1.Distribution of Asymmetrical Hearing Loss Among the Workers participated in Noise-specific Health Examination.

OBJECTIVES: Noise-induced hearing lose(NIHL) is characterized by bilaterally symmetrical hearing loss, but some screening audiometries of employees who were exposed noise showed asymmetry. Therefore, this study was carried out to evaluate distribution of asymmetrical hearing loss and factors influencing asymmetries. METHODS: Study subjects were 294 male employees who have participated in 1st and 2nd noise-specific health examination. RESULTS: The interaural threshold difference of 20 dB or more at 4,000 Hz was classified as asymmetry. Among 294 NIHL employees. 19% had left asymmetric hearing loss. 22% had right asymmetric hearing loss. and overall asymmetry were 41%. Prevalence of asymmetry at 4,000 Hz significantly more decreased in middle(80~84 dB (A)) and high-level noise exposure groups(> or =85 dB(A)) than low-level exposure group and significantly increased as greater hearing threshold level in the worse ear. Prevalence of left asymmetry at 4,000 Hz significantly increased as greater hearing threshold level in the worse ear than right asymmetry. The interaural difference was significantly greater in the left asymmetry. CONCLUSIONS: Noise exposure produced asymmetric hearing loss(interaural asymmetry was 20 dB and more in 41% of case) and left ear was more susceptible to noise demage than right ear.

2.The Effect of Parental Socioeconomic Status on the Nutrient Intake of Urban and Rural Adolescents.

This study was conducted to assess the nutrient intake patterns among urban and rural adolescents and to investigate the effects due to parent's socioeconomic status and other factors, such as mother's job, family type and regular exercise on that pattern. 2,455 middle and high school students living in Seoul and Yangpong, Kyounffi-Do participated in a self-administered questionnaire that was used to collect data. The one-day dietary intake was surveyed through a 24-hour recall method. The factors significantly different between urban and rural adolescents according to monthly income, parent's education level, mother's job, family type and exercise. Income, the parents' education level and regular exercise were associated with the patterns of nutrient intakes as a percent of the RDA. So, when adjusted for parental income, the father's and mother's education level and regular exercise, there were no signifcant differences within the patterns of nutritional intake between urban and rural adolescents. The results provided the information regarding the determinants of nutrient status among adolescents and were expected to be helpful for planning school health promotion programs.

3.The effect of ephedrine on intraoperative hypothermia.

BACKGROUND: Prevention of intraoperative hypothermia has become a standard of operative care. Since ephedrine has a thermogenic effect and it is frequently used to treat hypotension during anesthesia, this study was designed to determine the effect of ephedrine on intraoperative hypothermia of patients who are undergoing spine surgery. METHODS: Twenty-four patients were randomly divided to receive an ephedrine (the ephedrine group, n = 12) or normal saline (the control group, n = 12) infusion for 2 h. The esophageal temperature (the core temperature), the index finger temperature (the peripheral temperature) and the hemodynamic variables such as the mean blood pressure and heart rate were measured every 15 minutes after the intubation. RESULTS: At the end of the study period, the esophageal temperature and hemodynamic variables were significantly decreased in the control group, whereas those in the ephedrine group were stably maintained. The index finger temperature was significantly lower in the ephedrine group compared to that in the control group, suggesting the prevention of core-to-peripheral redistribution of the heat as the cause of temperature maintenance. CONCLUSIONS: An intraoperative infusion of ephedrine minimized the decrease of the core temperature and it stably maintained the hemodynamic variables during spine surgery with the patient under general anesthesia.

4.Doppler ultrasonography measurement of hepatic hemodynamics during Valsalva maneuver: healthy volunteer study.

PURPOSE: The aim of our study was to assess the hemodynamic change of liver during the Valsalva maneuver using Doppler ultrasonography. METHODS: Thirty healthy men volunteers were enrolled (mean age, 25.5+/-3.64 years). The diameter, minimal and maximal velocities, and volume flow of intrahepatic inferior vena cava (IVC), middle hepatic vein (MHV), and right main portal vein (RMPV) was measured during both rest and Valsalva maneuver. These changes were compared using paired t-test. RESULTS: The mean diameters (cm) of the intrahepatic IVC at rest and Valsalva maneuver were 1.94+/-0.40 versus 0.56+/-0.66 (P<0.001). The mean diameter (cm), minimal velocity (cm/sec), maximal velocity (cm/sec), and volume flow (mL/min) of MHV at rest and Valsalva maneuver were 0.60+/-0.15 versus 0.38+/-0.20 (P<0.001), -7.98+/-5.47 versus 25.74+/-13.13 (P<0.001), 21.34+/-6.89 versus 35.12+/-19.95 (P=0.002), and 106.94+/-97.65 versus 153.90+/-151.80 (P=0.014), respectively. Those of RMPV at rest and Valsalva maneuver were 0.78+/-0.21 versus 0.76+/-0.20 (P=0.485), 20.21+/-8.22 versus 18.73+/-7.43 (P=0.351), 26.79+/-8.85 versus 24.93+/-9.91 (P=0.275), and 391.52+/-265.63 versus 378.43+/-239.36 (P=0.315), respectively. CONCLUSION: The blood flow velocity and volume flow of MHV increased significantly during Valsalva maneuver. These findings suggest that hepatic vein might play an important role to maintain venous return to the heart during the maneuver.

