3.Clinical Study on Surgical Treatment of Spondylolisthesis

Spondylolisthesis has been become of considerable causative disease for low back pain in orthopedic field recently. Twenty-seven patients with spondylolisthesis, who were taken operation during a 7-year period from Jan. 1978 to Dec. 1984 at Department of Orthopedic Surgery of Presbyterian Medical Center, were studied and analized on clinical and radiological aspects and the results of obtained were as follows: 1. The averake age was 41.7 years with a range of 20 to 63 years, and the incidence was more common in female than male in all types. 2. By Wiltse's classification, isthmic type was most common and 19 patients were included and level of spondylolisthesis was common at fourth lumbar vertebra in degenerative type and fifth lumbar vertebra in isthmic type. 3. By Meyerding's grading, 14 patients (52%) were Grade 1, which was most common and the slipping measured by Taillard's method was most common with a range 10% to 19%, which included 13 patients (48%) and average slipping was 21.4%. 4. Meschan's angle was ranged from 5° to 19° with an average of 13.3°. 5. Myelogram was done in 13 patients and 9 of them showed positive findings. 6. Spina bifida was most common developmental abnormal finding and 11 patients (41%) were included. 7. Posterolateral fusion was done in 24 patients and anterior interbody fusion was done in 3 patients. 8. 24 patients who had taken posterolateral fusion were permitted to ambulate early at 2 weeks with Norton-Brown back brace. 9. After over a year follow-up, 23(85%) of 27 patients had satisfactory result. 10. After follow-up, 21(88%) of 24 patients who had taken posterolateral fusion had successful fusion.

4.Ehlers-Danlos Syndrome: 2 Cases Report

Author experienced two cases of the type 3 Ehlers-Danlos syndrome which present hypermobility of the joints, asymmetry of the thorax, anomalies of the thoracic spine, and flatfoot. So, we report these patients with references.

5.Transscleral Diode Laser Photocoagulation for Retinopathy of Prematurity: Short-term Structural Outcome.

PURPOSE: We observed the short-term outcome and associated complications of transscleral diode laser photocoagulation (TSDL) for acute proliferative retinopathy of prematurity (ROP) to evaluate the efficacy and safety. METHODS: 20 eyes of 10 infants with acute ROP were treated with transscleral diode laser photo-coagulation through conjunctiva by a single surgeon. Anatomical results and postoperative complications were main outcomes. Follow-up ranged from 11 to 40 weeks. RESULTS: In 1 eye (5%), there was a macular temporal dragging. In 19 eyes (95%), regression occurred after a single laser treatment with a flat retina. No significant anterior or posterior segment complications were occurred, even though there was a small amount of preretinal hemorrhage in 1 eye after laser photocoagulation. CONCLUSIONS: The results suggest that transscleral diode laser photocoagulation is a safe and effective alternative to cryotherapy or transpupillary diode laser photocoagulation in the treatment of ROP.

6.A Case of Primary Non-Gestational Choriocarcinoma of the Ovary.

The authors report a xase of primary non-gestational choriocarcinoma(PNGCO) of the ovary in a prepubertal female patient and reviewed. It is an extremely rere disease of which incidence is one in 369 million. Major clinical symptom is abdominal pain, precociois puberty and it can be misdiagneosed as ectopic pergnancy. Distinction from gestational choriocarcinoma(GCO) of the ovary is important because of the worse prognosis of PNGCO. But no distinctive ultrastructural or immunohistochemical differences are found between PNGCO and GCO. Most acceptable treatment modality is an aggressive surgical therapy and systemic chemotherapy, but its progrosis is poor.

7.Clinical Results of Combined Surgery: Pars Plana Vitrectomy, Phacoemulsification and Posterior Chamber Intraocular Lens Implantation.

PURPOSE: To evaluate the clinical outcomes and complications of combined phacoemulsification and vitrectomy surgery in patients who have vitreoretinal diseases with a concomitant cataract. METHODS: Combined phacoemulsification-vitrectomy surgery was performed in 50 eyes of 45 patients who had various vitreoretinal diseases and a significant cataract that is considered to disturb the postoperative visual rehabilitation or fundus examination. Main outcome measures were visual acuity and intraoperative and postoperative complications. RESULTS: We performed combined surgery consisting of pars plana vitrectomy (PPV), phacoemulsification, and posterior chamber intraocular lens (PC-IOL) implantation. Intraocular lens were inserted in the bag with one case of exception inserted in the ciliary sulcus. Thirty-seven eyes (74%) had improved postoperative visual acuity, in 9 eyes (18%) the acuity was unchanged and 4 eyes (8%) it deteriorated. Intraocular pressure elevation, recurrent vitreous hemorrhage, macular edema, retinal detachment and endophthalmitis were considered to be clinically significant postoperative complications. CONCLUSIONS: The results of this study suggest that combined phacoemulsification-vitrectomy surgery is a viable option in the management of most posterior segment diseases in the presence of cataract.

8.A Case of Subcutaneous Granuloma Annulare.

We report herein a case of subcutaneous granuloma annulare occured in a 3-year-old girl who had 7 subcutaneous nodules on the scalp, lumbar area, buttock, right tibial area and both ankles. liistopathologically, multiple foci of complete and incomplete collagen degeneration surrounded by histiocytes in a palisading arrangement were seen in the subcutaneous layer. Some vessel walls were thickened and infiltrated with inflammatory cells and many eosinophils were scattered among the inflamrnatory cell infiltrate around the palisaiing granuloma. Five of the lesions disappeared spontaneously and other s decreased in size without specific treatment in 2 months.

10.Analysis of Vitrectomy Results in Diabetic Macular Edema using Optical Coherence Tomography.

PURPOSE: This study evaluated the effectiveness of vitrectomy for diabetic macualr edema(DME) using optical coherence tomography (OCT). METHODS: The type of macular edema, macular center thickness and state of posterior hyaloid membrane were checked using OCT in 12 eyes of 10 patients. Visual acuity was checked and the follow-up after vitrectomy was at least 4 months in all eyes. RESULTS: There was a significant decrease in mean macular center thickness from preoperative 414.6 +/- 101.3 micro meter (mean +/- SD) to postoperative 223.9 +/- 73.6 micro meter (P=0.002). We performed combined cataract surgery in 10 eyes (83%). Visual acuity was improved by 2 or more lines in 8 eyes (67%), but there was no statistical relationship between the visual improvement and the amount of macular center thickness decrease. The decrease of macular center thickness was more effective in cystoid macular edema, the absence of posterior vitreous detachment and the presence of previous macular laser treatment, but there was no statistical significance. CONCLUSIONS: OCT may be helpful in quantitative analysis of the results of vitrectomy for DME.
