2.A Comparative Observation on Success Rates of Stone Dislodgers.

A comparative observation was done on 139 patients of the lower ureteral stone, those were applied cystoscopic manipulation with 3 different types of stone dislodger at the Department of Urology, Seoul Adventist Hospital, during the periods from Jan. 1982 to June 1985. During that periods, we applied end-loop catheter in 48 patients, blunt-tip Dormia basket in 38 patients. The results were as follows; 1. The success rate of passage of dislodger beyond the stone was the highest in filiform-tip Dormia basket (97.4%) and loop catheter was the lowest (8l.3%). 2. The success rate of the primary stone extraction with filiform-tip Dormia basket was the highest (86.8%). 3. Indwelling of the dislodger in the ureter loop catheter was the most successful method among them(62.5%). 4. Of the cases of passage beyond the stone, the success rates of the stone extraction were very high and similar in their results, as in 92.3% with loop catheter, in 90.9% with blunt-tip Dormia basket, in 91.9% with filiform-tip Dormia basket. 5. The overall success rate of stone extraction was 75.0% in loop catheter, 75.5% in blunt-tip Dormia basket 89.5% in filiform-tip Dormia basket.

3.Os Submedialis Malleolare (OSM): the clinical characteristics and surgical results.

PURPOSE: The objective of this study was to define the Os Submedialis Malleolare (OSM) as an any ossicles below medial malleolus of tibia and to determine the characteristics of clinical and radiological characteristics and to evaluate results of surgical treatment of OSM. MATERIALS AND METHODS: Twenty-six patients with OSM were identified between November 1, 1998 and June 30, 2002. RESULTS: The mean age was 18.3 years. All patient except one were male and soccer players comprised 71% (20 cases). Associated disease were 8 cases (29%) of chronic ankle instabilities, one case (4%) of anterior impingement syndrome and one (4%) of plantar fascitis. The most common clinical symptom was pain during walking and sports activity and sign was tenderness around medial malleolus. Only simple radiograph could reveal presence of ossicle and differentiate with acute fracture. If symptom and sign obscured , Bone scan (7 cases) and MRI (3 cases) identify causes of pain and tenderness. As a surgical treatment, all bony fragment caused symptom and sign were eliminated and medial collateral ligament was reattached meticulously. Associated chronic ankle instability were present, modified Brostrom procedure was done simultaneously. On follow-up, The symptomatic pain were wholly disappeared at average 2.2 months (1-6 months) after operation. On one year follow-up, all patients have been daily life without any complaints and all soccer players go back to the game within three months. CONCLUSION: Os Submedialis Malleolare have relatively rare incidence and most common clinical symptom and sign is tenderness on medial malleolar area of ankle, can be diagnosed both physical examination and plain X-ray film. As a treatment, Removal of ossicle and meticulous repair or reattachment of deltoid ligament could obtain excellent or good results.

Scrub typhus is a zoonosis transmitted by a chigger of trombiculid mite and manifested with fever, skin rash, myalgia, and hepatitis etc. The renal involvement of the disease is prese-ted with transient microscopic hematuria and/or proteinuria. However acute renal failure or meningoencephalitis is very rare. We report a case of scrub typhus manifested with acute renal failure, meningoencephalitis, and hepatitis. The patient was treated with doxycycline and continuous arteriovenous hemofiltration (CAVH) and recovered successfully.

8.Comparative Study between Glycopyrrolate-Neostigmine and Atropine-Neostigmine in Postanesthetic Arousal .

Atropine and glycopyrrolate have been used for the reversal of non-depolarizing neuromuscular blockade as anticholinergics. Atropine may produce the central anticholinergic effect because it is a tertiary amine which can cross the blood-brain barrier. Glycopyrrolate is a quaternary ammonium compound that does not cross the blood-brain barrier. and as a result, it has no significant central anticholinergic effect. This report compares postanesthetic arousal between glycopyrrolate-neostigmine and atropine-neostigmine mixtures. We conclude that the glycopyrrolate-neostigmine mixture group have significantly more rapid arousal than the atropine-neostigmine group.

9.Tracheal Tube Cuff Inflation in Oropharynx : An Useful Method in Blind Nasotracheal Intubation.

We designed a study to determine if the tracheal tube cuff inflation in the oropharynx improves the success rate of blind nasotracheal intubation in normal, paralyzed patients because of lacking of controlled study about it. In prospective, randomized fashion, 100 ASA I or II patients undergoing elective oral surgery were studied. The trachea was intubated once keeping the tracheal tube cuff deflated throughout the maneuver and once using the technique of tracheal tube cuff inflation in the oropbarynx. A maximum of two attempts was allowed for each technique. If the first attempt was failed, the second attempt was tried with an addition of application of thyroid cartilage compression in each technique. Witb the tracheal tube cuff inflated, the success rate was significantly higher than the cuff-deflated technique(p<0.05). A application of thyroid cartilage compression increased the success rate of the blind nasotracheal intubation in each technique, but it was more useful in the cuff inflation technique(p<0.05). Time taken to intubate the trachea was longer in the cuff inflation technique. We suggest that, in normal paralyzed patients, the tracheal tube cuff inflation in the oropharynx increases the success rate of blind nasotracheal intubation.
