enkephalin(DADLE) preconditioning on rabbit donor heart preservation and protective mechanism.Methods 24 rabbits were randomly divided into 4 groups. In group B, after preconditioning with DADLE(1mg?kg -1 ), the donor hearts were then arrested and preserved with Krebs-Henseleit buffer at 4℃ for 4h,and group A without conditioning as controls. Then the donor hearts were transplanted into the abdomen of recipient rabbits. Recovery situation of contraction of the donor heart was compared. The left ventricular tissues were obtained from the donor hearts after 2h of transplantation.The contents of free radicals and ATPase activity were determined and cardiac ultrastructure were observed.Results After 4h cold storage, the heart undergoing DADLE preconditioning in group B showed better recovery of left ventricular development pressure (LVDP).The activity of sodium-potassium ATPase in group B were higher than that in group A. DADLE increased free radical production 2-fold versus group A. Group B showed slighter injury in transmission electron microscopy observation than that in group A.Conclusion Our study demonstrated opioid preconditioning has protective function to rabbit heart injury caused by long term cold storage. The protective mechanism maybe related to increase of free radical content.
3.Experimental Study on Electro-acupuncturing Baihui and Sanyinjiao on REMl Sleep Deprivation (SD) Actions

[Objective] To investigate the effects of REMl sleep deprivation (SD) on actions to investigate the efficacy of application of electro-acupuncture for treating sleep deprivation.[Methods]Thirty healthy SD rats randomly allocated to three groups (normal-control,model (A1),electro-treatment groups).The rats of model (A1)group and electro-treatment group were induced in male rats by housing them on small platforms overwater,normal-control group rats were on big platforms overwater.After electro-treatment for electro-treatment groups,the changes of normal status and Open Field Test were measured with all rats.[Results]Electro-acupuncture treatment can improve the actions.[Conclusion]Application of electro-acupuncture can produce a fighting effect on sleep deprivation.The effect may be related to promoting the nerval endocrine cytokine of electro-acupuncture.
4.Research Progress on Fungi and Mycotoxin Infection of Chinese Medicinal Materials

Chinese medicinal materials are likely to be polluted by various kinds of fungal toxins during the plantation,collection,transportation and storage,which results in the possibility that quality,safety and treatment effect fails to meet the requirement of safe medicine.This paper reviewed the domestic and overseas studies on the pollution of fungus and fungal toxins in herbs,and made an introduction to the limit standard of fungus and fungal toxins.This paper discussed the importance to establish the inspection standard of various fungal toxins in Chinese medicinal materials and their limit standards to provide references to medication safety and improvement of quality control standard system of Chinese medicinal materials.
5.Effects of high-fat diet on skin wound rep air in mice:an experimental study

Objective To evaluate the effects of long-term high-fat diet on skin wound repair in mice, and to explore its related mechanism. Methods A total of 16 ten-week-old C57BL/6J wild-type mice were randomly and equally divided into two groups to be fed a high-fat diet(HFD group)and a standard diet(SD group)respectively for 8 weeks. Then, an 8-mm full-thickness skin wound was created on the back of each mouse by using a biopsy punch. The degree of wound healing was observed, wound healing rate and epithelialization level were evaluated every day. The weight of mice was measured before feeding, after 8-week feeding and on day 14 after the operation. Peripheral blood samples were obtained from these mice for the determination of total cholesterol(TC)and triglyceride(TG)levels after a 12-hour fast on day 14 after the operation. Then, all the mice were sacrificed, and wound tissues were resected from the dorsal skin of mice for a histological study. The two-sample t test was used to compare the thickness of new epidermis in the wound surface, collagen deposition rate in the wound bed, count of new vessels, levels of cell proliferation and degree of inflammatory cell infiltration between the two groups. Results The average weight of mice was significantly higher in the HFD group than in the SD group after 8-week feeding(27.3 ± 0.7 g vs. 21.2 ± 0.6 g, t = 21.98, P < 0.001)and on day 14 after the operation (28.8 ± 0.7 g vs. 23.1 ± 1.1 g, t = 25.22, P < 0.001). Similarly, there was a significant increase in the levels of TC(1.35 ± 0.32 mmol/L vs. 0.99 ± 0.28 mmol/L, t = 2.24, P < 0.05)and TG(4.21 ± 0.41 mmol/L vs. 2.71 ± 0.31 mmol/L, t = 6.49, P < 0.05)in the HFD group compared with the SD group 14 days after the operation. Compared with the SD group, the HFD group showed shorter healing time (13.5 ± 0.5 days vs. 12.6 ± 1.1 days, t = 1.99, P < 0.05), lower thickness of newborn epidermis on the wound surface (47.8 ± 13.8 μm vs. 95.7 ± 13.7 μm, t = 5.68, P < 0.001), decreased number of CD31-positive vessels(8 ± 1 vs. 13 ± 3, t = 4.1, P < 0.001)and count of ki-67-positive cells(21 ± 4 vs. 49 ± 10, t = 3.33, P < 0.001), but increased count of infiltrating macrophages and mast cells (both P < 0.05). No significant difference was found in collagen deposition rate in the wound bed between the two groups (P > 0.05). Conclusion Long-term high-fat diet can affect wound healing and delay skin wound repair in mice.
6.Determination of Vitamin B_1 and Vitamin B_6 in Gengnianling Capsules by HPLC Simultaneously

