1.Mass Screening for Stomach Cancer by Gastrofiberscopy in 1865 Employees of a Car Manufacturing Company.

Background: Stomach cancer is the most commonly diagnosed malignant tumor in Korea. Early detection by mass screening very important to increase survival rate of stomach cancer, because the prognosis of early gastric cancer is excellent, and specific symptoms and physical findings are rare with early gastric cancer. Methed: 1865 car manufacturing company employees were examinined by gastrofiberscopy at a dispensary of the company from May 6, l992 to July 13, 1992. Results: 1. Number of cases with normal finding was 1262(67.7%), gastritis 422(22.6%), gastric ulcer 47(2.5%), gasrtric cancer 4(0.2%), duodenitis 39(2.1%), duodenal ulcer 63(3.4%), combined ulcer(gastric & duodenal) 2(0.1%,), post operative state 6(0.3%), and others 4(0.3%) in total 1865 examinees. 2. Of 422 cases with gastritis, cases with superficial gastritis were 171 (9.2%), atrophic gastritis 138(7.4%), erosive gastritis 94(5.0%) and hemorrhagic gastritis 19(1.0%). 3. Of 4 cases with stomach cancer, 1 case was advanced stomach cancer and 3 cases were early gastric cancer(two cases of IIc and one case of IIc+III) by macroscopic classification. Conclusion: Mass screening provides an excellent opportunity to detect gastric cancer in populations without symytoms or with nonspecific symptoms. It should be recommended to undergo screening exam for high risk group of stomach cancer.

3.Malignant Atrophic Papulosis (Degos' Disease): A Case Report.

Malignant atrophic papulosis (Degosdisease) was first described in detail by Degos and his co-worker in I942, In this rare disease of unknown etiology, porcehun-white skin lesians develop, and death usually occurs from perforation of the intestine weeks to years after onset, and also but rarely from infarction of central nervous system. Our case of Degos disease in an 18 years old man with typical skin lesions and abdominal symptoms such as pain, nausea and distension due to gastrointestinal involvement is described. He died of peritonitis 9 months after onset. The biopsy specimen of an atrophic papule showed thrombotic occlusion of an arteriole. The authors experienced a case of Degos disease diagnosed by clinical and histopathologieal findings for the first time in Korea.

5.The Predictive Factors for Central Nervous System Lesion in Central Precocious Puberty and the Utility of Single Timed LH after GnRH Administration.

PURPOSE: This study was undertaken to determine whether the clinical presentation of patients with central precocius puberty(CPP) varies according to the etiology, whether this permits the differentiation between idiopathic and organic forms and to examine whether LH determination in a single timed blood sample after GnRH administration is sufficient to diagnose CPP. METHODS: This study included 33 girls with signs of breast development, of whom 23 were diagnosed as definite central precocious puberty. Sixteen patients had idiopathic CPP; the remaining 7 patients had organic CPP. Ten patients were classified as non-CPP. Potential clinical and laboratory predictors of CNS abnormalities were assessed and GnRH stimulation test was done. RESULTS: The age of onset in patients with organic CPP was 4.11+/-2.08 years, whereas the age in patients with idiopathic CPP was 7.25+/-1.34 years. This parameter is the only one showing statistical significance. We compared sensitivities and specificities at 0, 15, 30, 60, 90 and 120 min which yielded sensitivities of 8.7%, 87.0%, 91.3%, 87.0%, 73.9%, 60.9%. CONCLUSION: It is impossible to exclude a central nervous system lesion in patient with central precocious puberty without performing central nervous system imaging. However, this study indicates earlier the onset of disease, higher the possibility of presence of CNS lesion. According to the mean GnRH stimulated LH levels and sensitivity at each times, a single blood sample obtained for LH determined after GnRH administration at 30 min can be used to diagnose the central precocious puberty.

6.The Effects of Basic Fibroblast Growth Factor(bFGF)on Type I and VII Collagen Gene Expression in Cultured Dermal Fibroblast.

BACKGROUND: bFGF, a member of the fibroblast growth factor family, potently induces vascular smooth muscle cell proliferation and decreased synthesis of the collagens. OBJECTIVE: For further investigation of the effect of bFGF on extracellular matrix homeostasis in the skin, we evaluated the expression of type I and type VII collagen gene at the transcriptional levels. METHOD: We examined that recombinant human bFGF affects the expression of genes involved in ECM synthesis and remodeling in human dermal fibroblasts cultures as judged by Northern blot analysis. RESULTS: The steady state levels of type I and VII collagen gene mRNA were decreased with age dependent pattern up to 0.13 and 0.44 folds respectively. The transcriptional levels of type I collagen mRNA were increased by TGF-B, treatment but markedly decreased by bFGF as well as TNF-a. But there were no synergistic effects bFGF and TNF-a on type I collagen gene expression. The levels of type VII collagen gene expression were increased by both bFGF and TGF-B,. The TNF-a showed slightly antagnostic effects on type VII collagen gene expression. CONCLUSION: The type I and VII collagen gene expression in dermal fibroblasts is clearly subjected to modulation by the cytokines including bFGF with uncoordinate regulatory pathway. In addition to its function of vascular proliferation, bFGF also may play a major role in physiologic skin condition and in repair process such as formation of a stable dermoepidermal junction during skin wound healing.

