1.Measles Antibodies Measured by Plaque Reduction Neutralization Test in Infants during the First Year of Life.

PURPOSE: The 2000-2001 measles epidemic resulted in more than 50,000 cases with the highest attack rate occuring in infants less than one year of age, indicating the necessity of measles immunization before 12 months of age when a measles outbreak occurs again. The study was conducted to measure maternal measles antibody in infants by plaque reduction neutralization test(PRN), for the first time in Korea, to assess the optimal age for measles vaccination before the first birthday, when necessary. METHODS: Sera were obtained from 95 infants younger than 12 months of age who were healthy or recovered from mild llnesses, and had not had measles vaccination, measles infection, or blood transfusion. Measles antibodies were measured by PRN. RESULTS: Geometric mean titers and seropositive rates of measles antibody measured by PRN were 879.7 mIU/mL(100.0%), 690.0 mIU/mL(83.3%), 182.7 mIU/mL(50.0%), 91.3 mIU/mL(50.0%), 32.2 mIU/ mL(0.0)%, 25.1 mIU/mL(0.0%), 18.1 mIU/mL(0.0%), 38.4 mIU/mL(25.0%), 27.1 mIU/mL(0.0%), 31.2 mIU/mL(0.0%), 54.3 mIU/mL(0.0%), and 27.1 mIU/mL(0.0%) from 0 to 11 months respectively. CONCLUSION: By PRN, which was used for the first time to measure the measles antibody in Korea, placentally transferred measles antibody was detected in all newborns tested and decreased reciprocally to the age of infants, leaving almost all infants older than four months seronegative. These results indicate that measles vaccination at six months of age or older, which is the current recommendation during the period of epidemic issued by the Korean Society of Pediatrics, should not cause the primary vaccine failure. It seems advisable to utilize PRN further in order to find the optimal schedule for measles vaccination to infants born to women who were vaccinated.

Clostridium perfringens food poisoning ranks among the most common gastrointestinal diseases in developed countries. In Korea, C. perfringens food poisoning gradually increases. Using PCR, 72 strains of C. perfringens isolated in Seoul, 2013 were tested for the presence of toxin genes. Of the tested strains, 32 isolates carried the cpe gene, 37 isolates carried the cpb2 gene and 3 isolates carried the cpe and cpb2 genes, respectively. 32 cpe-positive strains were isolated from the food poisoning patient, whereas among 37 cpb2-positive strains, 22 strains were isolated from asymptomatic person. To investigate epidemiological relationship between the isolates, Pulsed-filed gel electrophoresis (PFGE) was performed. The genetic relatedness of the isolates ranged from 55.9% to 100% and 47 distinct PFGE profiles were observed. The results show that the cpe-positive outbreak strains showed close genetic relation, whereas the cpb2-positive isolates revealed a wide genetic diversity.

3.Epidemiological Relationship of Enterotoxigenic Escherichia coli and Enteroaggregative E. coli Isolated from Patients with Diarrhea in Seoul.

Of total 1,438 specimens of patients with diarrhea in Seoul, 2011, 217 samples (15%) were found pathogenic Escherichia coli that included 192 strains (89%) of enterotoxigenic E. coli (ETEC) and enteroaggregative E. coli (EAEC). The highest isolation rate for ETEC and EAEC was found in August and September. Sixty two pathogenic E. coli strains (34 ETEC and 28 EAEC strains) were selected from 175 strains (94 ETEC and 81 EAEC strains) isolated in August and September. Of 94 strains characterized for ETEC phenotype, 76 (81%) expressed heat-stable toxin (ST) only. Antimicrobial susceptibility test was carried out by using sixteen types of antibiotics. A high level of antimicrobial resistance to tetracycline (57%), ampicillin and ticarcillin (54%) was observed among EAEC isolates while the highest resistance rate of ETEC was found for nalidixic acid (47%), followed by tetracycline (32%). As to the antimicrobial susceptibility test, EAEC showed the complicated multi-drug resistant patterns in which the resistance was higher than ETEC. Pulsed-field gel electrophoresis (PFGE) was carried out to examine the genetic relatedness among ETEC and EAEC isolates. Except for 11 strains, 51 strains were divided by eight pulsotypes. In PFGE analysis, isolates from foodborne disease outbreaks in August and September 2011 showed close relation.

