1.Three Cases of Familial Benign Chronic Pemphigus.

Familial Renign Chronic Femphigus(FRCP) is rare, persistent and vesicobullous genodermatoai. Three casee of FRCP with typical clinical and histapathological findings are reported. The first case of FBCP was 27-year-oId man who had recurrent episodes of grou vesicles and rust with miId pruritue at the posterior neck, in both axillae and in inguinal area for 10 years. The second case was 49-year-old woman with skin lesions of recurrent persistent. vesicles, pustules on the erythematous skin, crust and lichenification on the flexural area of extrimeties, axillae, ingumal and neck area for 10 yeara. The third case was 49-year-old male patient who had recurrent pruritic grouped vesicles with crust on the neck and axillary area. In all three patients, skin eruptions usually began and or aggravated in the spring or summer and tend to heal spontaneously in the autumn and winter and showed positive Nikolsky sign. The only one case of the reported three casea had familiaI history of skin lesions similar to FRCP. Histopat,holag;ic finding of biopsy specimens from the vesicular eruption showed auprabasal vesiculation with acantholysis, lacunae and villi formation in all 3 cases.

3.Two Cases of Monochromatic Green Argon Laser Photocoagulation for Choroidal Neovascular Membrane in High Myopes.

High myopia, often called malignant, pathologic or degenerative myopia, involves a progressive elongation of globe, accompanied by degenerative changes of the retina and choroid. Continued stretching and degeneration of the choroid causes eyes with severe myopia to develop breaks in Bruch's membrane which, called lacquer cracks, and permits neovascular proliferation from the choriocapillaris into the subretinal pigment epithelial space. Neovascularization associated with severe myopia deteriorates central visual acuity due to exudation and/or hemorrhage. Laser photocoagulation therapy is useful in treating neovascular membrane outside the foveal avascular zone or extending to the edge of the perifoveal capillary network. We have evaluated three patients with macular hemorrhage which associated with high myopic choroidal degeneration. Subretinal neovascular membrane was detected by fluorecein angiography and was treated with monochromatic Argon green laser photocoagulation in two patients indicated. The visual acuities were improved. But one patient who had macular hemorrhage involving both foveas could not be treated with laser photocoagulation. Then his visual acuity severly decreased. We give our experiences with the review of other papers in this report.

4.Total Hyphema Treated with Ocutome.

Author have experienced 3 cases of intractable total traumatic hyphema on conservative medical treatment and have good results after surgical management by using Model 800(A) Ocutome vitrectomy instrument. For evacuation of blood clot in anterior chamber. The conclusion were as follow; 1. We experienced no significant intraoperative complication. 2. All 3 cases achieved intraocular presure controls in early post operative state and these controls were sustained till now without any antiglaucomatous agent. 3. Good final visual acuity was obtained in pure total hyphema in which there was no other associated intraocular injury at the time of trauma.

7.A Clinical Study of Ankle Fractures

The authors have reviewed 71 cases (70 patients) of the ankle fractures that were treated through the admission in Orthopedic Department, Paik Foundation Hospital, from June 1972 to April 1978. The average duration of the follow-up waa 15 months. The summary of the results were as follows; 1. Of the 71 cases, male was 54 cases and female 17 cases. The average age of the patients was 32. 2. The main cause of the injuries was traffic accident, and the other causes were sliding down and falling from a height, industrial accident, and sports injury in order. 3. By the classification of Lauge-Hansen, the pronation-external rotation and,supination-adduction types were commoner than the other types. 4. Open reduction was performed in 48 cases, and closed reduction in 23 cases. 5. The average duration of the cast immobilization after the closed reduction was 8 weeks, and that of the open reduction was 6 weeks. 6. The result of the treatment was better in those cases of the open reduction than that of, the closed reduction. 7. The good clinical results could be achieved by obtaining the anatomical position of the talus in the mortise and the reduction was achieved more satisfactorily when the key role of the lateral malleolus in complete reduction was well understood.

8.Two Cases of Cysticercosis in the Vitreous Body.

The authors recently experienced two cases of cysticercosis in vitreous body. In the first case of a 27 years old Korean male had a balloon-like 11mm3 cyst in the vitreous body of the left eye. The cyst was extracted and microscopic examination revealed a well preserved cysticercosis cellulosae. In second case, the cyst with the size of 8mm3 in diameter was seen in vitreous body of the right eye of a 35 years old Korean male. Although ocular and cutaneous cysticercosis are common, but cysticercosis in the vitreous body rarely been reported in Korea.

9.Ultrastructural Changes in the Exocrine and Endocrine Cells of Rat Pancreas in Endotoxin Shock.

