2.Mass Screening for Stomach Cancer by Gastrofiberscopy in 1865 Employees of a Car Manufacturing Company.

Background: Stomach cancer is the most commonly diagnosed malignant tumor in Korea. Early detection by mass screening very important to increase survival rate of stomach cancer, because the prognosis of early gastric cancer is excellent, and specific symptoms and physical findings are rare with early gastric cancer. Methed: 1865 car manufacturing company employees were examinined by gastrofiberscopy at a dispensary of the company from May 6, l992 to July 13, 1992. Results: 1. Number of cases with normal finding was 1262(67.7%), gastritis 422(22.6%), gastric ulcer 47(2.5%), gasrtric cancer 4(0.2%), duodenitis 39(2.1%), duodenal ulcer 63(3.4%), combined ulcer(gastric & duodenal) 2(0.1%,), post operative state 6(0.3%), and others 4(0.3%) in total 1865 examinees. 2. Of 422 cases with gastritis, cases with superficial gastritis were 171 (9.2%), atrophic gastritis 138(7.4%), erosive gastritis 94(5.0%) and hemorrhagic gastritis 19(1.0%). 3. Of 4 cases with stomach cancer, 1 case was advanced stomach cancer and 3 cases were early gastric cancer(two cases of IIc and one case of IIc+III) by macroscopic classification. Conclusion: Mass screening provides an excellent opportunity to detect gastric cancer in populations without symytoms or with nonspecific symptoms. It should be recommended to undergo screening exam for high risk group of stomach cancer.

3.Case report for Blepharospasm requiring careful considerations in the application of the blepharoplasty.

With the aging process, the senile lids are manifested by the excessive skin, decreased height of palpebral fissures with interference of the vision. Sometimes these patients often complained the blepharitis on the lateral canthal area and phtophobia. But there are many diseases to differentiate from senile lids such as dermatochalasia, blepharochalasia, hypertrophy of orbicularis oculi, herniation of orbital fat, myasthenia gravis, and blepharoptosis. Blepharospasm must also be differentiated because of sysmptoms similar to the above described senile lids. Essential blepharospam is an idiopathic progressively debilitating desease which involve bilateral spasm of the orbicularis oculi. So just only an conventional blepharoplasty could not get an improvement of symptoms and some times it makes the symptoms worsen. This case report is about blepharospasm patients visiting for blepharoplasty. And the patients got relatively the satisfied results with the injection of botulinum A toxin, aided by neurologist, instead of blepharoplasty. In conslutions, The blepharospasm in required to detect in preoperative evaluation for blepharoplasty. Once the blepharospasm is diagnosed, it is better to cooperate with neurologist and to do the blepharoplasty.

7.Factors affecting foveal threshold measured by short-wavelength automated perimetry in primary open angle glaucoma.

PURPOSE: To evaluate the factors affecting foveal threshold measured by blue on yellow perimetry in moderate to severe open angle glaucoma. METHODS: Fourteen eyes of 14 patients who were diagnosed as moderate to severe open angle glaucoma underwent macular threshold test of the Humphrey Field Analyzer using white and blue targets. The foveal threshold using white targets (white threshold) was more than or equal to 18 dB in all cases. The cases of foveal threshold using blue targets (blue threshold) less than or equal to 0 dB were defined as foveal sensitivity loss group (7 eyes) and those more than 0 dB as no foveal sensitivity loss group (7 eyes). Those of age, contrast sensitivity, visual acuity, refraction, mean deviation, pattern standard deviation, corrected pattern standard deviation, vertical and horizontal C/D ratio, foveal white threshold, intraocular pressure, any point within central 5degrees with sensitivity less than or equal to 0 dB in visual field test were statistically analyzed between the two groups. RESULTS: Foveal white threshold and any point within central 5degrees with sensitivity less than or equal to 0 dB in visual field test showed significant difference between the two groups. CONCLUSION: Foveal dysfunction represented as decrease in blue threshold is possibly related to the paracentral scotoma which means any point within central 5degrees with sensitivity less than or equal to 0 dB in moderate to severe open angle glaucoma. However, significant deficits in either visual acuity or white threshold were not presented.

8.Double eyelid operation in blepharoptosis patients.

In correction of ptosis, symmetry of lid contour, lash position and lid height must be matched. Most of blepharoptosis patients want to perform both the double eyelid operation and ptosis repair at the same time to achieve the best cosmetic and functional result. Using a surgical technique directed at the levator aponeurosis or the frontalis muscle with double eyelid operation, we corrected 13 cases of unilateral and 10 cases of bilateral congenital blepharoptosis. In unilateral ptosis with good or fair levator function, levator resection was performed and the position of the lid margin was adjusted at 1~2 mm below the superior limbus. The height of the lid crease of the ptotic eye is determined 1~3 mm lower than that of normal side. The position of the lid margin is adjusted at the level of the superior limbus and the height of the lid crease of the ptotic eye is determined 2~4 mm lower than that of nonptotic side. The advantages of this approach are normal anatomic planes of the eyelid are maintained, to obtain symmetry of both eyelids relatively easily, entropion may be prevented, and to achieve aesthetically pleasing results.

10.No Association between DRD2 polymorphism Ser311/Cys311 and Bipolar Disorder in Korean Population.

OBJECTIVES: This study was designed to investigate the association between Ser311/Cys311 polymorphism in the dopamine D2 receptor gene and bipolar disorder in korean population. METHOD: Ser311/Cys311 polymorphism in the dopamine D2 receptor gene was typed with PCR in 86 bipolar disorders and 100 normal controls. RESULTS: Genotype of Ser/Ser, Ser/Cys, Cys/Cys were 82 (95.3%), 4 (4.7%), 0 (0.0%) respectively in the bipolar disorders, 96 (96.0%), 4 (4.0%), 0 (0.0%) respectively in the controls. Allele frequencies of Ser and Cys were 168 (97.7%), 4 (2.3%) in the bipolar disorders, 196 (98.0%), 4 (2.0%) in the controls. There were no differencies in genotype distribution and allele frequencies of dopamine D2 receptor gene polymorphism Ser311/Cys311 between in the bipolar disorders and in the controls. CONCLUSION: These result suggests dopamine D2 receptor gene polymorphism Ser311/Cys311 is not causally related to the development of bipolar disorder in korean population.
