1.Hybrid Total Hip Replacement Arthroplasty: Minimum 3 Year Follow - Up Result.

Fifty-eight primary total hip arthroplasties with hyhrid cemented femoral component using contemporary cementing technique and cementless acctahular cup were followed for a minimum 3 year(ranged, 36 to 67 months, mean 4.1 years). Thc average age of the patients at the time of the operation was 54 years(ranged, 22 to 73 years). The preoperative common diagnosis were avascular necrosis of femoral head in 38 hips and miscellaneou. in 20 hips. The mean Harris hip score was 61 in preoperative and 91 in postoperative at last Follow-up. 5-1 hips(92%) were rated excellent and good clinically. One hip was revised for aseptic loosening of the femoral stem at 52 months follow-up. Radiographically no acetahular component was loose. Three femoral stems were definitely loose, one stem was classfied as prohahle loosenine and another one was ratde as possible loosening. There was no progressive radiolucent dermaation ol' cement-hone interf'ace in stable fermoral steni. Non progressive cement-bone radiolucency was lound seven hips in zone 1, two hips in zone 6, one hip in zone 2 and one hip in zone 7 at last follow-up. There were two postoperalive dislocations. Other complications included superficial infection(two hips) and sciatic nervc palsy(one hip). Our experience showed that the hyhrid hip replaccment with improved cementing technique provided satisfactory results at minimum 3 year follow-up study and emphasized the importantance of careful cementing techniques in survival ot cemented I'cmoral stem.

2.A Case of Cheolitis Granulomatosa.

Cheilitis granulomatosa is one of the rare dermatoses which is characterized by a sudden onset and a progressive course termination in chronic enlargement of the lips Usually, enlargement of lip is the only symtom without ulceration, fissurign or scaling. It may be considered a part of the Melkersson-Rosenthal syndrome and the etiology is still unknown. A 35 year old female with the clinlcal symptoms of cheilitis granulomatosa is presented; the diagnosis was confirmed by histopathologic study.

4.The experiments on the infectivity to mice of the Ascaris eggs irradiated with Cobalt(60).

In order to find out a valuable control measure for soil-transmitted parasties, the infectivity in mice of Ascaris eggs irradiated with Cobalt(60) were examined. The results were summarised as follows. In vitro, Ascaris eggs irradiated with larger doses of Cobalt(60) developed poorly, and no difference was found between fresh eggs and those cultured for a few days. Ascaris eggs irradiated with doses of 200,000 rad. developed at the rate of 90 percent after 4 weeks, whereas those irradiated with 1,000,000 rad. developed 28 percent. Ascaris eggs irradiated with Cobalt(60) after 2 weeks of culture were poor in development compared with those of 4 week culture. Eggs cultured for 5 weeks showed weaker infectivity in mice than those cultured for 8 weeks. In the control groups, the infectivity in mice of Ascaris eggs was remained the same between 5 and 8 weeks. The minimum dose of Cobalt(60) irradiation effective for preventing infectivity in mice was estimated to be 200,000 rad.

6.Primary Coronary Stenting as a Successful Treatment of Acute Myocardial.

Fewer than one half of patients with acute myocardial infarction is a candidate for thrombolytic therapy. Current data revealed that primary coronary stenting may be useful alternative to intravenous thrombolytic therapy for acute myocardial infarction in these subset of patients. We experienced a patient presenting with acute myocardial infarction and acute promyelocytic leukemia in whom thrombolytic therapy was thought be not eligible due to hemorrhagic tendency. Primary coronary stenting was performed successfully without complications. Follow-up angiography revealed no evidence of restenosis or stent occlusion.

7.The Effect of the Extents of Pleural Thickening in Tuberculous Pleurisy on the Impairment of Pulmonary Function.

