1.Congenital Cystic Adenomatoid Malformation of the Lung: Clinicopathologic analysis of 22 cases.

Congenital cystic adenomatoid malformation of the lung(CCAML) is a rare developmental anomaly characterized by an "adenomatoid" hyperplasia of terminal respiratory structures with formation of the cysts of varying sizes. CCAML is separated into three major types based on the gross and microscopic findings. We have analyzed 22 cases of CCAML, those consisted of 6 autopsy cases and 16 surgical specimens. Out of 22 cases, 5 cases were composed of large cysts(type I) and 9 cases had multiple small cysts(type II). Remaining one case revealed features of solid type(type III), and 7 cases were mixed form. There were 16 boys and 6 girls. All cases were below the age of 14 years. There was no clear-cut age difference between different types of CCAML. However, inflammation, fibrosis and pseudostratification of epithelium were often found in older age. All fetal autopsy cases of CCAML had hydrops fetalis and were associated with maternal hydramnios. One case of type III showed definite mucinogenic cells in the cysts unexpectedly, and one case of the mixed form(typeI+II+III) was found in a fetus of 22 weeks of gestational age. Above findings contradicted the classical description of the CCAML, and suggested that arbitrary classification into three types may not be the best way in understanding this condition.

2.Numerical aberrations of chromosome 17 and her2/neu gene amplification, her2/neu and p 53 protein expression in breast cancer.

Breast cancer is one of the leading causes of death attributable to cancer in women. In view of the limitations of conventional predictable factors of the breast cancer, additional second-generation parameters would be valuable in selecting the patients who would be most likely to be beneficial from adjuvant therapy and breast reconstruction. The author investigated the HER2/neu gene amplification and the number of chromosome 17 in 39 cases of paraffin embedded breast cancer tissues, 20 cases without lymph node metastasis and 19 cases with lymph node metastasis, using fluorescent in situ hybridization(FISH) and compared the results with HER2/neu and p 53 protein expression detected by immunohistochemical method. Eleven cases fibroadenoma were used as benign tumor control. Numerical aberrations of chromosome 17 were found in 17 out 39 breast cancer cases (44%)(monosomy in 10 cases, 26%; trisomy in 3 cases, 8%; tetrasomy in 3 cases, 8%; polysomy in 1 case ,3%), and the frequency of each type aberration was not significantly different between the negative and positive groups in lymph node metastasis. Monosomy of chromosome 17 was found in 2 out of 11(12%) fibroadenoma cases. HER2/neu gene amplification was found in 8 out of 39 cases (19%) and other 2 cases revealed HER2/neu gene amplification in lymph node metastatic tumor only, not in original tumor. Fourteen out of 19 cases of breast cancer with lymph metastasis showed HER2/neu protein expression both in original and metastatic tumors. All of the six cases showing HER2/neu gene amplification in original and/or metastatic tumor revealed HER2/neu protein expression. The frequency of HER2/neu gene amplification in the 39 breast cancer cases was not different between metastatic and non-metastatic groups(p= 0.284). However, HER2/neu protein expression was increased significantly in the metastatic group(p=0.028). None of the 11 fibroadenoma cases revealed HER2/neu gene amplification or HER2/ neu protein expression. Nine out of 19 cases of breast cancer with lymph node metastasis showed p 53 protein accumulation in original tumor(47%), but 3 of them revealed p 53 protein accumulation only in original tumor. The frequency of p 53 protein accumulation was not significantly different between metastatic and non-metastatic groups. None of the 11 fibroadenoma cases revealed p 53 protein accumulation. In conclusion, there are no differences between the lymph node metastatic group and non-metastatic groups in numerical aberrations of the chromosome 17 , amplification of the HER2/neu gene expression and accumulation of the p 53 protein in breast cancer. However, the HER2/neu protein expression was increased significantly in lymph node metastatic group, so it could be one of the predictors of the metastasis in breast cancer.

4.Angiosarcoma of Jejunum Following Therapeutic Irradiation: A case report.

