1.An Electroretinographic Changes of Laser-induced Experimental Branch Retinal Vein Occlusion in the Rabbits.

To verify whether laser-induced branch retinal vein occlusion in the rabbit can be used as the experimental model for the human study. The temporal retinal vein of the right eye was occluded with argon laser photocoagulator with parameters of 500 micrometerspot size, 0.2 second duration, 150-200 microWatt power in twelve pigmented rabbits. The left eye was used as a control. An electroretinogram was taken before and 1,3,7,15 and 28 days after vein occlusion. B/a ratio and oscillatory potentials increased in the eyes with occluded veins. Comparing the pre-occlusion values with those of post-occlusion, the changes were not statistically significant. But differences between the values of right and those of left eye were significant. We can suppose that the discrepancies may result from the difference in timing of examination. In conclusion, the rabbit model of laser-induced experimental branch retinal vein occlusion may not be adequate for the study of clinical branch retinal vein occlusion.

2.A comparative Study of SPECT with MRI in Internal Derangement of the Knee.

Bone single photon emission computed tomography(SPECT) of the knees has been shown to be of diagnostic value for the investigation of intemal derangement of knees(IDK). This study compared bone SPECT and magnetic resonance image(MRI) with arthroscopic findings for the diagnosis of IDK. Fifty patients who had been performed arthroscopy in Seoul National University Hospital from January 1997 to September 1997 were studied with both the modalities preoperatively. There were 38 meniscal injuries, 26 cruciate ligament injuries and 8 cases of patellofemoral joint problems. In view of SPECT, the most remarkable results were from meniscal tears. In meniscal tears, the diagnostic values of SPECT were 89.5% of sensitivity, 66.7% of specificity, 84% of diagnostic accuracy, which were comparable to those of MRI, 97.4%, 91.7% and 96%, respectively. But SPECT showed inferior diagnostic values in cruciate ligament injuries. For anterior knee pain, SPECT was more sensitive than MRI for the detection of pathology. And for functionally related cases, such as ACL impingement to intercondylar notch, SPECT was far more valuable than MRI. Bone SPECT will not provide the anatomical detail of MRI but seems to be a useful tool for the detection of wide range of knee disorders including IDK. Further research is justified to investigate the precise role of bone SPECT in clinical practice and its value in relation to MRI.

3.Comparison of Mineral and Vitamin Intakes According to the Stage of Change in Fruit and Vegetable Intake for Elementary School Students in Chungnam Province.

The purpose of this study was to compare the mineral and vitamin intake according to the stage of change in fruit and vegetable intake. The subjects consisted of 256 students, 122 males and 134 females, who are fourth, fifth and sixth grade in an elementary school located in Yeongi-Gun, Chungnam province. The dietary intake was examined by 24 hr dietary recall for 1 day and by food record for 2 days from April 19 to May 01, 2007. Stage of change of intake of fruits and vegetables of the students was categorized into three groups: precontemplation, contemplation and preparation, and action. The subjects at the stage of action took kimchi and vegetables more frequently, and also took more vitamin C as well. But the subjects at other two stages did not show any difference in the intake of any food group and nutrients. Percentage of the male subjects who took less than EAR did not show any significant difference by stage of change in all the nutrients. However, there was gradual decrease in the percentage of female taking less than EAR of vitamin C and vitamin B2. The result concludes that vitamin C intake significantly increase gradually as the stages of behavioral change of fruit and vegetable intake proceed although almost all vitamins and minerals tended to increase.

4.Choline Contents of Korean Common Foods.

