4.Evaluations of the Elderly Nutrition Improvement Program in the community health center: Effects of nutrition counseling and education program on elderly dietary behavior.

Low energy intakes, poor nutritional quality, and low food diversity are among the factors affecting the nutritional status of elderly in Korea. Therefore, a nutrition counseling and education program was conducted to promote dietary change and to improve nutritional status of elderly. The 7 step program consisted of a individual nutrition counseling and weekly or biweekly group nutrition education programs. Pre- and post-test measurings of dietary behaviors, attitudes, nutrition knowledge, and 24-hour dietary recall data were collected. Positive changes in the attitude related to diet were noted, whereas nutritional knowledge scores were not improved. Significant increases were noted in the frequency of vegetable consumption in female and diversity of food consumption in male. The densities of vitamin C(male & female), B1, B2, and niacin, calculated using the INQ(Index of Nutritional Quality), were significantly improved by nutrition counseling and education programs. These results suggest the appropriateness of nutrition intervention programs for the improvement of elderly nutrition.

6.Strategies to Improve Nutrition in the Elderly : An Analysis of Health Related Factors and the Nutritional Risk Index of the Elderly.

The elderly are the most rapidly growing segment of the population in Korea and the largest consumers of expensive medical care. It is reasonable to believe that improving the nutritional status would be beneficial to reducing morbidity and to improving the quality of their lives. This study was conducted to assess the health status and the Nutritional Risk Index of the elderly, and to provide the basic data for the Elderly Nutrition Improvement Program at the Public Health Center. One hundred and forty seven(76 males. 71 females) aged 60 years and over living in Suwon, were assessed with anthropometric measurements, body fat analysis, blood pressure measurements, and a set of about health and other related variables. Over half of them had less than a junior middle school education and were considered low income. The percentage of overweight and obese subjects was 33% by the BMI(Body Mass Index), and the prevalence of hypertension was 28%(males) and 31%(females). They had a lot of self-recognized health problems, the male elderly complained about more than the female elderly. In the case of psychological health status, however, the female elderly showed a higher proportions of depression than the male elderly(p<0.05). In social health status, the elderly had good relationships with friends and collegues, whereas they had poor relationships with their families. They had many nutritional risk factors, and smoking was the most prevalent risk factor for the male elderly and anemia was for the female elderly. The results of this study suggest that Elderly Nutrition Improvement Programs should be planned that can be easily followed. It would be helpful to design a program focusing on individual phychological and social health status, this would increase the efficiency of the program.

9.The Relationship between Health Value Cognition, Health Promotion Behavior and Health Examination Results Among Transit Corporation's Workers.

OBJECTIVES: This study was performed to compare health value cognition and health promotion behavior compliance between healthy group and not being healthy group, and to evaluate the difference of health promotion behaviors compliance between high group and low group in health value cognition. METHODS: The subjects of this study were 565 workers, 233 healthy workers, 172 observation cases and 160 disease cases, selected from 4,919 transit coporations workers. RESULTS: In health promoting behavior compliance, not being healthy group showed lower level than healthy group in exercise and showed higher level than healthy group in interpersonal support significantly. A worker who replied on health as the most value in life was consisted in 37.3% of healthy group and 36. 1% of not being healthy group, Health value cognition according to general characteristics showed no significant difference except only variable of age in healthy group. Health promoting behavior compliance according to cognition of health value showed in healthy group that a high level group of health value cognition was more significant difference than a low level group in self actualization and health responsibility of health promoting behavior and didn't show difference significantly in not being healthy group. CONCLUSIONS: In conclusion, the relationship between health value cognition, health promotion behavior compliance and periodic health examination results showed weakly. Therefore, in order to develop and apply spontaneous health promotion program, it was considered that should emphasize compliance than cognition.

10.Surgical Management of Middle Cerebral Artery Aneurysm.

Although the surgical technique of the management of an middle cerebral artery(MCA) aneurysm is not more difficult than that of an aneurysm in any other location, the surgical management outcome of this aneurysm is not better than that of other location aneurysms. This is probably due to occurrence of an intracerebral hemorrhage more frequently in an MCA aneurysm than other location aneurysms. This is required the proper surgical management of this aneurysm to be reviewed. The key points of surgery for MCA aneurysm can be summarized as follows: gently wide dissection and splitting of the sylvian fissure without injury of main sylvian veins; early adequate exposure of the M1 sites for temporary clips applications; avoid injuries of small branches of the MCA in sylvian fissure by using a low pressure suction during removing the intrasylvian hematoma; avoid injury of lenticulostriate arteries and narrowing of the parent arteries during clipping of aneurysms; and successful aneurysm obliteration with minimal brain retraction. Preoperative imagination of the configulation of aneurysm and the relationship between the aneurysm and surrounding vessels, such as M1, M2 and lenticulostriate arteries, from angiographic informations will decrease the incidence of premature aneurysmal ruptureduring surgery and improve the surgical management outcome of patients with MCA aneurysm. From September 1982 to December 1998, the authors has surgically treated 1025 patients of the cerebral aneurysms. Among these patients, 274 patients(26.7%) had the MCA anurysms. The surgical outcomes of these MCA aneurysm patients were good in 222 patients(81.0%) and death in 22 patients(8.0%). The authors discusses surgical anatomy and techniques for the management of MCA aneurysms on the basis of the author's experiences and a review of the literatures in order to improve the surgical management outcomes of patients with MCA aneurysm in the future.