9.Clinical Study on Avascular Necrosis of Femoral Head in Adult

One hundred twenty-one patients with avascular necrosis of the femoral head in adults, who were admitted in Department of Orthopadic Surgery, Kyungpook National University Hospital between January of 1972 and June of 1986 are investigated in terms of the history, clinical data, radiographic findings and laboratory examinations. And, the following results were obtained. 1. The highest incidence was in the 5th decade and the sex ratio between male and female was 5: l. 2. 62 cases(51.2%) had the bilateral hip involvement, and higher incidence of bilateral hip involvement in steroid or alcohol induced cases. 3. Etiological factors were unknown(38 cases, 31.4%), steroid (31 cases, 25.6%), alcohol (22 cases, 18.2%), femur neck fracture (15 cases, 12.4%), trauma (13 cases, 10.7%) and caissons disease (2 cases). 4. Underlying diseases in steroid-induced avascular necrosis of the femoral head were dermopathy (8 cases), pain over other joints (7 cases), head trauma (5 cases), nephropathy (4 cases), etc. 5. The time intervals between etiological events and appearance of clinical symptoms were 1 year and 7 months in steroid-induced cases, 1 year and 9 months in femur neck fracture cases, 1 year and 1 month in trauma-induced cases, 1 year and 5 months in caissons disease. 6. The duration of morbidity was 21 months on average. 7. At first hospital visit, the Ficat stage of femoral head avascular necrosis were stage II in 12 cases (10%), transition stage in 17 cases (14%), stage III in 34 cases (29%) and stage IV in 56 cases (47%). 8. Bone scanning was performed in 59 hips of 42 cases. Cold spot was observed in 17 hips and hot spot in 36 hips. There were false negative findings in 4 hips in spite of ischemic findings of femoral head on simple x-ray. 9. Bone marrow pressure was higher than 30mmHg and was increased 10mmHg more than initial pressure by stress test. 10. On intramedullary venography, diaphyseal reflux of contrast medium and diaphyseal stasis more than 15 minutes in 7 of 9 cases over stage II.

10.An Experimental Study of Activated Glutaraldehyde on Synovium

Steam autoclaving and ethylene gas sterilization method have been used for arthroscopes,but recently cold sterilization method using 2% glutaraldehyde solution was recommended.Although this method is economic, quick and excellent method, there has been a growing awareness of the minor complication associated with arthroscopy, particulsrily effusion and wound healing. So, we attemped to evaluate the effect of activated glutaraldehyde solution in knee arthroscopy. We have done so as follows : 1. Observed the intra-articular response (grossly, light and electron microscopically) of rabbit knees, three weeks after a single injection of glutaraldehyde at concentration of 0 to 1000 ppm. 2. Measured the glutaraldehyde concentration in the rinse solution using a sensitive spectrophotometric technique. 3. Measured the glutaraldehyde concentration in the intraoperative joint irrigation solution as same as above technique. The results were as follows : 1. Grossly, diffuse synovitis was observed with dose of 100 ppm or greater, and the degree of inflammation was proportional to the glutaraldehyde concentration. We have observed microscopic evidence of a few leukocyte infiltration and vascular congestion in the synovium of rabbit knees, injected with 5 ppm glutaraldehyde. At concentration of 50ppm, vascular hemorrhage and synovial edema, at 500ppm, focal synovial necrosis and fibrosis were observed. At 1000ppm, chondrolysis was observed. Electron microscopically we have observed synovial cells proliferation, especially type B cells and interstitial edematous changes, injected with 5-10 ppm glutaraldehyde. At concentration 50 ppm, synovial cells are decreased in number and marked vacuolization changes, at 100 ppm, synovial cells are denuded on the surface. 2. The concentration of glutaraldehyde present in the rinse basin was 16–30 ppm by thefirst rinsing procedure, if the same rinse solution was used for subsequent cases, the lutaraldehyde concentration was 117–149 ppm by the sixth rinsing procedure. 3. The concentration of glutaraldehyde present in the intraoperative joint irrigation solution was less than 1 ppm.