Choline is important for normal membrane function, acetylcholine synthesis and methyl group metabolism. In this study, 185 food items customarily eaten by Koreans were selected from the data of the 2001 Korean National Health and Nutrition Survey and analyzed on the total choline content of the foods using enzymatic method of choline oxidase. Foods with high choline concentration (mg/100 g) were listed in sequence of quail egg (476.04 mg), dried squid (452.42 mg), beef liver (427.16 mg), pork liver (424.92 mg), tuna canned in oil (414.44 mg), boiled and dried anchovy (381.30 mg), dried Alaskan pollack (378.88 mg), chicken egg (309.88 mg), chicken liver (259.38 mg), soybean (238.62 mg), French bread with garlic (193.18 mg) and barley (183.73 mg). From this result, it is shown that dried fishes, prepared fishes, livers, eggs, pulses and cereals might be categorized as high choline food. Citron tea and green tea showed low choline content below 1 mg. Vegetables and fruits were also categorized into low choline food. No choline was detected in red pepper powder, beer, soju, soybean oil and corn oil out of foods analyzed in this study. Further study is required for analytic procedure of the foods of which results are inconsistent with USDA's data such as rice and wheat flour.

5.Clinical Information on Green Tea Extract Used for Weight Loss.

BACKGROUND: Green tea extracts are approved as nonprescription drug and available as health functional foods, health foods, and beverages. Clinical information on the products is lacking. METHODS: Information about the products on green tea nonprescription drugs was obtained from the website of the Korea Pharmaceutical Information Center. The Naver, i.e., a top ranking online search portal, was used for compiling the list of the health functional food products using key words of ‘green tea catechin’ on August 23, 2018. The recommended daily dosages of catechins were calculated as 30% of the total dried mass of green tea and about 50% of the catechins were considered as epigallocatechin gallate (EGCG). RESULTS: A total of two types of nonprescription drugs containing green tea powder or extracts, nine health functional food products, and three types of health foods were found. The regulatory requirements of the EGCG exceeding 800 mg were reported to be associated with adverse effects of elevated liver enzyme. If consumers take several green tea products concurrently, such as nonprescription drugs with health functional foods or health foods, it could exceed the recommended amount of EGCG. CONCLUSION: The concurrent use of green tea products as nonprescription drugs, health functional foods, and healthy foods may lead to an increased exposure to EGCG. Pharmacists should be aware the availability of various types of green tea products and the potential risk of liver toxicity due to excessive consumption of EGCG.

6.Maximum Meal Calorie Variation and Cardiovascular Risk Factors.

BACKGROUND: Diet pattern of regular and three meals per day is commonly recommended. Studies investigated the health effect of gorging pattern of diet using meal frequency and meal skipping, but the health effect of meal calorie variation between three regular meals has never been investigated. In this study, maximum meal calorie variation was defined as subtraction calorie for a meal with minimum energy intake from calories for a meal with maximum energy intake between three meals and examined the effect of maximum meal calorie variation between three regular meals a day on cardiovascular risk factors. METHODS: A total of 4,680 healthy subjects aged 20-87 years who underwent medical screening examination, at one tertiary hospital health screening center and completed 24-hour dietary recall was included. Serum cholesterol subfractions, fasting glucose and blood pressure were measured. RESULTS: Maximum meal calorie variation was significantly related to serum concentration of total cholesterol (beta = 1.77; 95% confidence interval [CI], 0.36 to 3.18) and low density lipoprotein-cholesterol (LDL-C) (beta = 1.64; 95% CI, 0.37 to 2.91), body mass index (beta = 0.24; 95% CI, 0.12 to 0.37) and waist circumference (beta = 0.66; 95% CI, 0.34 to 0.98) after adjustment for potential confounders. CONCLUSION: This study suggests the notion that concentration of total cholesterol and LDL-C and obesity indices are related to maximum meal calorie variation between three meals, independently of energy intake and other confounding factors in free-living population.

7.Changes of Bone Mineral Density and Biochemical Bone Markers during Perimenopausal Period for Healthy Women: Retrospective Cohort Study.

