1.Cholelithiasis with Mucosal Dysplasia of the Gallbladder in a 2-year-old Child.

Traditionally, it has been emphasized that hemolytic disease was the primary cause of gallstones (cholelithiasis) in most young patients. In recent years, gallstones and common bile duct calculi have been increasingly diagnosed in infants and children, unrelated to hemolytic diseases. On the matter, the World Health Organization (WHO) classified intraepithelial neoplasia (dysplasia) of gall bladder as one of the precursor lesions of invasive cancer. The following article describes the case of a 2-year-old girl who had a laparoscopic cholecystectomy due to cholelithiasis and the pathologic diagnosis was chronic cholecystitis with diffuse, mild mucosal dysplasia.

4.Optimum Culture Conditions for Production of Proteinases secreted by Candida albicans , C. tropicalis , and C. parapsilosis.

In the present study, culture conditions to secrete proteinases from C. albicans, C. tropicalis and C. parapsilosis were examined. All three Candida species were found to secrete proteinases from acceleration phase to stationary phase, although the proteinase activities in culture filtrate were maximal during late exponential or early stationary phase. The proteinase activity in the culture filtrate of C. albicans cells grown at 30'C, was much higher than those grown at either 20 or 37'C. In culture of C. tropicalis and C. parapsilosis, the highest activity was found in culture filtrate grown at 37C. C. albicans secreted proteinases well in medium at initial pH 4.0-7.0. The optimal initial pH of medium for proteinase secretion was 7.0 for C. tropicalis and 5.0-6.0 for C. parapsilosis. All three Candida species secreted proteinases to greater amount in aerobic state. The most effective carbon source for proteinase secretion was xylose, glucose, maltose and sucrose for C. albicans, xylose for C. tropicalis and trehalose for C. parapsilosis. The effects of proteins, hydrolyzed proteins, ammonium sulfate as a sole nitrogen source on proteinase secretion were examined. Bovine serum albumin was the most effective nitrogen source of those tested and a little proteinase activity was detected in the culture filtrates when yeast cells were incubated in the medium containing ammonium sulfate. C. parapsilosis secreted proteinases to greater amount than the other Candida species in all nitrogen sources under study, indicating that C. parapsilosis proteinase would not be a inducible but a constitutive enzyme.

5.Effect of Persistent Mild Hyperglycemic Hyperinsulinemia on Development of Insulin Resistance in Rats.

The effect of persistant mild hyperglycemic hyperinsulinemia on the development of the insulin resistance in rats was studied in vivo. Also, the characteristics of the insulin resistance compared with the insulin resistance of STZ diabetic rats. Persistant mild hyperglycemic hyperinsulinemic rat model was produced by ingestion of glucose polymer for 8 days. The glucose disappearance and infusion rate was measured by hyperinsulinemic euglycemic clamp to"Imique at steady state of blood glucose and insulin levels. The clamped level of blood glucose was 100 mg/dl, and the clamped levels of insulin were 70 pU/ml (physiologic condition) and 3000 pU/ml (supramaximal condition). Hepatic glucose producticon rate was calculated using measured data. And the glycogen synthetic capacity of skeletal muscle(soleus) and liver was measured after 2 hours of hyperinsulinemic euglycemic clamp study. The glucose disappearance and glucose infusion rate in glucose polymer group was decreased in the both physiological and supramaximal insulin level compared to the rate of the normal control group. The rate of STZ diabetic group wase lowest at supramaximal insulin level among two another experimental groups. The hepatic glucose production rate of glucose polymer group was decreased compared to normal control but increased in STZ diabetic group.

6.Effect of Propofol and Thiopental on the Isolated Stunned Rat Heart.

BACKGROUND: Propofol(2,6-diisopropyl phenol) is an intravenous anesthetic agent, which was first introduced in 1986. It has 178.27 molecular weight and 1.8 fold stronger action of induction than that of thiopental. The effect of propofol, lowering blood pressure by depressing the cardiovascular system, has been mentioned in many papers, but its effect on myocardium is still in controversy and differs according to the method and interpretation of experiments used. This animal experiment was conducted to compare the effects of propofol and thiopental on myocardial contractility and coronary flow in isolated stunned rat hearts. METHOD: The hearts were isolated from twenty-four Sprague-Dawley rats and were perfused with modified Krebs solution. After isolation, the hearts were left in stabilizing period for 30 minutes. Then, myocardial stunning was induced by global ischemia for 15 minutes. In Group I, propofol was added to modified Krebs solution, which was used to perfuse the hearts at 40, 80, 120, 160, and 200micrometer concentration. In Group II, thiopental was added to perfusate at same concentration. Between each perfusion with anesthetic-added solution, the hearts in both groups were perfused with normal Krebs solution for 30 minutes to eliminate cumulative effect of anesthetics added. Left ventricular pressure, rate of ventricular pressure generation(dP/dt), and coronary flow were measured. RESULT: In both groups, anesthetics reduced left ventricular pressure and dP/dt in a dose dependent fashion. These changes were not statistically significant between two groups. However, at the 40 and 80micrometer concentration, the increment of coronary flow by propofol was significantly larger than that by thiopental. At the 120, 160, and 200micrometer concentration of propofol, coronary flow was gradually decreased with increasing concentration, and these decrease in coronary flow were not statistically significant between two groups. CONCLUSION: The effects of propofol on myocardial function were similar to those of thiopental in isolated stunned rat hearts and the increment of coronary flow might develop the coronary stealphenomenon. So, propofol may not have the benefit compare to thiopental for cardiac function.

10.Effects of insulin and exercise on glucose uptake of skeletal muscle in diabetic rats.

The effects of insulin and exercise on glucose uptake of skeletal muscle were investigated in soleus muscle isolated from low dose streptozotocin induced diabetic rat in vitro. Glucose uptake was assessed by measuring ³H-methylglucose uptake in vitro. Basal glucose uptake in diabetes was reduced by approximately one-third of the control value (5.6±0.73µMol/g/20min. in diabetes versus 8.4±0.77 in control, P<0.01). There was also a significant decrease (P<0.01) in glucose uptake of diabetes at physiologic insulin concentration (200 µIU/ml) by 40% (6.1±1.20 versus 10.0±0.81). Furthermore, maximal insulin (20000 µIU/ml)-stimulated glucose uptake was 36% lower in diabetes as compared with control (7.3±1.29 versus 11.4±1.29, P<0.01). In contrast, exercise (1.0 km/hr, treadmill running for 45 min.) effect on glucose uptake was so dramatic in diabetes that glucose uptake at basal state was 8.+1.09 and insulin stimulated-glucose uptake were 10.2±1.47 and 11.9±1.64, in 200 and 20000 µIU/ml added insulin, respectively. These results suggest that insulin insensitivity develops in skeletal muscle after 2 weeks of streptozotocin-induced diabetes, but these insensitivity was recovered significantly by single session of running exercise.
