1.Alternative impression technique for multiple abutments in difficult case to control.

BACKGROUND: Even though excellent impression materials are now available for making accurate replication for hard and soft tissue, the numerous dentists have faced lots of obstacles in making simultaneous impressions of multiple abutments. CASE DESCRIPTION: This article describes a modified method of tray fabrication using auto-polymerizing acrylic resin and impression technique for multiple prepared teeth in cases with limitations and difficulties in taking dental impressions. CLINICAL IMPLICATION: This segmental tray technique has several advantages, including higher impression quality, fewer impressions, and being more comfortable for the patient and less stressful for the clinician.

2.Echocardiographic Changes after Mitral Valve Replacement.

Echocardiographic examination was performed before, immediately after, 4-6 months after and 10-12 months after mitral valve replacement(MVR) surgery in 46 patients with mitral valve disease(8 patients with mitral regurgitation, 24 patients with mitral stenosis and 14 patients with mitral stenosufficiency) to evaluate the effects of mitral valve replacement on dimension of left atrium and left ventricle, volume of left ventricle, ejection fraction(EF) and fractional shortening(FS) of left ventricle. The results are as follows : 1) The endsystolic dimension(ESD), enddiastolic dimension(EDD), endsystolic volume(ESV) and enddiastolic volume(EDV) decreased significantly after operation in patients with mitral stenoinsufficiency(MSR), the ESD, EDD, ESV and EDV increased significantly after the operation, but returned to preoperative value 10-12 months after the operation. 2) The EF and FS of left ventrcle after MVR were significantly lower than preoperative value throughout the postoperative period in patients with MR. However in patients with MS or MSR, there were no significant postoperative changes in EF and FS, except transient depression in the patients with MS at the immediate postoperative period. 3) In all patients with mitral valve disease, the left atrial dimension and the ratio of domension of left atrium to the dimension of aorta decreased significantly after MVR. From above results, it is suggested that surgery should be considered seriously for the patients with MR before the ESD, EDD and ESV increase maekedly, even if the EF anf FS are in normal range and the symptoms are not severe, to prevent irreversible depression of myocardial function. It seems that serial echocardiographic examination is very helpful in this respect.

4.The Association of Job Characteristics and Stress Indicators in TV Manufacturing Plant Workers.

This study was conducted to evaluate the association of job characteristics and stress indicators, from May 1st to 10th, 1996. The study population was 210 workers (clerical workers, 67; engineers, 82; assembly line workers, 61) engaged in a television manufacturing company. A questionnaire method was used to collect the data regarding to job demand, job control, behavioral type, life style and psychiatric symptoms. Clinical examination was conducted to measure blood pressure, serum cholesterol, fasting blood sugar and HbA1c. Percentage of high subjective job strain group was significantly higher in clerical workers(9.0 %) than in engineers(4.9 %) and in assembly line workers(3.3 %) (p < 0.01). Mean values of fasting blood sugar and HbA1c were significantly different between types of occupation and were the highest in clerical workers (p < 0.05). Significant association was found between perceived job characteristics and psychiatric symptom score (p < 0.01). The more job demand and the less job control was, the higher psychiatric symptom score was reported. Diastolic blood pressure was significantly related to job control (p < 0.05). The more job control was, the higher blood pressure was reported. In multiple regression analysis, job demand, job control and age were significant variables explaining the psychiatric symptoms and R2 was 0.23. Perceived job characteristics and type of occupation were not significant in explaining diastolic blood pressure and cholesterol. Type of occupation, job demand, age and BMI were significant variables explaining lasting blood sugar and R2 was 0.25. Type of occupation was the only significant variable explaining HbA1c and R2 was 0.07. According to this result, occupational type and subjective job characteristics must be considered in study of job stress. As a result of multiple analysis, subjective job characterics are significantly related to psychiatric symptoms and type of occupation is significantly related to fasting blood sugar and HbA1c.

6.Congenital Mesoblastic Nephromas with lmmunohistochemical and Flow Cytometric Analysis.

We reviewed 7 cases of congenital mesoblastic nephroma (4 cases of classical mesoblastic nephroma (CMN) and 3 cases of atypical mesoblastic nephroma (AMN)) using immuno-histochemical and flow cytometric study. Results are as follows. 1) The mean tumor size was 5 (3 to 7cm)cm in CMN and 9 (7 to 10cm)cm in AMN. The AMN revealed hemorrhage and necrosis in two Of three cases. A case of AMN showed cystic change without hemorrhage and necrosis. Mitotic count ranged in 0~4/10HPF in CMN and 20-35/10HPF in AMN. 2) Immunohistochemistry for vimentin was all positive. Actin, desmin were weakly positive in CMN, but negative in AMN. The findings were consistent with myofibroblastic differentiation in CMN and AMN was considered to be the less differentiated form of CMN. 3) Flow cytometiic analysis showed diploidy in two of two CMNs and two of three AMNs. Only one AMN showed aneuploidy with DNA index of 1.41. %SG2M were 8.1 and 15.9 (mean 12.0) in CMN and 16.9, 32.9 and 19.3 (mean 22.9) in AMN, respectively. We concluded that AMN should be distinguished from CMN, clinicopathologically.
7.A Case of Incontinentia igmenti Achromians ( Ito ).

Incontinentia pigmenti achromians (Ito) begins during childhood and is characterized by progressive appearance of bizarre patchy or whorl-like hypopigmentation without any preceeding or associated inflammatory changes in a fashion that resembles a negative piture of pigmentation in patients with incontinentia pigmenti. Mental, bony and ocular symptoms are not infrequently found in association with incontinentia pigmenti achromians. The incidence is high in the female sex, and there is no hereditary background. A 24-year-old Korean soldier was first seen in May 1974 in dermatologic clinic of Capital Armed Forces General Hospital. Bizarre, reticulated, linear and whorllike hypopigmented macular lesions were noted. On the skin of the trunk and both extremities. The lesions started at the age of 4 year and developed without any inflarnmatory signs Family history did not disclose any type of pigmentation The disorder, and mental, bony and occular manifestations were not found. Biopsy specimen demonstrated that the amount of melanin in the basal layer was decreased in the hypopigmented area, and reveals neither inflammatory changes nor dropping off of melanin granules into the dermis.
