1.A Morphologic Study of the Structural Changes of Normal Aging Facial Skin.

In an attempt to elucidate the morphologic changes of normal aging skin, the present study was undertaken in human facial and chest well skin of individuals aged from 4 months to 76 years old. Biopsied skin was studied with light microscope, scanning electron microscope, and transmission electron microscope, using both conventional and tannic acid staining. The morphologic changes in the skin were noted as follow: 1. Structurally, the aged epidermis gradually became somewhat thinner, with flattening of the dermoepidermal interface. The number of melanocytes also decreased. 2. Abnormalities of elastic fibers such as loss of oxytalan fibers were observed from age 30. It was the initial sign of the aging process of elastic fibers, followed by abnormal changes in elaunin and mature elastic fibers. The degree of facial skin abnormality was rather more severe than that of the chest wall skin. 3. In individuals more than 50 years old, the age related changes in mature elastic fibers were more severe. Transmission electron microscopically, the electron density in elastin was irregular. The elastic fibers also showed pores and irregular splitting with fragmentation. Fine granular materials were scattered near the elastin. 4. Scanning electron microscopically, the elastic fibers in young adult skin showd ribbon-like fibers aligned in the same direction. They were either cylindrical or elliptical, having smooth surface. In old skin, the elastic fibers ran in various directions, forming complicated networks. These were larger, more elliptical and more branched than those in young adult skin. In summary, histologic changes of aging are much more prominent in sun-exposed skin(facial skin) than in sun protected skin(chest wall skin). A completely different spectrum of elastic fiber abnormalities was found in individuals more than 30 years old. The result indicates that elastic fiber abnormalities are related to aging skin.

3.The Result of Sugioka's Transtrochanteric Rotational Osteotomy

Avascular necrosis of the femoral head is generally localized in the anterosuperior aspect of the head. The posterior part of the head often retains a normal, smooth contour containing healthy cartilage even in much advanced cases. Because of this anatomical localization of the lesion, Y. Sugioka has devised a transtrochanteric rotational osteotomy in young and middle-aged person, which reduces the weight-bearing force on the necrotic area, and transfers shearing forces to the healthy posterior cartilage of the femoral head to prevent progressive collapse of the articular surface and improve alignment when articular collapse followed femoral head subluxiation. The authors reviewed 13 cases of Sugioka operation, performed at the Department of Orthopedic Surgery, Seoul National University Hospital during the period from December 1983 to Feburary 1986 and the following results were obtained. 1. There were 11 males and 1 female. The average age of the 13 cases was 33.3 years. 2. In all the cases of Grade I and Grade II hips, there was not progressiveness of necroticarea of femur heads. 3. By U.C.L.A. hip rating system, preoperative mean scores of pain, walking ability, function and activity were 2.5, 5.7, 4.8 and 4.3, respectively, while postoperative scores of these were increased to 7.7, 9.5, 8.4 and 7.8, respectively. 4. Average ranges of motion of hip joint were increased more postoperatively than preoperatively.

4.Systemic Lupus Erythematosus Showing Vesiculobullous Eruptions: Report of Two Cases.

l6-year-old male and 48-year-old female with systemic lupus erythematosus (SLE) and renal disease presented with vesiculobullous eruptions during severe attack of the disease. They showed hypocomplementernia, and high ANA titers. Histopathologic findings revealed subepidermal blister and leukocytoclastic vasculitis, Direct immunofluorescence demonstratedlinear deposits of IgG, IgA and lgM at the dermoepidermal junction. The vesiculobullous eruption of SLE may be an important marker reflecting disease activity and prognosis.

6.A Case of Eosinophilic Pustular Folliculitis.

We report herein a case of eosinophilic pustular follicultis in a 20-year-old man. The patient showed typical clinical picture with specific laboratory and histopathological findings of eosinophilic pustular folliculitis. The patient responded well to systemic administration of corticosteroid and dapsone.

7.Cloning and Sequencing of Nonstructural Protein (NSP4) Gene of Bovine Rotavirus Isolated in Korea.

The nonstructural protein NSP4, encoded by gene 10 of rotavirus, has been shown to play a role in viral assembly and known to be an enterotoxin, causing diarrhea in mouse pups. NSP4 gene was cloned from CBNU-2 (virulent bovine rotavirus/diarrheic fecal sample) and CBNU-1 (cell-culture adapted bovine rotavirus/isolated from CBNU-2 and 75 times passaged on MA104 cells), respectively, by reverse transcriptase-polymerase chain reaction (RT-PCR) and sequenced and compared. The sequence data indicated that the NSP4 genes of bovine rotavirus (BRV) were 751 bases in length and encoded one open reading frame of 175 amino acids beginning at base 42 and terminating at base 570. Differences in nucleotide sequence between CBNU-2 and CBNU-1 were observed at 6 positions (base 274, 296, 391, 394, 396 and 579). NSP4 gene of BRV exhibited a high degree of nucleotide (90% and 94%) and amino acid sequence (91% and 94%) and amino acid sequence (91% and 97%) homology with those of SA11 and UK but a low degree of nucleotide (77% and 79%) and amino acids sequence (81% and 85%) homology with those of Wa and OSU.

8.A Case of Acute Reticulate Hyperpigmentation on the Face and Neck.

Multiple, reticulated, dark brown colored, macules and patches suddenly developed on the fac, and neck of a 48-year-old Korean woman two days after a traffic accident. On physical examination and laboratory tests including serum melanocyte-stimulating hormone, estrogen and progesterone level, no abnormalities were found except cervical pain. Histological examination of hematoxylin-eosin stained sections revealed increased melanin pigmentation in the basal layer, but the number of melanocytes was not changed in DOPA stained section. Topical application of 2% hydroquinone and 20% azelaic acid ointments had been applied successively for two months each without any apparent improvement. Herein we present a case of reticulate hyperpigmentation on the face and neck, which is very acute and whose causative factors are not certain.

9.Acute Interstitial Pneumonia: HRCT Findings in Five Patients.

PURPOSE: To describe HRCT findings in five patients with pathologically proved acute interstitial pneumonia MATERIALS AND METHODS: This study included 5 patients with pathological and clinical diagnosis of acute interstitial pneumonia. Mean age of the patients was 40(range, 31-53 years). CT scans were reviewed by two chest radiologists retrospectively. CT scans were assessed for the presence and distribution of ground-glass attenuation, air-space consolidation, interlobular septal thickening, honeycombing, and pleural effusion. RESULTS: The area of ground-glass attenuation and air-space consolidation were seen at HRCT in all 5 patients. These lesions were distributed diffusely in both lungs, but involved predominantly subpleural lungs in 2 patients and posterior lungs in 3 patients. Three patients had mild interlobular septal thickening. None of them showed honeycombing. Three of the 5 patients died within 52 days of initial manifestation. CONCLUSION: Acute interstitial pneumonia differs from the more chronic form of idiopathic interstitial pneumonia in their HRCT findings.

10.Induction of DNCB Oral Tolerance in Mice.

Tolerance to contact hypersensitivity was induced by feeding of different DNCB doses in mice. A total of 40 mice were divided into 4 groups(control group, 6 mg feeding group, 10 mg feeding group, 14 mg feeding group) in experiment I, Degree of tolerance to contact hypersensitivity was rneasured by incremert rate of ear swelling after challenge with DNFB. Experiment 2 was performed in the same method of cxperiment: I with addition of 3 mg DNCB feeding group. The increment ratee were significantly decreased in DNCB feeding groups in experirnent 1 and 2(p<0.0l). But there were no differences statisticalIy between increment rates of DNCB feeding groups.
