The authors experienced a case of secondary glaucoma combined with mesodermal dysgenesis of anterior segment, which disclosed extensive peripheral iridocorneal adhesion of congenital origin while his fellow eye revealed a sclerocornea. This case is extremely rare and entirely different from the previously reported Axenfeld's and Rieger's anomalies. We report this interesting case with brief review of the literatures.

We report two cases of linear focal elastosis. They are a 16-year-old girl and a 14-year-old boy who had had asymptomatic, several, yellow-red, slightly elevated, palpable, striae-like lesions on their middle and lower back since their early childhood. Light microscopic examinations of skin biopsy specimens demonstrated dermal thickening and focal increases of thin wavy fibers in the upper and mid-dermis but no changes in the epidermis. Verhoeff-van Gieson stains for elastic fiber revealed aggregated, clumped, curled, or fragmented elastic fibers.

Trichilemmal carcinoma is a rare tumor that is located predominantly on sun-exposed skin of the elderly. It is thought to be related to the external root sheath of the hair follicle and the malignant counterpart of a trichilemmoma. We report a case of trichilemmal carcinoma in a 88-year-old woman who had a 10 year history of a single, dark brown tumor on the right preauricular region. In her past medical history, it had been excised twice at a private clinic without a histopathological evaluation and recurred 2 years later. Histopathological findings showed atypical clear cells resembling those of the outer root sheath. Tumor cells showed PAS-positive cytoplasm. Immunohistochemistry revealed positive high molecular weight cytokeratin expression. To our knowledge, this is the second case of trichilemmal carcinoma in the Korean literature. Intravascular injection of hydrogen peroxide produces oxygen microbubbles suitable for echocardiographic contrast enhancement. To evaluate the effect of a method of myocardial contrast 2-D-echocardiographic delineation of myocardium during acute coronary occlusion, injection of a fresh mixture of 2ml of 0.2% H2O2 and 1ml of heparinized dog blood into aortic root were made in 12 poenchest dogs 10 minutes after occlusion of left anterior descending coronary artery distal to the first diagonal branch and left ventricular short axis 2-D echocardiographic images at the midpapillary muscle level were obtained. On injection of H2O2 blood mixture normally perfused myocardium was enhanced in echodensity but the area of malperfusion did not change in echodensity. The borderlines between the area of normal perfusion and malperfusion was well delineated. The malperfused area measured at mid papillary muscle level by planimetry area method was 29.7+/-6.0% and 32.6+/-6.7% by endocardial circumferential length method. There was a linear correlation between planimetric estimate of area of malperfusion by H2O2 contrast echocardiography and visual determination of regional wall motion abnormality by 2-D echocardiography(r=0.93, P<0.001). There was no change in heart rate before, during and after H2O2 injection. Infection of H2O2 blood mixture caused bradycardia(8.3%), second degree A-V block(16.6%) and ventricular fibrillation(8.3%). H2O2 clearance was achieved in 3-10 minutes. These findigs suggest that H2O2 enhanced myocardial contrast ehocargiography using 2ml of 0.2% H2O2 and 1ml of blood muxture is an accurate, reproducible, real-time in vivo method of quantifying the extent of myocardial perfusion defect during acute coronary occlusion in dog.

A 5-year-old boy was admitted to the hospital with a high fever and abnormal gait. Magnetic resonance imaging showed extensive swelling of the quadriceps and adductor muscles around the right hip and anterior thigh. A duplex scan demonstrated a thrombus from the external iliac vein to below the popliteal vein. The blood culture revealed Streptococcus pyogenes. The patient was immediately treated with antibiotics and anticoagulants. Follow-up sonography demonstrated complete disappearance of the venous thrombosis. We report a case of streptococcal myositis complicated by deep vein thrombosis.

We evaluate the effects of a calcium antagonist(nifedipine) and a xanthine oxidase inhibitor (allopurinol), drugs having a protective effect against shock wave induced renal dysfunction, on acute changes of renal function after piezoelectric ESWL. A total of 40 patients with renal stones undergoing piezoelectric ESWL with LT02 lithotriptor was randomly assigned to 4 groups. Group 1 received no medication and the others received nifedipine(group 2), allopurinol(group 3), and nifedipine plus allopurinol(group 4), respectively. NAG, LDH, 7-GTP, D2M, and microalbumin were measured in the 24-hour urine before and after ESWL. Baseline levels of these parameters were not statistically different between the control group and the others. After ESWL, NAG and microalbumin were significantly increased in group l(p<0.01). In groups 2 and 4, all of the parameters after ESWL were not significantly different from the Values before ESWL. Although the level of NAG after ESWL was significantly higher(p<0.01) than that of the pre-ESWL in group 3, the change of NAG was milder in group 3 comparing to group 1. The range of increase of NAG in groups 2 and 4 were significantly low(<0.01) compared to group 1, and the range of increase of microalbumin in groups 2, 3, 4 were significantly low compared to group l(group 2, 4; p<0.01, group 3; p<0.05). Our results indicate that nifedipine and/or allopurinol can prevent or decrease acute changes of renal function after ESWL using LT02 piezoelectric lithotriptor and especially nifedipine seems to be more efficient than allopurinol.
