1.Alternative impression technique for multiple abutments in difficult case to control.

BACKGROUND: Even though excellent impression materials are now available for making accurate replication for hard and soft tissue, the numerous dentists have faced lots of obstacles in making simultaneous impressions of multiple abutments. CASE DESCRIPTION: This article describes a modified method of tray fabrication using auto-polymerizing acrylic resin and impression technique for multiple prepared teeth in cases with limitations and difficulties in taking dental impressions. CLINICAL IMPLICATION: This segmental tray technique has several advantages, including higher impression quality, fewer impressions, and being more comfortable for the patient and less stressful for the clinician.

Albinism is an inherited ongenital disarder in which there is sgr neralized decrease or absence of pigrnent in the eyes, skin, and hair. There are two general groups (1) Oculocutneou albinism, (2) Ocular albinism. Skin cancers in albinos are rare in ternperate clirnats and t.hey occur rnostly on sun ex posed areas. We present a case of recurrance of squarnous cell carcinoma ir a Korean albinoid on both sides of the neck and right ear. On liistopathologic examination, we find I or pearls, and irregular mases of epidermal cells that proliferite downward into the devmis. The invaing cell masses are composed of atypical squamous cells.

4.The Prognostic Significance of Blood and Lymphatic Vessels Invasion of Upper Urinary Tract Transitional Cell Carcinoma.

The prognostic significance of vascular (blood and lymphatic vessels) invasion was evaluated in a retrospective review of 27 patients with upper urinary tract transitional cell carcinoma from January 1985 to December 1993, who underwent a nephroureterectomy with bladder cuff resection. Vascular invasion was found in 10 patients (37%). The incidence of vascular invasion was well correlated with tumor grade and stage. The incidence of postoperative metastases was significantly higher in the patients with (70%) than without (17.6%) vascular invasion (p<0.05). The survival rate of the patients with vascular invasion was significantly lower than in those without vascular invasion (p<0.01). In multivariate Cox`s regressional analysis the prognostic value of vascular invasion was independent of tumor grade and stage. These results indicate that vascular invasion should predict a more unfavorable outcome in patients with upper urinary tract transitional cell carcinoma as an independent morphological indicator.

5.A Case of Fusarium solani Keratitis mixed by Acremonium falciforme.

Although more reports on the fungal keratitis are recently coming out, cases of mixed infection by two or more fungal agents have not yet beenreported in the literature. The authors experienced a case of fungal keratitis caused by a combination of Fusarium solani and Acremonium falciforme in which central corneal ulcer with severe suppurative infilteration and hypopyon were observed. Since the conditions were resistant to conservative treatment and corneal perforation occurred despite of medical treatment, penetrating keratoplasty was performed along with postoperative topical and systemic antifungal therapies for several weeks and good results were obtained with no evidence of recurrence.

7.A Case of Eccrine Nevus.

Eccrine nevus is an unusual entity with various clinical manifestations. Histologically the eccrine apparatus appears simply increased in number and/or size, but structurally normal. We present a case of eccrine nevus on the dorsum of the left 3rd and 4th fingers in a 20 year-old female. The lesions were 3 × 2cm sized, well demarcated, smooth surfaced, brownish plaques with mild hyperhidrosis. Histological examination revealed hyperkeratosis in the stratum corneum and hyperpigmentation of basal layer, and an increased number of eccrine sweat glands and many eccrine coils in the dermis was seen.

9.The Role of nm23-H1 Gene in Urologic Cancer (II): Expression and Mutation of nm23-H1 Gene in Urothelial Cancer.

BACKGROUND AND PURPOSE: We had previously demonstrated by immunohistochemical study (IHS) that nm23-H1 gene may play an important role in disease prognosis as well as its participation in metastasis of urothelial cancer. The purpose of present study was 1) to reexamine the role of nm23-H1 gene in urothelial cancers at molecular level, 2) to identify the molecular mechanism of decreased immunoreactivity for nm23-H1 protein in metastatic urothelial cancers, and 3) to identify whether IHS is reliable in studying the expression of nm23-H1. MATERIALS AND METHODS: We studied expression level and mutation profiles of nm23-H1 gene in 25 fresh surgical specimens of urothelial cancer by reverse transcription polymerase chain reaction(RT-PCR)-Southern blotting analysis, and PCR of genomic DNA followed by single strand conformation polymorphism and sequencing analysis. The results of RT-PCR-Southern blotting were comparatively analyzed with those of IHS. RESULTS: mRNA transcript levels of nm23-H1 gene were significantly decreased in tumor tissues with metastasis as compared with those without metastasis. The transcript levels of nm23-H1 gene were also significantly decreased in metastatic tumor tissues as compared with primary tumor tissues. Point mutation of nm23-H1 gene was detected in only 1 of 13 urothelial cancer tissues with metastasis, whereas, mutation was observed in none of those without metastasis. The results of IHS corresponded with those of RT-PCR-Southern blotting analysis in 23 of 25 specimens. CONCLUSIONS: The nm23-H1 gene may play an important role in metastasis of urothelial cancer. Decreased transcription at mRNA level may be a major molecular mechanism of loss of immunoreactivity for nm23-H1 protein in urothelial cancer. IHS used in the present study may be a clinically useful method to study the expression of nm23-H1 and to predict metastasis potential and prognosis of urothelial cancers.
