1.Postpartum Choriocarcinoma Preceded by Neonatal Anemia and Intrauterine Fetal Death: A case Report.

We report a case of postpartum choriocarcinoma from a 23-year old primiparous woman which was preceded by fetal anemia and intrauterine fetal death at 37 weeks' gestation. It has not been previously described in Korean literatures. The placenta, weighing 550 gm, contained multiple intervillous thrombi with laminated fibrin residue and pale cut curface. Histologic evidence of choriocarcinoma was not found in the placenta or in any fetal organs at autopsy, but large number of nucleated red blood cells and immature hemopoietic cells in the fetal capillaries of the placenta suggested the presence of fetal anemia. It was assumed that feto-maternal transfusion might had played a major role of fetal anemia and death.

2.pharmacogenomics and Schizophrenia.

The pharmacotherapy of schizophrenia exhibit wide inter-individual variabilities in clinical efficacy and adverse effects. Recently. human genetic diversity has been known as one of the essential factors to the variation in human drug response. This suggests that drug therapy should be tailored to the genetic characteristics of the individual. Pharmacogenetics is the field of investigation that attempts to elucidate genetic basis of an individual's responses to pharmacotherapy, considering drug effects divided into two categories as pharmacokinetics and pharmacodynamics. The emerging field of pharmacogenomics. which focuses on genetic determinants of drug response at the level of the entire human genome, is important for development and prescription of safer and more effective individually tailored drugs and will aid in understanding how genetics influence drug response. In schizophrenia, pharmacogenetic studies have shown the role of genetic variants of the cytochrome P450 enzymes such as CYP2D6, CYP2C19, and CYP2A1 in the metabolism of antipsychotic drugs. At the level of drug targets, variants of the dopamine D_(2), D_(3) and D_(4), and 5-HT_(2A) and 5-HT(2C) receptors have been examined. The pharmacogenetic studies in schizophrenia presently shows controversial findings which may be related to the multiple involvement of genes with relatively small effects and to the lack of standardized phenotypes. For further development in the pharmacogenomics of schizophrenia, there would be required the extensive outcome measures and definitious, and the powerful new tools of genomics, proteomics and so on.

3.Expression of type IV collagen and laminin genes by skin fibroblasts in culture.

Basement membrane zone gene expression by fibroblast cultures was examined by molecular hybridizations with human sequence specific cDNAs corresponding to type 1V procollagen and laminin subunit polypeptides. Northern transfer analysis with total RNA revealed the presence of specific mRNA transcripts for al (IV) and a2 (1V) chains of type 1V procollagen as well as Bl and B2 chains of laminin. Laminin A chain mRNAs were not detected using the same RNA preparations. The molecular size of al (1V) and a2 (1V) procollagen mRNA revealed 6.8kb and 6.7kb, respectively. The molecular size of Bl chain of laminin revealed 5.6kb, and B2 chain revealed 8.2 and 5.5kb polymorphic transcripts. In slotblot analysis using densitometer, steady-state levels of type IV procollagen and laminin mRNAs indicated that they were in relatively low abunclance, as compaired with type I procollagen mRNA. Quantitative levels of al (IV) and laminin Bl chaii mRNAs were more abundant than those of a2 (IV) and laminin B2 mRNAs. The mRNA ratio of al (IV)/a2 (lV) and laminin Bl/B2 were 1.9 and 1.5, respectively. These results demonstrate evidence for differential regulation of the expression of different basement membrane zone molecules during the formation of basement membrane.

5.COMPARISON OF WEAR RESISTANCE AMONG RESIN DENTURE TEETH OPPOSING VARIOUS RESTORATIVE MATERIALS.