5.Tumor Margin Infiltration in Soft Tissue Sarcomas: Prediction Using 3T MRI Texture Analysis

Purpose:
To determine the value of 3 Tesla (T) MRI texture analysis for predicting tumor margin infiltration in soft tissue sarcomas.
Materials and Methods:
Thirty-one patients who underwent 3T MRI and had a pathologically confirmed diagnosis of soft tissue sarcoma were included in this study. Margin infiltration on pathology was used as the gold standard. Texture analysis of soft tissue sarcomas was performed on axial T1-weighted images (WI) and T2WI, fat-suppressed contrast-enhanced (CE) T1WI, diffusion-weighted images (DWI) with b-value of 800 s/mm², and apparent diffusion coefficient (ADC) was mapped. Quantitative parameters were compared between sarcomas with infiltrative margins and those with circumscribed margins.
Results:
Among the 31 patients with soft tissue sarcomas, 23 showed tumor margin infiltration on pathology. There were significant differences in kurtosis with the spatial scaling factor (SSF) of 0 and 6 on T1WI, kurtosis (SSF, 0) on CE-T1WI, skewness (SSF, 0) on DWI, and skewness (SSF, 2, 4) on ADC between sarcomas with infiltrative margins and those with circumscribed margins (p ≤ 0.046). The area under the receiver operating characteristic curve based on MR texture features for identification of infiltrative tumor margins was 0.951 (p < 0.001).
Conclusion
MR texture analysis is reliable and accurate for the prediction of infiltrative margins of soft tissue sarcomas.
6.Benign versus Malignant Soft-Tissue Tumors: Differentiation with 3T Magnetic Resonance Image Textural Analysis Including Diffusion-Weighted Imaging

To investigate the value of MR textural analysis, including use of diffusionweighted imaging (DWI) to differentiate malignant from benign soft-tissue tumors on 3T MRI.
Materials and Methods:
We enrolled 69 patients (25 men, 44 women, ages 18 to 84 years) with pathologically confirmed soft-tissue tumors (29 benign, 40 malignant) who underwent pre-treatment 3T-MRI. We calculated MR texture, including mean, standard deviation (SD), skewness, kurtosis, mean of positive pixels (MPP), and entropy, according to different spatial-scale factors (SSF, 0, 2, 4, 6) on axial T1-and T2-weighted images (T1WI, T2WI), contrast-enhanced T1WI (CE-T1WI), high b-value DWI (800 sec/mm2 ), and apparent diffusion coefficient (ADC) map. We used the Mann-Whitney U test, logistic regression, and area under the receiver operating characteristic curve (AUC) for statistical analysis.
Results:
Malignant soft-tissue tumors had significantly lower mean values of DWI, ADC, T2WI and CE-T1WI, MPP of ADC, and CE-T1WI, but significantly higher kurtosis of DWI, T1WI, and CE-T1WI, and entropy of DWI, ADC, and T2WI than did benign tumors (P < 0.050). In multivariate logistic regression, the mean ADC value (SSF, 6) and kurtosis of CE-T1WI (SSF, 4) were independently associated with malignancy (P ≤ 0.009). A multivariate model of MR features worked well for diagnosis of malignant soft-tissue tumors (AUC, 0.909).
Conclusion
Accurate diagnosis could be obtained using MR textural analysis with DWI and CE-T1WI in differentiating benign from malignant soft-tissue tumors.

8.Ten-Year Trends of Hematopoietic Stem Cell Transplantation in Korean Pediatric Cancer from the National Health Insurance Claims Data

Purpose:
We aimed to determine the current application and survival trends of hematopoietic stem cell transplantation (HSCT) among Korean children and adolescents with cancer.
Materials and Methods:
Data of patients aged < 20 years with KCD-10 (Korean Classifications of Diseases, 10th revision) C codes and specific designation codes were collected from the National Health Insurance Service database. Thirty claim codes for HSCT were included, and data from 2009 to 2019 were analyzed.
Results:
The operational definition of pediatric cancer yielded an annual average of 2,000, with annual cases decreasing. In 2019, 221 HSCTs were performed, a decrease from the ten-year average of 276. Allografts outnumbered autografts with a ratio of 1.5:1. The source of allograft was bone marrow in 15% of patients in 2009; however, it substantially decreased to 3.3% in 2019. Furthermore, 70.5% of allogeneic HSCT used peripheral blood stem cell (PBSC) grafts, which increased to 89.3% by 2015. Cord blood utilization markedly decreased to 2.7% in 2018. The 5-year overall survival (OS) rate of all patients was 85.1%. Overall mortality decreased among patients who underwent recent HSCT, and they exhibited a higher 5-year OS rate.
Conclusion
In Korea, the number of pediatric patients with cancer is declining; however, the ratio of transplants to all patients remains constant. Patients who recently underwent transplantation showed better survival rates, possibly due to HSCT optimization. Korea showed a substantially greater PBSC utilization in pediatric HSCT. An in-depth examination encompassing donor relations and cause of death with a prospective registry is required in future studies.