OBJECTIVE:To determinate vitamin B1 and vitamin B6 in Gengnianling capsules by HPLC simultaneously .METHODS: The separation was performed on Hypersil-ODS C18 column, methanol - sodium hexanesulfonate solution(20 : 80) was used as mobile phase with a flow rate of 0.8ml/ min and detection wavelength of 280nm.RESULTS: Linear correlations with peak area scores were achieved when the sample size of vitamin B1 and vitamin B6 were with a range of 0.884?g-2.652?g (r = 0.9 999) and 0.714?g-2.142?g(r = 0.9 999) .respectively, the average recovery of which were 95.87%(RSD = 0.82%) and 101.96% (RSD = 0.86%), respectively .CONCLUSION: The method is simple, accurate and it can be used for quality control of Gengnianling Capsule.
7.Pretreatment with Xuebijing injection alleviates systemic inflammatory response induced by severe heat-stroke via ameliorating intestinal injury in rats

ObjectiveTo observe the effect of Xuebijing injection pretreatment on systemic inflammatory response induced by severe heat-stroke, and to investigate the mechanism of alleviation of intestinal injury in rats. Methods Thirty-six healthy adult male Wistar rats with grade SPF were randomly assigned into three groups with randomized number method, namely sham group, severe heat-stroke model group, and Xuebijing pretreatment group (XBJ group), with 12 rats in each group. The animals were placed in a pre-warm chamber [temperature (40±2)℃, humidity (65±5)%] in order to induce typical heat-stroke. The duration of heat-stress was 60 minutes, while the animals in sham group were exposed to ambient temperature of 25℃. Arterial blood samples were collected at the beginning and the end of heat-stress, the concentrations of tumor necrosis factor-α(TNF-α), interleukins (IL-1β, IL-6), and lipopolysaccharide (LPS) in peripheral blood were determined by enzyme linked immunosorbent assay (ELISA). The intestinal tissues were harvested after heat-stress, and the pathological changes in intestine tissues were observed after hematoxylin-eosin (HE) staining and under optical microscope. The pathological injury scores were calculated. Immunohistochemistry was performed to determine inducible nitric oxide synthase (iNOS) expression in intestinal tissue. Apoptosis was determined by terminal deoxynucleotidyl transferase-mediated dUTP nick end labeling (TUNEL) staining. Western Blot was used to measure the tight junction protein occludin expression.Results The concentrations of TNF-α, IL-1β, IL-6 and LPS in blood of the rats after heat-stress in model group were significantly higher than those of sham group [TNF-α (μg/L): 443.00±110.10 vs. 98.36±44.61, IL-1β (μg/L): 436.37±163.64 vs. 64.24±16.15, IL-6 (μg/L): 342.70±92.42 vs. 54.40±13.22, LPS (μg/L): 0.68±0.22 vs. 0.09±0.02, allP< 0.01], but the levels of these parameters in XBJ group were significantly lower than those of model group [TNF-α (μg/L):340.45±68.57 vs. 443.00±110.10, IL-1β (μg/L): 191.33±82.78 vs. 436.37±163.64, IL-6 (μg/L): 192.21±37.89 vs. 342.70±92.42, LPS (μg/L): 0.43±0.17 vs. 0.68±0.22, allP< 0.01]. Infiltration of inflammatory cells, necrosis and hemorrhage in intestinal mucosa were found in the intestine of heat-stroke animals in model group. The pathological lesions in XBJ group were milder than those of model group, with a decreased pathological injury score compared with model group (2.10±1.15 vs. 3.20±0.67,P< 0.01). The expression of iNOS and apoptosis of cells in intestinal tissue in model group were increased compared with that of sham group, but they were significantly less marked in XBJ group compared with model group [iNOS (adjustedA value): 0.32±0.15 vs. 0.74±0.17, apoptotic index: 0.23±0.08 vs. 0.56±0.07, bothP< 0.01]. The order of expression for occludin protein from high to low was sham group, XBJ group and model group (A value was 0.96±0.25, 0.62±0.20, 0.33±0.11, respectively). Furthermore, there was significant difference in the expression of occludin protein between model group and both XBJ group and sham group (bothP<0.01).Conclusions Xuebijing injection alleviates inflammation and endotoxemia produced by severe heat-stroke in rats. The mechanism may be related to amelioration of oxidative injury, apoptosis, and dysfunction of tight junction protein occludin expression.