We report a case of angiosarcoma of the jejunum in a 65-year-old female. This angiosarcoma developed 20 years after postoperative irradiation for cervical carcinoma of the uterus. Grossly, the resected jejunum showed a 8 cm long segmental hemorrhagic lesion with multiple, small hemorrhagic nodules or cysts on its serosa and mesentery. Microscopically, the wall of jejunum was infiltrated by epithelioid or polygonal tumor cells arranged in solid nests or lining irregular vascular spaces. The case is of interest that the tumor occurred in the very unusal site for angiosarcoma and the patient had a past history of irradiation for uterine cervical carcinoma 20 years ago. In addition, the resected intestine showed histologic changes of chronic irradiation effect. Therefore, this case supports the view that there is cause and effect relationship between irradiation adn angiosarcoma.

7.Development of a Semi-Quantitative Food Frequency Questionnaire for Pre-school Children in Korea.

The purpose of the study was to develop and evaluate the reliability of a semi-quantitative food frequency questionnaire (SFFQ) intended to measure mainly nutrient intake (energy, protein, fat, calcium and iron) related to growth in pre-schoolers in Korea. Based on foods with up to 90 cumulative percent contribution of the 5 nutrients, we developed a SFFQ for 86 foods. In order to evaluate the reliability of both nutrient and food intakes, caregivers of a sample of 101 children aged 4 to 6 years completed the SFFQ repeatedly at 3 month intervals. There were small differences in the nutrient intakes assessed by the SFFQ ranging from 0.55% to 9.91%. There were no significant differences in nutrient values calculated from the repeat questionnaires, except in the case of niacin and vitamin C. The Pearson correlation coefficients of most of the nutrients ranged form 0.54 to 0.75 (mean = 0.66). When energy intake was adjusted, there was approximately a 0.2 decrease in the correlation coefficient of most nutrients, due to the high correlation of energy intake with other nutrients (r=0.7-0.9). The amount of food intake differed by 0.1 to 66.4% (mean = 17.5%), depending on the food item. Out of 86 foods, 74 foods (86%) showed less than a 30% difference in intake and 30 foods (35%) showed less than a 10% difference. Only 6 (7%) out of the 86 foods presented statistically significant differences in intake. The Spearman correlation coefficients of most food intakes assessed repeatedly by the SFFQ ranged from 0.4 to 0.7. Reproducibility of the nutrient and food intakes found in this study was better or similar to those found in cases of Korean adults. Therefore, the SFFQ developed in the present study can be a useful tool to assess the dietary intake of pre-schoolers in Korea.

8.Relationship between Personality Type, SAT score and GPA of Student Nurses.

PURPOSE: This study was to identify the relationship between personality type, college admission SAT scores and GPA scores of student nurses. METHOD: The data was collected from 270 student nurses enrolled in a baccaleaureate program in Seoul. MBTI was used to identify students' personality and SAT score and GPA score were collected over 4 years. The collected data was analyzed by using SPSS Win. package. RESULT: 1. There were slightly more extrovert (E) type (54.4%) students than the introvert (I) type; more sensing (S) type (71.1%) than the intuitive (N) type. 2. The introvert type students had significantly higher SAT scores than those of the extrovert type (p=.002). 3. The judging type students had significantly higher GPA scores throughout their college years than the perceiving type. 4. There was no statistically significant relationship between SAT and GPA scores. SAT scores did not accurately predict students' academic achievement in college in this sample. CONCLUSION: The distribution of the personality types in the sample was different from the general population which may suggest that college admission criteria is biased toward certain personality type. Since different personality types process information and cope with the outside world differently, effective teaching strategies need to be considered for each class.

9.The Effects of Self-Administered Hand Reflexology on Fatigue in Student Nurses in Clinical Practice.

PURPOSE: This study was to identify the effects of self-administered hand reflexology on level of fatigue in college student nurses during clinical practice. METHOD: A non-equivalent control group, quasi-experimental research with pre and post-test design was used. Forty-seven student nurses in clinical practice were assigned to either an experimental or control group. The experimental group performed self-hand reflexology for 10 minutes once a day during the 5 days of clinical practice. RESULTS: 1. While there was a significant difference in degree of fatigue and fatigue scores in the experimental group over time, there was no significant difference between the two groups. 2. Vigor score for the experimental group was significantly higher than those of the control group during the 5 days except for day 5, but there was no significant difference between the two groups. 3. Levels of fatigue for the experimental group increased after 1 hour in the 1st post time, but there was no significant difference between the groups. CONCLUSION: Self-administered hand reflexology was not found to be an effective method for relieving fatigue for student nurses in clinical practice. Also physiological effects were not identified in this study, and so further research is needed.
