2.A Case of Multiple Bowen's Disease with Small-Cell Lung Cancer.

Bowen's disease represents an intraepidermal squamous cell carcinoma and can be associated with internal malignancy. Internal cancers originate predominantly from respiratory, gastrointestinal, and genitourinary tracts. A 71-year-old man presented with erythematous crusted plaques with emsion on the right lower abdomen, left elbow, and left 4th finger. He had been diagnosed with small-cell lung cancer 17 months ago. The histopathologic features of the skin lesions showed Bowens disease. The abdominal lesion was removed by surgical excision and the lesions on the elbow and finger were treated by topical 5-FU application.

3.Hypertrichosis in a Woman During Treatment with 3% Topical Minoxidil.

Hypertrichosis has been frequently observed during the oral administration of the potent antihypertensive agent, minoxidil. However, hypertrichosis is uncommon after treatment with topical minoxidil for alopecia, and usually occurs in areas close to the site of the application. We describe a 52-year-old woman with diffuse facial hypertrichosis. She developed abnormal hypertrichosis while applying the optimal dose of 3% topical minoxidil for 2 months for the treatment of androgenetic alopecia.

4.Unusual Type of Talar Body Fracture: A Case Report

Fractures of the body of the talus are extremely rare, constituting only about 0.1 percent of all fractures (Coltart 1952). Consequently, the prognostic problems have so far been elucidated only on the basis of few reports in the literature. The authors experienced an unusual type of the shearing fracture of the talar body with total disocation that the talar body was lying upside down in the ankle joint. Satisfactory results were obtained after follow up period of 20 months and some are reporting this case with bibliographic reviews.

5.Assessment of Aortic Regurgitation by Real-time Two-dimensional Doppler Flow Mapping System.

In the real-time two-dimensional Doppler flow mapping(2DD) system, Doppler signals are processed using auto-correlation technique, so that the direction, velocity and turbulence of the intracardiac blood flow are displayed by coloration on the B mode image of the heart in real time. Aortic regurgitant flow is imaged as a mosaic jet spurting out from the aortic valve orifice to cardiac chamber. Dynamic features in the direction and extent of regurgitant flow and the site of regurgitation on the aortic valve orifice are readily obtained. Feasibility of the 2DD system in the assesment of aortic regurgitation was examined in 30 cases documented by angiography. In 30 cases, there were 20 cases with aortic regurgitation(AR) aged 16 to 57 years(mean 34) and 10 cases without AR aged 17 to 39 years(mean 30). The underlying disorders of AR were rheumatic in 15 cases, bicuspid aortic valve in 1, ventricular septal defect in 1, aortitis in 1, Marfan's syndrome in 1 and unknown in 1. The results are as follows : 1) In 19 out of 20 cases with AR the 2DD showed regurgitant jet spurting out from valve orifice(sensitivity=95%). One case missed by the 2DD had 1+AR. None of the 10 cases who had no AR manifested the evidence of AR on the 2DD(specificity=100%). 2) There was an excellent positive correlation between the maximal jet length of regurgitant flow on the 2DD and angiographic severity of regurgitation(r=0.998, p<0.001). The maximal jet length was less than 3cm for cases with 1+, 3-4.9cm for 2+, 5-5.9cm for 3+, and 6cm or more for 4+AR. 3) In 19 out of 20 cases with AR, the 2DD identifed the anatomic valvular site of regurgitation(sensitivity=95%, specificity=100%). 4) The regurgitant aortic valvular area was measured as 0.6cm2 or less in all of 12 cases with 2+ or less AR, while 0.9cm2 or more in 6 out of 7 cases with 3+or more AR. Thus, a less or greater than 0.8cm2 regurgitant aortic valvular area provides to discriminate between mild(< or = 2+) and severe(> or = 3+) AR. 5) In all 8 cases with fluttering of anterior mitral leaflet and 8 out of 9 cases with fluttering of interventricular septum, a regurgitant jet impinged on them. The results of this investigation indicate that the 2DD system is a very useful and unique noninvasive technique in the detection, estimation of severity and spatial orientation of AR.

7.Periumbilical Perforating Pseudoxanthoma Elasticum.

Periumbilical perforating pseudoxanthoma elasticum (PPPXE) is a localized acquired disorder found most frequently in obese, multiparous, middle-aged women. It is characterized clinically by yellowish, lax, well-circumscribed, reticulated or cobblestoned patches or plaques in the periumbilical region. Multiparity, obesity, massive ascites, and abdominal surgery are thought to the initiating factors. There is controversy about the etiology of PPPXE. Some authors have classified it as a separate entity from hereditary pseudoxanthoma elasticum (PXE), while others speculate that this condition merely represents a variable expression of PXE in which systemic associations are likely. We report a case of periumbilical perforating pseudoxanthoma elasticum associated with a clinical PXE lesion on the anterior neck.

8.Partial Unilateral Lentiginosis.

Partial Unilateral Lentiginosis (PUL) is a rare pigmentary disorder characterized by the numerous lentigines confined to a body segment, with a sharp demarcation at the midline. We report two cases of PUL. A 38-year-old woman had asymptomatic discrete small hyperpigmented macules that were scattered on the T7~L1 dermatomes on the left side of her trunk and were clearly demarcated in the midline on both anterior and posterior sides. In the other case, an 18- year-old woman had hyperpigmented macules that were scattered on the left neck, shoulder, and anterior chest. Other anomalies including neurofibromatosis, neurologic anomalies, and multiple lentiginous syndrome were not related in both cases. We report two cases of PUL having no other anomalies.

10.Neurolytic Blockade of the Ganglion Impar for Relief of Cancer-Related Perianal Pain: A case report.

The perineum refers to the anatomical area immediately below the pelvis, and is comprised of diverse anatomic and somatic innervation. Although various interventions have been proposed for the management of intractable cancer-related perineal pain, their efficacy are limited because the target of nerve blocks has focused on somatic rather than sympathetic components. Recently, blockade of the ganglion impar has been introduced as an alternative means of managing intractable neoplastic perineal pain of sympathetic origin. We successfully treated a patient who had suffered from cancer-related intractable perineal pain with ganglion impar block.