BACKGROUND: Although it is well known that bone mineral density (BMD) loss occurs after menopausal transition, there are only few previous studies that describe differences of BMD and biochemical bone markers in women of pre- and postmenopausal periods. The purpose of this study was to find factors that contribute to loss of BMD after menopause and to show changes of BMD and biochemical bone markers during pre- and postmenopausal periods by retrospective cohort study. METHODS: This retrospective cohort study was performed from Jan. 1995 to Jan. 2001 at a health promotion center. Twenty one healthy perimenopausal women were enrolled. BMD and biochemical bone markers were checked more than two times during the study period. Changes of BMD and biochemical bone markers between pre- and postmenopausal state were compared by paired t-test. Pearson correlation and multiple regression were performed to find the contributing factors to loss of BMD after menopause. RESULTS: Postmenopausal BMD (164.65 36.34 mg/cm3) was significantly decreased to 16.49 16.91 mg/cm3 (P<0.001) as compared with premenopausal BMD (181.14 40.81 mg/cm3). In biochemical bone markers only urine deoxypyridinoline had a significant difference (3.30 3.97 nMDP/mMcre, P<0.05) Only premenopausal BMD contributed to decreasing rate of BMD between the two states and the loss of BMD after menopause (P<0.05). CONCLUSION: In perimenopausal healthy women, postmenopausal BMD was significantly decreased as compared with premenopausal BMD. And only premenopausal BMD was shown to be a contributing factor to decreasing rate of BMD between the two states and the loss of BMD after menopause. It suggests that premenopausal BMD is important in predicting postmenopausal osteoporosis and efforts to prevent loss of BMD before menopause can prevent progress of postmenopausal osteoporosis.

8.Impact of graft composition on the systemic inflammatory response after an elective repair of an abdominal aortic aneurysm.

PURPOSE: The present study aimed to evaluate the risk factors and the role of graft material in the development of an acute phase systemic inflammatory response, and the clinical outcome in patients who undergo endovascular aneurysm repair (EVAR) or open surgical repair (OSR) of an abdominal aortic aneurysm (AAA). METHODS: We retrospectively evaluated the risk factors and the role of graft material in an increased risk of developing systemic inflammatory response syndrome (SIRS), and the clinical outcome in patients who underwent EVAR or OSR of an AAA. RESULTS: A total of 308 consecutive patients who underwent AAA repair were included; 178 received EVAR and 130 received OSR. There was no significant difference in the incidence of SIRS between EVAR patients and OSR patients. Regardless of treatment modality, SIRS was observed more frequently in patients treated with woven polyester grafts. Postoperative hospitalization was significantly prolonged in patients that experienced SIRS. In multivariate analyses, the initial white blood cell count (P = 0.001) and the use of woven polyester grafts (P = 0.005) were significantly associated with an increased risk of developing SIRS in patients who underwent EVAR. By contrast, the use of woven polyester grafts was the only factor associated with an increased risk of developing SIRS in patients who underwent OSR, although this was not statistically significant (P = 0.052). CONCLUSION: The current study shows that the graft composition plays a primordial role in the development of SIRS, and it leads to prolonged hospitalization in both EVAR and OSR patients.

9.A Retrospective 10-Year, Single-Institution Study of Carotid Endarterectomy with a Focus on Elderly Patients.

BACKGROUND AND PURPOSE: This study evaluated the outcome following surgery for carotid artery stenosis in a single institution during a 10-year period and the relevance of aging to access to surgery. METHODS: Between January 2001 and December 2010, 649 carotid endarterectomies (CEAs) were performed in 596 patients for internal carotid artery occlusive disease at our institution; 596 patients received unilateral CEAs and 53 patients received bilateral CEAs. Data regarding patient characteristics, comorbidities, stroke, mortality, restenosis, and other surgical complications were obtained from a review of medical records. Since elderly and high-risk patients comprise a significant proportion of the patient group undergoing CEAs, differences in comorbidity and mortality were evaluated according to age when the patients were divided into three age groups: <70 years, 70-79 years, and > or =80 years. RESULTS: The mean age of the included patients was 67.5 years, and 88% were men. Symptomatic carotid stenosis was observed in 65.7% of patients. The rate of perioperative stroke and death (within 30 days of the procedure) was 1.84%. The overall mortality rate was higher among patients in the 70-79 years and >80 years age groups than among those in the <70 years age group, but there was no significant difference in stroke-related mortality among these three groups. CONCLUSIONS: CEA over a 10-year period has yielded acceptable outcomes in terms of stroke and mortality. Therefore, since CEA is a safe and effective strategy, it can be performed in elderly patients with acceptable life expectancy.
