1.Clinical Results and Complications of LASIK using Technolas 217 C-LASIKTM.

We evaluated clinical results and complications of LASIK using Technolas 217 C-LASIKTM excimer laser. This study included 102 patients(203 eyes) who had LASIK from March 1998 to February 1999 and followed up for at least 6months. Preoperative mean spherical equivalent was -3.99+/-1.198(SD), -7.928+/1.138(SD)and -11.92+/-1.060(SD)in each group I, II, III.The results at postoperative 6months were as following: 1. Mean spherical equivalent was -0 .44+/-0.410(SD), -0.34+/-0.500(SD), -0.37+/-0.430(SD)in each group I, II, III. 2. The 70 eyes(65%), 48 eyes(61%), 10 eyes(60%)in each group I, II, III were within+/-0.5D. 3. Mean uncorrected visual acuitywas 1.00+/-0.130(SD), 0.96+/-0.140(SD), 0.86+/-0.150(SD)in each group I, II, III. 4. Loss of best spectacle corrected visual acuity of two or more lines was 6 eyes(3%). Intraoperative complications was thin flap(5 eyes, 2.5%), flap tear(1 eyes, 0.5%)and postoperative complications was night glare and halo(58 eyes, 28.8%), punctate epithelial keratopathy(35 eyes, 12.3%)and interface foreign body(23 eyes, 11.3%). Above results suggests that myopic keratomileusis using Technolas 217 C-LASIKTM is a safe, effective way to correct myopia.

2.A Case of Congeital Occipital Dermoid Cyst.

A 18-year-old girl, who presented with painful scalp mass on occipital region, was found in have a congential dermal sinus. At surgery, upon incision of the occipital muscle fascia, pus poket was encountered and it was connected with thickened occipito-cervical dura through the small foramen on midline occiptial bone. The pathogenesis and principles of management are briefly reviewed.

Chronic fatigue of the respiratory muscles has contributed to the decreased ventilatory capacity and reduced excercise tolerance of individuals with COPD, especially in kyphoscoliosis. Nasal mask BiPAP has been shown to be useful for the patient with nocturnal muscle fatigue and COPD. A 35-year-old man with severe kyphoscoliosis was admitted to ICU due to acute respiratory failure. He had been diagnosed of COPD and had been intubated with mechanical ventilatory support for 7 times. This time he was intubated with ventilatory support, too, in ICU and readmitted to the ICU for severe hypoxemia and hypercarbia from general ward. Thereafter he refused the intubation. Nasal mask BiPAP ventilatory support system was applied and IPAP, EPAP level being adjusted to the 12, 4 cmH2O under monitoring vital signs and arterial blood gas analysis. His condition was improved and discharged home with support of nasal mask BiPAP system after 33 day-stay in the ICU.

6.Two Cases of Anterior Uveitis with Marie-Strumpell Ankylosing Spondylitis.

About 10% ~ 40% of patient with Marie-Strumpell ankylosing spondylitis have anterior uveitis. Recently, relationship between ankylosing spondylitis and anterior uveitis was discoverd by several studies of HLA-B27 antigen. Ankylosing spondylitis was found to be the commonest single etiological factor in analysis of 653 cases of uveitis seen in the Uveitis Clinic in United Kingdom. Ankylosing spondylitis is mostly found in male and most of their natures of uveitis are acute unilateral nongranulomatous anterior uveal involvement. Roentgenological findings of ankylosing spondylitis are characteristic demineralization and subchondral sclerosis of sacro-iliac joint and "Bamboo spine". Elevation of erythrocyte sedimentation rate in ankylosing spondylitis indicates active status of the disease. Authors have experienced 2 cases of anterior uveitis with Marie-Strumpell ankylosing spondylitis in our clinic. These cases have shown typical findings of "Bamboo spine", sacro iliac joint blurring and acute nongranulomatous unilateral uveal involvement.

8.Apnea and Contralateral Ptosis following Interscalene Brachial Plexus Block.

Brachial plexus block may cause many complications such as pneumothorax, inadvertent subarachnoid or epidural blockade, permanent neurologic damage to the motor outflow of the brachial plexus, hoarseness, Homer's syndrome, carotid bruit, convulsions, phrenic nerve palsy, etc. Since Winnie (1970) introduced interscalene approach for brachial plexus block, this has been one of the most popular methods in recent years because of infrequent complications and the technique is still considered safe. We experienced a rare, unusual complication after interscalene approach in a thirty eight year old woman. She was scheduled for left index finger amputation because of crushing injury. A 20 ml of 2% lidocaine and 20 ml of 0.5% bupivacaine was injected through interscalene groove after paresthesia was elicited on the patient's left thumb. Fifteen minutes after injection, she complained of respiratory difficulty and became apneic after five minutes later, and finally she did not respond to stimulation. Endotracheal intubation was proceded for respiratory support. Although neuromuscular blocking drug was not injected, vocal cord was paralysied on laryngoscopy. Pupillary light reflex and eye lash reflex were abscent. Blood pressure and heart rate decreased slightly. Ninety minutes after conservative treatment, self respiration was restored and pupillary light reflex and consciousness were recovered. She was discharged from recovery room to general ward after 4 hours without any events. We suspected that local anesthetics might be injected through epidural space or subarachnoid space in this patient.

9.Giant Internal Carotid Artery Aneurysm:Posterior Cerebral Artery Territory Infarction After Ligation of the Right Internal Carotid Artery: Case Report.

A 62-year-old woman, who presented with dipolipia, was found to have a giant aneurysm of the right internal carotid artery. Gradual ligation of the right internal carotid artery was followed by nausea and vomiting 10 days after the complete ligation. Follow up MRI showed intraluminal thrombus formation of the aneurysm and right temporo-occipital infarction.