The aim of this study was to compare wear resistance of resin denture teeth opposing various restorative materials. The wear resistance of conventional acrylic resin teeth (Trubyte Biotone) and three high-strength resin teeth(Bioform IPN, Endura, SR-Orthosit-PE) opposing different restorative materials (gold alloys, dental porcelain, composite resin) was compared. Wear tests were conducted with a sliding-induced wear testing apparatus which applied 100,000 strokes to the specimen in an mesio-distal direction under conditions of 100 stroke/min and constant loading of 1Kgf/tooth. Wear resistance of the resin denture teeth was evaluated by the following criteria : 1) wear depth, 2)weight loss, and 3) SEM observation. Results were as follows. 1. When opposed to gold alloys and composite resin, high-strength resin teeth showed superior wear resistance compared to acrylic resin teeth. But, in cases opposing dental porcelain, differences between the wear of the high-strength and acrylic resin teeth were not statistically significant (p<0.05). 2. When comparing wear resistance among high-strength resin teeth, opposing gold alloys, Endura was slightly more resistant and while in cases opposing dental porcelain, SR-Orthosit-PE was showed to be slightly resistant(p<0.05). 3. The wear of high-strength resin teeth was greater by 5 to 7 times when opposing porcelain and 2 to 3 times when opposing composite resin compared to gold alloys(p<0.05). 4. SEM observations of the wear surface showed that wear of resin teeth opposing gold alloys is a fatigue type of wear and wear of resin teeth opposing dental porcelain is fatigue and abrasion type of wear. Trubyte Biotone showed more severe fatigue type of wear than high-strength resin teeth. In conclusion, the use of dental porcelain should seriously be considered as restorative material in cases opposing resin denture teeth and improvement seems to be needed on resin teeth in the areas of wear resistance.

6.Interleukin-10 Differently Regulates Type I Collagen and Stromelysin-1 Promoter Activities in Dermal Fibroblast Cultures.

BACKGROUND: Interleukin-10(IL-10) was initially discovered and isolated on the basis of its ability to suppress cytokine by Thl helper T cell. Recently, the effect of IL-10 has been reported in cultured connective tissue cells. OBJECTIVE: To further investigate the mechanisms of IL-10 on extracellular matrix(ECM) homeostasis, we evaluated the expression of type I collagen and stromelysin-1 at the transcriptional level and also observed their promoters activities. METHODS: We examined that effect of the recombinant human IL-10 on the expression of genes involved in extracelluar matrix(ECM) synthesis and remodelimg in human dermal fibroblast cultures. Quantification of collagen synthesis, Northern blot analysis, transient transfection and a chloramphenicol acetyl transferase(CAT) assay were performed. RESULTS: In studying the dose and time response effect of IL-10 on collagen synthesis, maximal reduction was seen at 1.0 ng/ml concentration and 24 hours incubation. The synthesis of type I collagen mRNA was downregulated, while stromelysin-1 gene expression was enhanced by IL-10. In the transient transfection and CAT assay, the type I collagen promoter/CAT reporter gene construct showed downregulation by IL-10, while the stromelysin-1 gene promoter activities were upregulated. CONCLUSION: It is suggested that IL-10 differently regulates the transcriptional levels of type I collagen and stromelysin-1 gene and denoting IL-10 seems to take part in the homeostasis of ECM at pretranscriptional level.

8.Immunohistochemical Study of bcl - 2 Expression in Basal Cell Carcinoma.

BACKGROUND: Basal cell carcinoma(BCC) is the most common tumor of light-skinned people. Most of them are relatively innocuous, but some may cause extensive tissue destruction, and result in death by local infiltration of vital structure or metastatic disease. The bcl-2 is a protooncogene involved in tumorigenesis by blocking apoptosis or programmed cell death. OBJECTIVE: To investigate whether there was a difference in bcl-2 expression in different histopathological types of BCC. Method: We performed immunohistochemical staining by using a monoclonal mouse anti-human bcl-2 oncoprotein in 28 BCC(nodular: 7 cases, micronodular; 3 cases, adenoid; 1 case, infiltrative; 9 cases, and mixed; 8 cases). Result: All 28 BCC expressed bcl-2, and the degree of staining was different according to histopathological types; poor prognostic micronodular(100%) and infiltrative(73.3%) types were relatively strong, and nodular(66.7% ) and adenoid(75.0%) types were moderate. Conclusion . These results suggest that the degree of bcl-2 expression in BCC seems to be dependent to the histopathological types together with clinical behavior.

9.A Case Report of Enchondroma on Right Ischjum

The Enchondroma is a benign cartilage forming harmartoma arising chiefly from short tubular bones on hands and feet. If on a long tubular bone or flat bone such as femur or ilium, it is most likely to be a true neoplasm such as chondrosarcoma, not a harmartoma. A case of enchondroma was reported. It occurred over right ischium of 19 years old male and was found incidentally by roentgenogram. The lesion was biopsied and diagnos's was confirmed by histologic examination. It was treated by curretage with filling of cavity with bone chips. Post operative-course was uneventful.
