1.A Case of pemphigus Vulgaris Expired by Acute Renal Failure.

Pemphigus is a rare, blistering autoimmune disease in which the action of antibodies is now well established. The most effective therapy is the administration of cortioseroids. Prior to the use of steroids, it was commonly fatal. Since the availability of corticosteroidsnortality has been markedly reduced. However prolonged administration of steroids may be difficult to sustain because of severe side effects. For these, other sdjuvant therapies were considered. Such as immunosuppresive drugs, gold, dapsone, plasmapheresis and intralesional injection of corticosteroid. Of these, immunosuppressive therapies are most commonly used. Recently cyclosporine which was for human organ transplantation is used in dermatology. Cyclosporine is a potent immunosupprc save agent with no appreciable effect on bone marrow and a selective inhibition to helper T cells. In our case, cyclosporine was administrated to the patient who had suffered from severe pemphigis vulgaris for 3 years without responding to treatment. We gave cyclosporine 400mg and prednisolon, 90mg per day. But we found a sudden increase of the RUN/Cr level and a shut down of the renal funtion. Therefore we discontinued cyclosporine and treated acute renal failure. However the renal damag e was irreversible and two days later the patient died.

2.Congenital Mesoblastic Nephromas with lmmunohistochemical and Flow Cytometric Analysis.

We reviewed 7 cases of congenital mesoblastic nephroma (4 cases of classical mesoblastic nephroma (CMN) and 3 cases of atypical mesoblastic nephroma (AMN)) using immuno-histochemical and flow cytometric study. Results are as follows. 1) The mean tumor size was 5 (3 to 7cm)cm in CMN and 9 (7 to 10cm)cm in AMN. The AMN revealed hemorrhage and necrosis in two Of three cases. A case of AMN showed cystic change without hemorrhage and necrosis. Mitotic count ranged in 0~4/10HPF in CMN and 20-35/10HPF in AMN. 2) Immunohistochemistry for vimentin was all positive. Actin, desmin were weakly positive in CMN, but negative in AMN. The findings were consistent with myofibroblastic differentiation in CMN and AMN was considered to be the less differentiated form of CMN. 3) Flow cytometiic analysis showed diploidy in two of two CMNs and two of three AMNs. Only one AMN showed aneuploidy with DNA index of 1.41. %SG2M were 8.1 and 15.9 (mean 12.0) in CMN and 16.9, 32.9 and 19.3 (mean 22.9) in AMN, respectively. We concluded that AMN should be distinguished from CMN, clinicopathologically.

4.Anticancer Effect and Apoptosis of All-trans-retinoic Acid on the Human Ovarian Epithelial Carcinoma Cell Lines.

Ovarian carcinoma is a serious disease in women. Some reports revealed all-trans-retinoic acid (tRA) inhibited the proliferation of ovarian carcinoma cell lines and induced apoptosis. The aim of this study was to evaluate the anticancer and apoptotic effects of tRA and the expression of the retinoic acid receptor (RAR) alpha, beta, gamma, p53, bcl-2, and c-myc genes on the ovarian carcinoma cell lines, NIH OVCAR3 and SKOV3. In both cell lines, the proliferation of tumor cells was inhibited and characteristic morphologic patterns of apoptosis were shown after treatment of tRA. The number of apoptotic cells and the percentage of apoptosis were increased after treatment of tRA. The gel electrophoresis revealed the DNA ladder pattern in the NIH OVCAR3. Gene expressions were observed using northern blotting. There was no RARalpha expression in both cell lines. In NIH OVCAR3, there was no changes in the expression of RARbeta and bcl-2 gene. The RARgamma gene expression of tRA treated group was slightly increased, but p53 gene expression was decreased, and c-myc gene was not expressed. In SKOV3, the expressions of RARbeta, gamma, and p53 genes were increased and that of bcl-2 was decreased in the tRA treated group. There was no change in c-myc gene expression. These results suggest tRA has anticancer and apoptotic effect on both ovarian carcinoma cell lines. RARbeta, RARgamma, bcl-2, and p53 gene expressions were correlated with these effects of tRA on SKOV3 but not on NIH OVCAR3.

5.Experience of Double J Stents on Urinary Diversion.

Double J stents have been advocated for drainage or splintage of the ureter. Fifty-four attempts at double J stent insertion were made in 46 patients. In 93% attempts the stent was satisfactorily placed. Their use in adjunct to urologic and gynecologic surgery, percutaneous nephrolithotomy and ureterorenoscopy has been encouraging but poor results have been obtained in patients with ureteral obstruction due to tuberculosis and malignancy.

8.Laparoscopy-assisted Billroth I Gastrectomy Compared with Hand-assisted Laparoscopic Surgery for Early Gastric Cancer -A Prospective Study-.

PURPOSE: To compare standard laparoscopy-assisted Billroth I gastrectomies including standard lymph node dissection (LABIG) with hand-assisted laparoscopic surgery with the HandPort system (HALS) for the removal of early gastric cancers (EGC). METHODS: A prospective study was performed on 26 patients of EGC at Ewha Womans University Mok-Dong Hospital from July 1999 to August 2001. Seventeen patients (Group L) received LABIG using conventional laparoscopy-assisted methods and 9 patients received LABIG using HALS (Group H). We used staplers for the anastomosis, and a standard D2 lymph node dissection was done with ultrasonic shears or electrocautery. RESULTS: In group L, pathologic reports revealed 14 EGC (stage IA 14 cases), and 3 pm cancers (stage IB 1 case, II 2 cases). In group H, there were 9 early gastric cancers (stage IA 8 cases, IB 1 case). Significant differences (P<0.05) were present between group L and H in regards to the number of harvested lymph nodes (30.8 vs 18.9), estimated blood loss (462.1 vs 286.7 ml) and postoperative transfusion amounts (0.59 vs 0 unit). There were no differences in the mean operating time, distance from the lesion to the resection margin, postoperative leukocyte count, frequencies for pain control, wound size, time to diet, weight loss, serum protein, and postoperative hospital stay. Complications were present in 1 case in group L (enterocutaneous fistula) and 1 case in group H (gastric atony). There was one conversion to open surgery in group H. CONCLUSION: LABIG including standard lymph node dissections with both standard laparoscopic surgery and HALS were performed with equal outcome. The choice of surgical method depends on the characteristics of the lesion and the patient's physical factors.

9.A case report on correction of Angle's Class III malocclusion with macroglossia.

This is case report of true class III malocclusion with macroglossia is corrected by glossectomy in 13 years female patient. After orthodontic treatment, the patient is bound to glossectomy because the corrected condition is relapsed to the previous condition due to relatively enlarged tongue compared with the original dental arch. By the interpretation of the cephalogram and model analysis, it is approved that the growth pattern and direction are normal range and mandible is located anterioly to the cranium. The results are follows: 1. We could treat the true Cl III malocclusion. 2. We could prevent the relapse of the treated condition by the surgical intervention, such as partial glossectomy. 3. Sensory, speech, swallowing and so other functions after the operation have been with in normal limit without any serious complications or seguellae.
