5.Effects of Clonidine Pretreatment on Bupivacaine-Induced Cardiac Toxicity Resuscitation in Dogs.

BACKGROUND: Bupivacaine is a amide type local anesthetic agent, widely used for its excellent quality of analgesia and long duration of action. But unintended intravenous injection causes severe complication such as convulsion and cardiovascular collapse, which is known for its difficulty in resuscitation. With all the study, the exact mechanism is still unclear and there are much debate on the method of resuscitation. METHOD: We studied the effect of clonidine pretreatment on bupivacaine-induced cardiac toxicity and resuscitation in anesthetized dog. Twelve dogs were divided into two groups. : saline pretreatment group (control, N=6) and clonidine pretreatment group (clonidine group, N=6). The dogs were anesthetized with N2O-O2-enflurane and vecuronium. Thoracotomy was done in 4th or 5th intercostal space for open cardiac massage. After confirming stability of vital signs, we administered clonidine (10 mcg/kg) or saline, and then administered bupivacaine with the rate of 2 mg/kg/min. When the electeocardiogram showed asystole, 20 mcg/kg of epinephrine was administered via central venous line and open cardiac massage with the rate of 120 beat/min. was performed. We observed electrocardiogram (lead II), arterial blood pressure, heart rate, dose of infused bupivacaine to be required for QRS widening and arrest, required time and administered dose of epinephrine for resuscitation. RESULTS: Clonidine group showed significant decrease of heart rate after pretreatment (p<0.05). There was no significant difference in required dose for QRS widening between two groups. The dose administered for inducing arrest was less in clonidine group than control group (p<0.05). The time required for resuscitation was shorter in clonidine group than control group (p<0.05). The total dose of epinephrine required for resuscitation was less in clonidine group than control group (p<0.05). The blood concentration of catecholamine did not showed significant difference during the whole course of experiment. CONCLUSIONS: Above results demonstrated that clonidine, a central nervous system-mediated sympatholytic agent, facilitated cardiac arrest when bupivacaine was infused intravenously and cardiac rescucitation.

6.Relationship among Life Style, Body Composition, and Bone Mineral Density in Community Dwelling Korean Adults.

PURPOSE: The purpose of this study was to analyze the relationship among life style, body composition and Bone Mineral Density (BMD) in community dwelling Korean adults. METHODS: Data were collected from 140 adults who participated in a health check-up program at community health departments in D city, Choong-chung providence. Subjects' life style was assessed with a structured interview survey. Body composition analyses were performed by the bioimpedence method and BMD was measured by peripheral dual energy X-ray absorptionmetry. RESULTS: Among the subjects, 39.3% showed normal BMD values, 50.7% were osteopenic and 10% were assessed as osteoporotic. BMD was significantly different by gender, age, education, economic status and BMI. Subjects who had three or more meals/day had higher BMD then who had less than three meals (t=-2.273, p=.026). BMD was not influenced by regular exercise, alcohol consumption, or smoking. In terms of body composition, there was a significant relationship between fat free mass and BMD (r=.172, p=.043). CONCLUSION: Implementing an osteoporosis prevention program would be warrented considering the significant proportion of osteopenic or osteoporotic subjects. Regular eating habit with three meals for adequate nutrition need to be emphasized to prevent further bone loss in this population. Among the body composition, fat free mass seem to be the mostly predicting factor for BMD.

7.A Case Report of Benign Recurrent Aseptic Meningitis Mollaret`s Meningitis.

A 52-years-old man had at least 6 attacks of benign recurrent aseptic meningitis in about 1 year. The clinical manifestation, progression, recurrence, and cerebrospinal fluid findings were correlated to the Mollaret's Meningitis. This meningitis is very rare in Korea.

9.Clinical Evaluation of the Strabismus Patient.

The authors studied 410 cases of strabismus patients who visited our clinic from 1973 to 1978 Among them, 170 cases of esotropia, 234 cases of exotropia and 6 cases of hypertropia were observed. The average age at the time of onset was 3.4 years in esotropia and 5.2 years in exotropia. Visul acuity in exotropia was relatively good. 0.7 or more visual acuity was 53 % in exotropia and 34.7 % in exotropia. Refractive status, hypertropia above 1.5D was 64. 2 % in esotropia, but, no specific correlation was found between exotropia and refractive status. The incidence of amblyopia was 33.1 % in esotropia and 19.7 % in exotropia. In accomodative esotropia, the patients with high AC/A ratio were treated with bifocal segment glasses. and the results were good.