This study was carried out to investigate the morphological changes in the exocrine and endocrine glands of rat pancreas treated with endotoxin. Thirty-five male Sprague-Dawley rats, maintained on a stock diet, weight 200.0 gm. average. were divided into two experimental groups. Group 1. Control group. Five rats. Intraperitoneal infections of 0.3 ml normal saline only. Group 2. Endotoxin-treated group. Thirty rats. 7.6 mg of endotoxin per kg. of body weight was administered intraperitoneally. Each of 5 experimental animal was sacrificed 30 minutes, 1, 2, 4, 6 and 8 hours after endotoxin treatment, followed by examinations of histochemical, light and electron microscopy of both transmission and scanning modes. The results were as follows: A. Light microscopic findings: A mild interstitial edema and hyperemia were noted 1-hour after endotoxin treatment. Cytoplasmic vacuolization at 2-hour level(2-hours after endotoxin administration), diminished staining quality of both endocrine and exocrine cells at 6-hour level. B. Electron microscopic examination: a. Transmission electron microscopy. The acinar cells of pancreas showed a mildly increased pre-lysosome at 30-minute level. At 1-hour level, appearance of secondary lysosome was noted in addition to the findings of mitochondrial swelling and decreased cristae; disarray and vacuolization of the RER; vacuolar change of Golgi apparatus. At 6-hour level, post-lysosomes. The changes in the endocrine glands were similar to the findings of exocrine glands just described with time lag of 1 to 2 hours. The endothelial cells of capillaries show swelling and pinocytotic vesicle formation, protrusion of the cytoplasmic processes into the capillary lumen and increased heterochromatin at 1-hour level. These findings became more prominent as time lapses. The lumen of the endothelium tends to be narrowed, filled with fibrin and other blood cell components which later terminated with occasional complete occlusion by the formation of thrombi. b. Histochemical study: Primary lysosomes of the control group revealed a strong reaction of the acid phosphatase whereas the endotoxin treated group with less reactivity limited in the peripheral zones of the lysosomes. Secondary lysosomes with partial reactions. However, the pre-lysosomes and post-lysosomes failed to demonstrate any acid phosphatase activity at all. c. Scanning electron microscopy. The endothelial cells of the capillaries, arterioles and venules demonstrated increased microvillous activity, broad bled formation, cytoplasmic protrusion into the luminal spaces and microthrombi formation at 1-hour level. Six-hour level onward there noted a junctional disruption and partial detachment from the subendothelium of the wall. It can be concluded, therefore: When the endotoxin enters the blood stream, it elicits endothelial injury followed by both exudation with resultant edema of the surrounding tissue and concomitant vascular occlusions due to thrombosis. This vascular occlusion, in turn, causes ischemic degenerative change of the cells of exocrine and endocrine glands of the pancreas which are followed by digestions of degradational materials from the injured cells through the lysosomal phagocytic system. Besides the above pathogenetic pathway, one can not rule out the possibility of the direct effects of the endotoxin to the cells of exocrine and endocrine cells of the pancreas also so rendered.

10.Clinkal Observation on 28 Cases of Simplified Dacryocystorhinostomies.

From January, 1973. to June, 1976 the Auther Performed simplified dacryocystorhinostomies on 28 cases with tearing and mucopurulent material discharge from the lacrimal sac. The auther's surgical technic was a modified Iliff technic and the addition of the polyethylene tube fistulization procedure for complicated case of lacrimal sac are introduced. The results of observations are sumerized: 1) Among 28 cases, 15 of the subjects r were male and 13 of the subjects were female. The ages of the patients ranged as follows: The youngest was 7 years of age and the oldest was 59 years of age. 2) The success rate of this method was 89% and Author's polyethylene tube fistulization procedure in selected case gave a higher success rate. 3) When the occulusion of the canaliculi, lower and upepr punta was noted, the polyethylene tube fistulization procedure was added. 4) Some common complicacion, mild wound bleeding and subconjuntival hemmorrhage etc, were observed. This complication could be easily controlled by adequate treatment. 5) The explanation for the obstruction was apparent in fewer than half of the patients. We can assume that the 2 children had a congenital membrane at the lower end of the tract. All of these tracts had had repeated probings. 3 patients had injuries with breaks in the bony lacrimal canal. In 2 patients the obstruction had been caused by chronic sinusitis. 6) Among the 25 successful cases, the symtoffis were relieved immediately in 24 cases, but tearing and mucopurulent material persisted in 1 case. So a probe was passed into the nose to reestablish patency of the tract. In Conclusion, the operation is simple to do and within the range of any ophthalmic surgeon. A simplified dacryocystorhinostomy is the best treatment of tearing and mucopurulent material drainage from the nasolacrimal duct.