BACKGROUND: Tuberculous pleurisy treatment improve symptoms such as fever, chest pain, cough, and prevents the progression to active pulmonary tuberculosis and the development of residual pleural thickening that decrease diaphragm and rib cage movement. This study investigated how the degree of residual pleural thidkening affects the pulmonary function. METHODS: Fifty seven patients who were initially diagnosed as having tuberculous pleurisy, were treated with anti-tuberculous medication for 6 months and had residual pleural thickening between May 1998 and January 2000 at the Eulji university hospital were reviewed. A chest X-ray and pulmonary function test(PFT, Sensormedics 2200) were perfored. The predicted value (%) of the forced vital capacity(FVC), forced inspiratory vital capacity(FIVC) and total lung capacity(TLC) were measured. The residual pleural thickening was defined the average of the summation in the lateral chest at the level of the imaginary line intersecting from the cardiophrenic angle to the diaphragmatic dome and the lowest part of the costophrenic angle between them. The results were sorted into three grades according to pleural thickness; <2mm(grade I), 2~10mm(grade II), 10mm(grade III). RESULTS: 1. FVC(% pred) and FIVC(% pred) were statistically different between grade I and III, and II and III. However, there was no difference between the TLC(% pred) between each of the groups. 2. The pleural thickness that cause restrictive dysfunction(FVC<80%) and a statisticall difference, is 3 mm. CONCLUSION: The larger the extent of the residual pleural thickness after antituberculous medication, the greater the reduction in the FVC, FIVC, TLC. A pleural thickness of 3 mm is recommended as a guideline for diagnosing a restrictive pulmonary dysfunction.

8.The Results of Modified Trabeculectomy with a Slicone Tube for Eyes with Neovascular Glaucoma.

Neovascular glaucoma is considered to be one of the most difficult forms of glaucoma to control. In neovascular glaucoma fibrovascular tissue grows over the surface of the iris and trabecular meshwork. Many modified filtering surgery with artificial drainage devices have been tried in attempts to maintain openings for aqueous humor to leave the eye. Modified trabeculectomy was performed with inexpensive easily available simple silicone tube in 8 eyes with neovascular glaucma. Of the 8 eyes, 6 had postopeoative intraocular pressures lower rhan 25mmHg through a mean follow-up period of 6 months.

9.Fine structure of Toxoplasma gondii.

The importance of Toxoplasma gondii in human disease stimulated a number of electron microscope studies on the structure of this protozoan parasite. Gustafson et al. first studied the fine structure by means of thin sections in 1954. Many other papers havs subsequantly appeared. It is well known that Toxoplasma gondii has two stages in its life cycle-the proliferative forms and the cyst. The purpose of the electron microscopical work reported here was to study the fine structure of Toxoplasma gondii with recent techniques clarifying the correlation between the proliferative forms and cyst. RH strain and KM strain as proliferative forms on the one hand and Beverley strain as a cyst form of Toxoplasma gondii on the other hand were used throughout this study. The conoid, toxoneme, nucleus, nucleolus, osmiophilic granules, mitochondria and vacuoles were found in RH strain as wsll as in KM strain and Beverley strain. The endoplasmic reticulum was found in the cytoplasm of RH strain and KM strain. It was better developed in KM strain than in RH strain. The outside contour of the organism of Beverley strain was somewhat irregular and toxoneme of this organism was better developed than in the other two strains. Vacuoles were found in RH strain, KM strain and Beverley strain. Furthermore, tube-like bodies were observed in the vacuoles of the organism of RH strain. In KM strain, two organisms of the same size were demonstrated in the leucocytes. It was presumed that they were products of longitudinal division.

10.Chiari Pelvic Osteotomy in Children and Adolescent.

Twenty five patients (twenty seven hips) who had Chiari osteotomy at Kang Nam St. Marys Hospital between 1980 and 1995 were reviewed to evaluate the factors in the operative technique that contribute to successful outcome and assess the clinical results in various conditions. The length of follow-up ranged from one to fourteen years and the age of at operation ranged from four to twentythree years. Eighteen patients had developmental dysplasia of the hip: four, septic hip: three had another disorders, Prior to the Chiari osteotomy, fourteen hips had an femoral osteotomy and four, trochanteric arthroplasty. We used to the standard osteotomy as described by Chiari with certain modification. A pneumatic saw and osteotome are used instead of Gigli saw. This technique is simple procedure to make the correct level and angle. Bone graft was not performed in all cases even the osteotomy was displaced more than 50 percent of the iliac width. The overall results were 12 excellent, eight good, five fair, and two poor. In eleven patients, the osteotomy had to be displaced more than 50 percent to provide good coverage of the femoral head. Their results were good or excellent. A good result will be obtained if enough attention is paid to displacing the osteotomy. The osteotomy using the pneumatic saw provides accurate level and direction of osteotomy and it is an simple procedure also.