4.Infection Frequency and Mixed infection on Eight Viruses from Patients with Acute Respiratory Syndromes in Seoul.

For our survey of the infection frequency and mixed infection of the viruses causing acute respiratory syndromes, we analyzed those viruses from acute respiratory patients in Seoul. Total 1,038 specimens of oropharyngeal swab were tested by the real-time polymerase chain reaction (PCR) kit (Kogenebiotech, Korea) from Jan. to Dec. in 2013. Virus detection rate causing acute respiratory infection was 46% (476/1,038). The most frequently isolated virus was only hRV (21.6%, 103/476), followed by only ADV (8.96%, 93/476), only IFV A (H3N2) (18.1%, 86/476), and only hCoV (7.8%, 37/476) etc. Most of acute respiratory viruses had severe fever. Infection frequency information and mixed infection status on respiratory viruses circulating in Seoul will be helpful for the management of acute respiratory infection and for epidemiological continuous studies.

5.An Outbreak of Trichinellosis with Detection of Trichinella Larvae in Leftover Wild Boar Meat.

The clinical diagnosis of trichinellosis can be difficult due to lack of pathognomonic signs or symptoms. In Korea, since the first report of human infection by Trichinella spiralis in 1997 following the consumption of raw badger meat, there have been occasional trichinellosis outbreaks. We describe an outbreak of 12 cases of trichinellosis in Korea and implicate raw wild boar meat as the culprit. A total of 27 larvae of Trichinella (0.54 larvae per gram of meat) were recovered from the leftover raw wild boar meat.

After introduction of highly active antiretroviral therapy, the incidence of opportunistic infections and malignancies in HIV patients decreased. On the other hand, several osteoarticular complications are increasingly reported. Avascular necrosis of femoral head is one of such complications. HIV-infected patients have an 100-fold increased risk of avascular necrosis of femoral head compared to the general population. Many risk factors for this debilitating complication have been suggested. This paper reports five cases of avascular necrosis of femoral head in HIV-infected patients with review of literature.

7.Systematic Review of Heavy Metal Concentrations in Fish and Shellfish in Korea

OBJECTIVES: Heavy metals ingested through the consumption of aquatic products can accumulate in the human body over the long-term and cause various health problems. This study aims to present comprehensive data on the amount of heavy metals found in fish and shellfish in Korea using a systematic review of studies that report on that issue. METHODS: The study used the following databases: PubMed, Korean Studies Information Service System, and Research Information Sharing Service. The search terms for PubMed included fish OR shellfish OR seafood AND mercury OR cadmium OR lead OR heavy metal AND Korea. The search terms for Korean Studies Information Service System and Research Information Sharing Service included eoryu sueun, eoryu kadeumyum, eoryu nab, eoryu jung-geumsog, paeryu sueun, paeryu kadeumyum, paeryu nab, paeryu jung-geumsog, eopaeryu sueun, eopaeryu kadeumyum, eopaeryu nab, and eopaeryu jung-geumsog. RESULTS: A total of 32 articles were selected for review. The total mercury, lead, and cadmium concentrations in fish and shellfish reported in each of the articles are summarized, as are the species of fish and shellfish with relatively high concentrations of heavy metals. Total mercury concentrations tended to be higher in predatory fish species, such as sharks, billfishes, and tuna, while lead and cadmium concentrations tended to be higher in shellfish. CONCLUSION: This paper is the first to report a comprehensive summary of the concentrations of heavy metals in fish and shellfish. This data could be used as evidence to protect Koreans from exposure to heavy metals due to the consumption of highly polluted aquatic products.

