1.A case of congenital tricuspid stenosis.

Congenital tricuspid stenosis is an uncommon congenital heart anomaly. Tricuspid stenosis is usually associated with other anomalies, most commonly severe pulmonary stenosis or atresia with secondary hypoplasia of the right ventricle. Clinically it may be difficult to distinguish from tricuspid atresia. A case of congenital tricuspid stenosis in a 32 day-old-girl is presented. The patient had cyanosis and dyspnea. Echocardiographic study revealed tricuspid stenosis with right ventricular hypoplasia, atrial septal defect and pulmonary stenosis.

4.Report of A Case of Situs Inversus Totalis.

A case of situs inversus totalis was observed in the cadaver of a 35 year old Korean female and the anatomical structures were investigated. The fact that she had received intestinal resection due to tuberculosis of intestines was confirmed. The results are summarized as follows : 1. All thoracic and abdominal viscera were completely transposed and in the left lower quadrant of abdomen, it was observed that the distal portion of the small intestine, cecum, appendix and part of the ascending colon had been resected. 2. Classic mirror-image dextrocardia with patent foramen ovale (large diameter, 7mm), sinusitis in the left maxillary sinus, bilateral thoracic duct, variation of the opennig site of the common bile duct and the main pancreatic duct, and maldvelopment of the left adrenal gland were observed together with the sinus inversus totalis.

BACKGROUND: The main function of melanocyte is known to proiect the skin from hazardous sun-light. But, some investigators have claimed lately that melanocytes are also related to the immunologic role in the epidermis becauase these cells produce IL-1 activity and IL-lb convertase activity, in vitro. OBJECTIVE: Our purposee were to investigate the effects of rIFN-b on the proliferation of melanocytes, melanin content, and the expression of HLA-DR aritign on melanocytes after a rIFN-y exposure. MEHTODS: The number of melanocytes, the melanin content, and the expression of HLA-DR antigen were evaluated on culturect human melanocytes according to a time sequence and various concentrations of rIFN-y. RESULTS: Antiproliferative activity on melanocytes was dependent on the exposure time and the concentration of rIFN-r. According to the exposure time and the concentration of rIFN-r, melanogenic activity was inhibited or stimulated, Normal melanocytes didnt express HLA-DR antigen, but when normal melanocytes were exposed to rIFN-r, the expression of HLA-DR antigen increased in a timeand concentration-dependent fashion. After the removal of rIFN-r fiom the culture media the expression of HLA-DR antigen on melanocytes also disappeared. CONCLUSION: In our study, melanocytes seem to be related to the irnmunologic role in the epidermis because these cells expressed HLA-DR antigen after rIFN-r exposue and we think that study could help to investigate between melanocytes and immunalogic mechanisms in various inflammatory skin diseases.

7.The Mechanism of Blood Flow Generation during Closed Chest Cardiac Massage: Transesophageal Echocardiographic Monitoring.

The mechanism of forward blood flow during closed chest cardiac massage remains controversial. Two theories have been suggested: the cardiac pump theory and the thoracic pump theory. Case report is presented to illustrate the use of transesophageal echocardiography during cardiopulmonary resuscitation. The findings included right and left ventricular compression, closure of the mitral valve during compression, opening of the mitral valve during the release phase, and atrioventricular regurgitation during compression, indicating a positive ventricular-to-atrial pressure gradient. These findings suggest that direct cardiac compression was the predominant mechanism of forward blood flow during cardiopulmonary resuscitation in this patient. Transesophageal echocardiography offers a new approach for study of the flows and cardiac morphologic features during chest compressions in humans. An understanding of the actual mechanisms involved is necessary if improved cardiopulmonary resuscitative techniques are to be rationally developed for enhancing the outcome of resuscitation.

8.Poland's Syndrome: A Case Report

Poland's syndrome is congenital anomaly which was described first by Alfred Poland in 1841. The clinical features are variable but always include congenital aplasia of fingers and syndactyly. We have experienced a case of Poland's syndrome. This case was male children and revealed thoracic anomaly of right side (absence of pectoral muscles and anterior axillary fold), atrophy of forearm muscles, ipsilateral syndactyly with aplasia of thumb and middle phalanges of 2, 3, 4, 5,th fingers, and ipsilateral congenital radio-ulnar synostosis.

9.Marginal microleakage of self-etching primer adhesives and a self-etching adhesive.

This study evaluated the microleakage performance of four self-etcing primer adhesives(Clearfil SE Bond, Clearfil Liner Bond 2, UniFil Bond, and FL Bond) and one self-etching adhesive(Prompt L-Pop). Class V cavity preparations with occlusal margins in enamel and gingival margins in dentin were prepared on both buccal and lingual surfaces of 50 extracted human molar teeth. Prepared teeth were randomly divided into five groups and restored using one of five adhesives and composite resins: Prompt L-Pop/Filtek Z 250(Group 1), Clearfil SE Bond/Clearfil AP-X(Group 2), Clearfil Liner Bond 2/Clearfil AP-X(Group 3), UniFil Bond/UniFil F(Group 4), and FL Bond/Filtek Z 250(Group 5). Following one day storage in room temperature water, the restored teeth were thermocycled for 500 cycles between 5degrees C and 55degrees C. Marginal microleakage was assessed by dye penetration using 2% methylene blue dye. After 24 hours, the teeth were sectioned longitudinally and evaluated for microleakage under steromicroscope. The data were statistically analysed by Kruskal-Wallis Test, Mann-Whitney and Wilcoxon signed ranked tests. The results of this study were as follows; 1. The microleakges at both enamel and dentinal margins were the lowest in group 4, increasing among groups in the following order: group 2, follwed by group 5, follwed by group 1, and the highest in group 3. 2. At the enamel margins, the microleakage of group 3 was significantly higher than those of groups 2, 4 and 5(p<0.05), and also the microleakage of group 1 was statistically higher than those of groups 2 and 5(p<0.05). 3. At the dentinal margins, microleakage of group 3 was significantly higher than microleakages of groups 1, 2, 4 and 5(p<0.05). 4. Compared with microleakages between the enamel and dentinal margins of each group, groups 1, 4 and 5 at enamel margin and group 2 and group 3 at dentinal margin were higher microleakage. But there was no significant difference between enamel and dentinal microleakages of each group(p>0.05).
