3.The Incidence of parasites found of vegetables.

The high incidence of intestinal parasites among the Korean people almost same as 40 years ago might be partly interpreted by the fact that they are all fond of raw vegetables cultivated on the farms fertilized with non-treated night soil. The cabbages, onions and radishes were collected from the five areas of Seoul vicinity and the number of parasites attached to those vegetables were investigated to compare with the results obtained 10 years ago by the same methods of sampling and examination as the present survey. The resultant findings were summarized as follows: The eggs of Ascaris, hookworm, Trichostrongylus and whipworm, rhabditoid larvae and filariform larvae of hookworms were detected from those vegetables. Additionally, a number of larvae of free living nematodes and morphologically undetermined eggs were also observed. The maximum number of parasites found from one vegetable were as follows: 16 Ascaris eggs on a cabbage, 27 hookworm eggs on an onion, 8 whipworm eggs on a cabbage, 68 filariform larvae of hookworm on an onion. The onions were highly contaminated by the number of parasites as much as 177.6 including the larvae of free living nematodes, the next being the cabbages with 25.3 and the radishes with 4.1 in average. No notable relationship was observed between the size of vegetables and the number of parasites attached to them. Ascaris eggs were found from 56 percent of all cabbages, 28 percent of the onions and 40 percent of the radishes; whipworm eggs were detected on 40 percent of radishes and the filariform larvae of hookworms were observed from 40 percent of onions examined. It may be concluded that there is no notable change in the number of parasites attached to the vegetables collected from the Seoul vicinity compared to the results investigated by the same method 10 years ago.

4.AmpC-type beta-lactamases in Clinical Isolated of Cefoxitin-resistant E. coli and K. pneumoniae.

To elucidate the role of plasmid-mediated AmpC-type B-lactamases in clinical practice, cefoxitin-resistant isolates of E. coli (19 strains) and K. pneumoniae (7 strains) from three hospitals in Korea were studied. All of the 26 isolates produced at least one j3-lactamase and 16 (62%) isolates produced AmpC-type B-lactamases poorly inhibited by clavulanic acid. In 16 such isolates, 4 kinds of AmpC enzymes were detected; the pI 8.0 AmpC enzyme in 11 isolates, the pI 8.9 in 3 isolates of E. coli, the pI 8.5 in 1 isolate of E. coli, and the pI 7.8 in 1 isolate of K pneumoniae. The pI 8.0 and 7.8 AmpC enzymes had an apparent molecular mass of 38 kDa and the pI 8.5 and 8.9 AmpC enzymes had a molecular mass of 35 kDa. Cefoxitin resistance was transmissible in six E. coli and three K pneumoniae strains due to a common AmpC-type B-lactamase with a pl of 8.0. This enzyme was confirmed to be CMY-1 B-lactamase by Southern blotting and PCR analysis. Four E. coli isolates produced large amounts of AmpC-type j3-1actamase. They were chromosomal AmpC hyperproducers carrying some alterations in the promoter and attenuator regions of the ampC chromosomal gene. The pI 7.8 AmpC enzyme is currently under study. In conclusion, this study showed that the CMY-1 plasmid-mediated cephamycinase play an important role in cephamycin resistance of K. pneumoniae and E. coli clinical isolates in Korea.

5.Total Hip Replacement Arthroplasty for the Old Dislocated Hip

After the success of total hip replacement arthroplasty by John Charnley in 1962, it was well known fact that total hip replacement arthroplasty can be applied to degenerative arthritis, rheumatoid arthritis, post-traumatic arthritis, ankylosing spondylitis, even on the fused hip. However the old dislocated hip gives arise a few problem for its total hip replacement with the follwing causes:, 1. floor of the falad acetabulum is very thin and inadequate to receive a socket. 2. distorted anatomy of the acetabulum and proximal femur. 3. short and atrophic abductor mechanism. Total hip replacement arthroplasty could be successful for the old dislocated hip, if the adequate size of cup ard prosthesis are available and there is adequate length of the hip abductor. Preoperatively we can measure the size of the acetabulum and the distorted femur with roentgenogram. But it is not known how to measure or predict whether or not there is enough length of the abductor muscle mechanism preoperatively. Therefore the question arises how to measure or predict the length of the hip abductor in the old dislocated hip. Although this is a preliminary report, it is our feeling that the more the range of motion the hip has, the better the length of the hip abductor muscle. In our series acetabular socket can be positioned at the original site and the osteomized greater trochanter reduced to the femur easily in the cases more than 190 of total range of motion of the affected hips, The range of motion of the affected hip can be measured preoperatively.

6.Anesthetic Experience of Dynamic Cardiomyoplasty: A case report.

Dynamic cardiomyoplasty is a recently introduced surgical method to improve myocardial performance. It consists of a placement of a skeletal muscle flap around the heart and stimulation of the flap in synchrony with ventricular contraction. We experienced a case of cardiomyoplasty in a 25 year old male patient with congestive heart failure. Anesthesia was induced and maintained with fentanyl, midazolam and isoflurane. The operation was performed for 8hrs without cardiopulmonary bypass and the patient was transferred to the intensive care unit. He was mechanically ventilated electively overnight and extubation was done 18hrs postoperatively. The patient was discharged home on the 40days after operation and improved in exercise tolerance. We report the anesthetic management and hemodynamic changes in a patient who underwent dynamic cardiomyoplasty.

7.An Analysis of Histopathologic Evaluation of Lung Carcinomas in Last Ten Years.

Lung cancer is one of the most common types of malignancy in western nations with serious health problem, and it has become the leading cause of cancer death of males, second only to stomach cancer, in Korea. A review of the histopathology of 1363 cases (1231 patients) of lung carcinoma, diagnosed at the Keimyung University Medical center from 1987 to 1996, was performed to reclassify the type of carcinomas and to investigate the change in the distribution of histologic types of lung carcinoma according to age, sex and year. Among the 1363 cases, 132 patients underwent a surgical operation after biopsy. The diagnosis of each case was proven by histopathologic analysis of surgical specimens (13.2%) and biopsy materials (86.8%). The histologic types in our study were basically based on modified WHO classification (1982) and on new WHO classification (1999). The classification of small cell carcinoma was based on International Association for the Small Cell Lung Cancer (IASLC, 1988). Of the 1231 patients with lung carcinoma, 1012 were male and 219 were female (male to female ratio was 3.6:1). According to the analysis of age distribution, the most prevalent age group was 60~69 years in both sex as (n=516, 42.0%). Changing trends in sex distribution of lung carcinoma patients showed that the proportion of men had decreased throughout the years, whereas the proportion of women had significantly increased. Histologically, squamous cell carcinoma was the most common (n=624, 50.7%), followed by small cell carcinoma (21.1%), adenocarcinoma (18.1%), large cell undifferentiated carcinoma (2.1%), adenosquamous carcinoma (0.4%), and large cell neuroendocrine carcinoma (0.4%), in order of frequency. In men, squamous cell carcinoma was the most frequent type (55.1%). In women, adenocarcinoma was the most frequent type (39.7%). In both sexes, adenocarcinoma was the most common type in patients under the age of 40 (n=12, 41.4%), while squamous cell carcinoma proved the most frequent type in patients over the age of 40 (n=617, 51.3%). Changing trends of histologic types of lung cancer showed that the incidences of squamous cell carcinoma had significantly decreased throughout the years, whereas those of adenocarcinoma and small cell carcinoma had increased. In conclusion, the results showing increases in the percentage of female patients and in the number of cases of adenocarcinoma were noteworthy, and well correlated with other related reports.