9.Characteristics of acupoints selection of moxibustion for primary dysmenorrhea based on data mining technology.

By using data mining technology, the characteristics of acupoints selection of modern moxibustion for primary dysmenorrhea (PD) were analyzed. The modern literature regarding moxibustion for PDl was coll6cted~ from a comprehensive retrieval in Chinese Biomedical Medicine (CBM), China National Knowledge Infrastructure (CNKI), VIP and PubMed. The prescription database of moxihustion was established, and the association rule4 method of data mining technology was adopted to analyze characteristics and rules of acupoints selection of modern moxibustion for PD. As a result, a total of 35 acupoints were involved with a total frequency of 399. The acupoints, with higher selected frequency included Guanyuan (CV 4, 90 times), Sanyinjiao (SP 6, 47 times), Shenque (CV 8, 43 times), Zhongji (CV 3, 3 times) and Qihai (CV 6, 28 tirmes); the ratio of yin-meridian acupoints to1 yang-meridian acupoints was approximately 341; the utilization of the specific acupoints accountedfor68. 6% (24/35); the crossing acupoints were used mostly (203 "times), featured with high acupoints selected, especially Guanyuan (CV 4), Sanyinjiao CSP 6) and Zhongji (CV 3); the front-mu points (130 times), he-sa points (19 times) and lower he-sea points (18 times) were frequently used. The results reveal thiat acupoints selection of moxibustion of PD focuses on the local acupoints and comnbination with the distal acupoints. The yin-meridian acupoints are preferred and the specific acupoints are the main part of the prescription. This conclusion can also be used to optimize the choosing of acupoints in clinic.

10.Effect of ulinastatin on acute lung injury induced by severe heat stroke and its mechanism

Objective To investigate the effect ofulinastatin on severe heat-stroke with acute lung injury induced by severe heat stroke.Methods Thirty severe heat stroke patients were divided into conventional group (n=15) and ulinastatin group (n=15) randomly,with another 80 healthy adults serving as controls.The baseline data such as age,gender,onset period and APACHE Ⅱ scores were recorded and compared between the two groups on admission.Peripheral leucocyte counts,oxygenation index and Murray scores were determined on the 1st,3rd and 5th day.The concentration of inflammatory mediators in bronchoalveolar lavage fluid (BALF) and alveolar macrophage supernatant were detected by enzyme-linked immunosorbent assay (ELISA).Western blotting and real-time PCR were used to measure expression of triggering receptor-1 on myeloid cells (TREM-1) on alveolar macrophages.Furthermore,comparison was made in terms of the ventilation period,ICU stay time and mortality in 28 days between the two groups.Results No differences were found in age,gender,onset period and APACHE Ⅱ scores between the two groups (P>0.05).Compared with the conventional group,peripheral leucocyte counts and Murray scores in the ulinastatin group significantly decreased on the 3rd and 5th day (P<0.05,P<0.01).But oxygenation index was higher in the ulinastatin group than in the conventional group (P<0.05).The concentration of TNF-α and IL-6 in BALF was lower in the ulinastatin group than in the conventional group (on the 3rd day:P<0.05,P<0.01;on the 5th day:P<0.01,P<0.01).The concentration of TNF-α and IL-6 in alveolar macrophage supernatant was lower in the ulinastatin group than in the conventional group (on the 3rd day:-P<0.05,P<0.01;on the 5th day:P<0.01,P<0.05).The expression of TREM-1 protein on alveolar macrophages were lower in the ulinastatin group than in the conventional group (on the 3rd day P<0.01;on the 5rd day P<0.05).TREM-1 mRNA was lower in the ulinastatin group than in the conventional group (on the 3rd day:P<0.05;on the 5th day:P<0.05).Eventually,the treatment of ulinastatin shorten ventilation period and ICU stay time (P<0.01,P<0.05).Nonetheless,it failed to show any improvement in terms of the mortality during 28 days (P>0.05).Conclusion Our study exhibited that ulinastatin had good effect on the heat stroke patients with acute lung injury and it helped reduce the inflammatory reaction of pulmonary tissues.The underlying mechanism of these effects might lie in its ability to reduce heat stroke-induced inflammatory secretion and expression of TREM-1 on alveolar macrophage.
