2.Epidural Hematoma Following Lumbar Epidural Anesthesia: A case report.

Epidural hematoma associated with epidural anesthesia is rare. We describe a case of a epidural hematoma after continuous epidural anesthesia and analgesia for arthroscopic patella shaving. Anesthesia was indudced with 8 ml lidocaine, 2.0% with epinephrine, 1:200,000 and surgery proceeded. When additional boluses of bupivacaine were administered later, aspiration of the epidural catheter still yielded no blood. At the end of the operation, an epidural infusion of 0.125% bupivacaine was done 2 ml/hr by two-day infusor. In the third postoperative day, the patient suffered from acute myocardial infarction and then heparin was infused. In the fourth postoperative day, she complained lumbar back pain and developed depressed dorsiflexion (Grade II) of great toe in left foot during the infusion of heparin. Sagittal MRI showed posterior thickened epidural hematoma along the segments of T12-L4. Despite the delayed laminectomy and decompression, which was performed six days after her initial operation, she had a good postoperative response, followed by a complete neurological recovery.

5.A Case of Primary Cutaneous Leiomyosarcoma.

Primary leiomyosarcomas are extremely rare. Its clinical configuration is a round to oval, more or less hemispherical elevation of the skin, but the appearance of these lesion is not distinctive so that the diagnosis is established by the histopathologic findings. Microscopically the cutaneous leiornyosarcomas consist of a poorly delineated proliferation of spindle shaped atypical myomatous cells arranged in interlacing fascicles which merge into collagenous stroma. We experienced a case of primary cutaneous leiornyosarcoma which developed as a hen egg sized tumor on the left shin in a 71-year-old woman and present this case with the review of literatures.

6.Doppler Sonographic Evaluation of Ophthalmic Arterial Flow Pattern in Hypertensive Patients.

PURPOSE: To compare the Doppler velocity waveform pattern of ophthalmic artery of hypertensive patients with that of normotensive subjects. MATERIALS AND METHODS: Doppler velocity waveform was obtained from ophthalmic artery in 45 hypertensive patients and 60 normotensive subjects. Both hypertensives and normotensive subjects were classified according to age into those younger than and those older than 45 years. Doppler indices[pulsatility index(PI), resistance index(RI), the first systolic peak/the second systolic peak(S1/S2), the first sytolic peak/diastolic peak (S1/D)] measured in hypertensive patients were compared with normotensive subjects. RESULTS: Among the various doppler indices, only S1/S2 showed significant difference(p < 0.05) between the hypertensive patients and normotensive subjects younger than 45 years. Doppler velocity waveform of hypertensive patients older than 45 years showed no significant difference from that of normotensive subjects with corresponding age. CONCLUSION: Doppler velocity waveform of ophthalmic artery in hyopertensive patients younger than 45 years shows pattern with S2 higher than that of normotensive subjects. High S2 component(reflective-wave) may represent increased vascular impedance ~ue to vasococonstriction of retinal arterioles in hypertensve patients.

8.Clinical Study on Cast-Brace in Femoral Fractures

14 cases of femoral shaft fractures were treated with cast-brace for two years from June, 1974 to June, 1976 at the Department of Orthopedic Surgery, College of Medicine, Seoul National University Hospital. The results were as followings; 1. Of the 14 cases, eleven cases were male and three were female, and they were between 18 to 51 years of ages. 2. The fracture site was accounted that three of 14 cases were at the distal one third of the femur and the other eleven at the mid one third with four cases being comminuted. 3. 7 cases were treated with cast-brace initially, and the other 7 cases were secondarily treated, because of being complicated. 4. The average time of fracture healing was 14 weeks in the cases with cast-brace which is compared with 20 weeks of spica-cast control group in the same period. 5. One of 7 cases treated primarily developed shortening and angulation, which was corrected by open reduction and internal fixation.

9.Intracranial Lipoma: A case report.

Lipoma involving brain and spinal cord is a rare tumor that most commonly located in the midsagittal region. This lesion is usually asymptomatic and has been reported incidentally at postmortem examination. Recently, the CT scan establishes the diagnosis of intracranial lipoma on the basis of typical X-ray absorption and location. The authors experienced a case of intracranial lipoma of occipital lobes. The patient was 7 months-old male who had protruding mass on the occipital region after birth. The mass revealed an uncapsulated lipoma with foca cartilage formation. We report this case with brief review of literatures.
