1.A Case of Epithelioma Cuniculatum.

Epithelioma cuniculatum or plantar verrucous carcinoma shows a great tendency toward deep penetrating growth, resulting in numerous crypts filled with horny material and pus. We herein described a 44 year-old female who showed a warty, relatively round, brown to yellow colored protruding mass with many crypts filled with pus-like exudates, measuring 2.3 X 2.5 X 1.5 cm in size, and of 5 years duration. The biopsy specimen taken from the tumor mass revealed a deeply penetrating, well-differentiated squarnous cell carcinoma without nuclear anaplasia and individual cell keratinization.

2.Clinical Observation of Incontinentia Pigmenti.

The clinical observation was done on 9 patients of incontinentia pigmenti(I,P.) who were seen at the Department of Dermatology, Chonnam University Hospital from January, 1970 through December, 1982. The incidence of I.P. patients among the population of Chonnam province was higher than 1/142,000 whereas the frequency of I.P. among the outpatients was 0.016% The sex of I.P. patients was all female and there was no cases showing faiailial occurrence. The onset of the cases were within 4 months after birth, 44%, being at birth and 89% being within 2 weeks after birth. The durations of first, second and third stages of I.P. patients were respectively 14 days, 15 days to 2 months, and 2 months to 10 months. The trunk and extremities were involved in 4 cases, and the extremities only were involved in 4 cases while the trunk only was involved in J case. The blood eosinophilia was observed in 2 cases among 7 cases, with their results being 18%, and 11% respectively. The biopsy examinations done in 7 cases at the time of their first visits revealed that 3 cases, 2 cases and 1 case respectively conformed to first, second and third stages with 1 case having first and second stages simu 1taneously. There was no I.P. patient showing the associated congenital abnorma- lity.

3.Prosthetic Replacement of the Hip

Prosthetic replacement was performed in 31 patients (32 cases) with femoral neck fractures at National Medical Center from 1968 to 1981. We reviewed the records of 28 patients having Austin-Moore prosthetic arthroplasty in 29 cases, and of 3 patients having Thompson prosthetic arthroplasty; then obtained following results. l. Of the 31 patients, 23 were female and 8 were male. The left hip was involved in 20 patients, right in 10, and the both sides in one. 2. The age range was between 35 and 84 years, with the average age of 61.2 years. 74.2% of patients were older than 51 years of age. 3. The most common cause of fracture was slipping or fall (65.6%), and the fracture sites were subcapital in 11 (34.4%), transcervical in 12 (37.4%), and basilar in 3 (9.4%). 4. Three cases (9.4%) were operated within a week, and 21 cases (65.6%) more than 3 weeks after injury. Good results were obtained in fresh fracture which was operated within a week. 5. In comparing the blood loss in Moores and modified Gibson's surgical approaches, the average total loss by the former was 1,290 ml and 1,720 ml by the latter. 6. In complications, one case of fracture of the posterior rim of acetabulum with subsequent subluxation and two cases of fracture of the greater trochanter were notable as operative complicatiens. 7. In a follow-up study averaging 13.1 months, good to excellent results were obtained in 8 cases, fair in 6, poor or bad in 5, respectively.

5.Leiomyosarcoma of the Skin: Report of A Case.

Leiomyosarcoma arising in the skin is rare tumor, and diagnosis usually is made microscopically. After local excision, these lesions recur in large proportion of pat ients. The authors herein report a 53-year-old male with leiomyosarcoma appeared in the skin of the right forearm and presenting as a dark reddish colored, 5*6cm in diameter, superficial ulcerated single firm nodule with intermittent pain. Histopathological examination showed poorly circumscribed tumor consisting of interlacing bundles of spindle shaped smooth muscle cells in the middle and lower parts of the dermis. The nuclei were hyperchromatic, large, vacuolated, and irregular in shape. Electron microscopic findings revealed cytoplasmic organelles such as rough endoplasmic reticulum and mitochondria of malignant smooth muscle cells in the paranuclear area, Characteristic subsarcoelmmal caveolae and dense plaque were noted and myofilaments were distributed in the peripheral cytoplasm. The tumor did not recur in 10 months' follow-up.

6.Clinical Evaluation of the Strabismus Patient.

The authors studied 410 cases of strabismus patients who visited our clinic from 1973 to 1978 Among them, 170 cases of esotropia, 234 cases of exotropia and 6 cases of hypertropia were observed. The average age at the time of onset was 3.4 years in esotropia and 5.2 years in exotropia. Visul acuity in exotropia was relatively good. 0.7 or more visual acuity was 53 % in exotropia and 34.7 % in exotropia. Refractive status, hypertropia above 1.5D was 64. 2 % in esotropia, but, no specific correlation was found between exotropia and refractive status. The incidence of amblyopia was 33.1 % in esotropia and 19.7 % in exotropia. In accomodative esotropia, the patients with high AC/A ratio were treated with bifocal segment glasses. and the results were good.

9.Clinical Results of Unilateral Medial Rectus Recession in Consecutive Esotropia and Non-accommodative Esotropia.

PURPOSE: To compare the effect of medial rectus recession in consecutive esotropes who had previous monocular medial rectus resection and lateral rectus recession and in non- accommodative esotropes (NAET) with small angle of deviation who had no prior operation. METHODS: We studied the results of unilateral medial rectus recession at postoperative 2 and 6 months in 7 consecutive esotropes and 19 NAET with deviation angle around 20 PD, and analyzed the corrected amount for 1 mm medial rectus recession. We regarded deviated angle of+/-8 PD at orthophoria as successful operation. RESULTS: The abosolute value of remained deviation in consecutive esotropes and NAET were 4.86+/-5.67 PD and 9.05+/-3.95 PD at postoperative 2 months, and 5.42+/-7.89 PD and 8.26+/-4.45 PD at postoperative 6 months, respectively. There were significant differences between at postoperative 2 and 6 months (p<0.05). The success rates in consecutive esotropes and NAET at postoperative 2 months were 86% and 42%, and 71% and 47%, respectively at postoperative 6 months, which were no significant differences (p>0.05). The average amount of corrected deviation for 1 mm recession at postoperative 6 months was 5.84 PD in consecutive esotropes and 2.97 PD in NAET, which were significant differences (p<0.05). CONCLUSION: Unilateral medial rectus recession in consecutive esotropes may be a useful and first choice of operation procedure when considering second operation. The amount of correction for 1 mm medial rectus recession is larger in consecutive esotropes than in NAET, possibly due to previous resection of medial rectus or release of fat adhesion. Therefore the amount of medial rectus recession in consecutive esotropia should be determined after considering the degree of abduction and operative findings.

10.Epikeratophakia for Adult Aphakia.

Epikeratophakia is a newly developed refractive corneal surgery for the correction of aphakic vision. Two aphakic patients have received hyperopic epikeratophakia grafts. Three months postoperatively, two patients had visual acuities of 0.6 without spectacle overcorrection. Epikeratophakia appears to be effective for the correction of adult aphakia.