8.Comparison of Automated Brain Volume Measures by NeuroQuant vs. Freesurfer in Patients with Mild Cognitive Impairment: Effect of Slice Thickness

Purpose:
This study aimed to examine the inter-method reliability and volumetric differences between NeuroQuant (NQ) and Freesurfer (FS) using T1 volume imaging sequence with different slice thicknesses in patients with mild cognitive impairment (MCI).
Materials and Methods:
This retrospective study enrolled 80 patients diagnosed with MCI at our memory clinic. NQ and FS were used for volumetric analysis of three-dimensional T1-weighted images with slice thickness of 1 and 1.2 mm. Inter-method reliability was measured with Pearson correlation coefficient (r), intraclass correlation coefficient (ICC), and effect size (ES).
Results:
Overall, NQ volumes were larger than FS volumes in several locations: whole brain (0.78%), cortical gray matter (5.34%), and white matter (2.68%). Volume measures by NQ and FS showed good-to-excellent ICCs with both 1 and 1.2 mm slice thickness (ICC=0.75–0.97, ES=-1.0–0.73 vs. ICC=0.78–0.96, ES=-0.9–0.77, respectively), except for putamen, pallidum, thalamus, and total intracranial volumes. The ICCs in all locations, except the putamen and cerebellum, were slightly higher with a slice thickness of 1 mm compared to those of 1.2 mm.
Conclusion
Inter-method reliability between NQ and FS was good-to-excellent in most regions with improvement with a 1-mm slice thickness. This finding indicates that the potential effects of slice thickness should be considered when performing volumetric measurements for cognitive impairment.

Since 1995, the Korean Society for Cytopathology has overseen the Continuous Quality Improvement program for cytopathology laboratories. The Committee of Quality Improvement has carried out an annual survey of cytology data for each laboratory and set standards for proficiency tests. Methods: Evaluations were conducted four times per year from 2008 to 2018 and comprised statistics regarding cytology diagnoses of previous years, proficiency tests using cytology slides provided by the committee, assessment of adequacy of gynecology (GYN) cytology slides, and submission of cytology slides for proficiency tests. Results: A total of 206 institutes participated in 2017, and the results were as follows. The number of cytology tests increased from year to year. The ratio of liquid-based cytology in GYN gradually decreased, as most of the GYN cytology had been performed at commercial laboratories. The distribution of GYN diagnoses demonstrated nearly 3.0% as atypical squamous cells. The rate for squamous cell carcinoma was less than 0.02%. The atypical squamous cell/squamous intraepithelial lesion ratio was about 3:1 and showed an upward trend. The major discordant rate of cytology-histology in GYN cytology was less than 1%. The proficiency test maintained a major discordant rate less than 2%. The rate of inappropriate specimens for GYN cytology slides gradually decreased. Conclusions: The Continuous Quality Improvement program should be included in quality assurance programs. Moreover, these data can contribute to development of national cancer examination guidelines and facilitate cancer prevention and treatment.

BACKGROUND: From May to July 2015, the Republic of Korea experienced the largest outbreak of Middle East respiratory syndrome (MERS) outside the Arabian Peninsula. A total of 186 patients, including 36 deaths, had been diagnosed with MERS-coronavirus (MERS-CoV) infection as of September 30th, 2015. MATERIALS AND METHODS: We obtained information of patients who were confirmed to have MERS-CoV infection. MERS-CoV infection was diagnosed using real-time reverse-transcriptase polymerase chain reaction assay. RESULTS: The median age of the patients was 55 years (range, 16 to 86). A total of 55.4% of the patients had one or more coexisting medical conditions. The most common symptom was fever (95.2%). At admission, leukopenia (42.6%), thrombocytopenia (46.6%), and elevation of aspartate aminotransferase (42.7%) were observed. Pneumonia was detected in 68.3% of patients at admission and developed in 80.8% during the disease course. Antiviral agents were used for 74.7% of patients. Mechanical ventilation, extracorporeal membrane oxygenation, and convalescent serum were employed for 24.5%, 7.1%, and 3.8% of patients, respectively. Older age, presence of coexisting medical conditions including diabetes or chronic lung disease, presence of dyspnea, hypotension, and leukocytosis at admission, and the use of mechanical ventilation were revealed to be independent predictors of death. CONCLUSION: The clinical features of MERS-CoV infection in the Republic of Korea were similar to those of previous outbreaks in the Middle East. However, the overall mortality rate (20.4%) was lower than that in previous reports. Enhanced surveillance and active management of patients during the outbreak may have resulted in improved outcomes.
