1.Clinical Experiences of Therapeutic Leukapheresis.

Therapeutic leukapheresis is a treatment modality which selectively remove abnormal leukocytes and relieve symptoms produced by abnormally high concentration of leukocytes in the blood stream. Authors report the experiences of therapeutic leukapheresis at Seoul National University Hospital during the past 7 years(1988 to 1995). A total of 48 procedures were done for 29 patients(24 males and 5 females). The age distribution was from 16 years to 60 years and mean age was 30 years. The most common diagnosis of patients were acute lymphoblastic leukemia(7/15, 46.7%) and major indications for therapeutic leukapheresis were dyspnea, headache, abdominal pain and mental alteration. The mean leukocyte count before and after leukapheresis were 277,300/micro liter and 220,700/micro liter, respectively(20% decrease, P<0.001). Hemoglobin concentration was also decreased from 7.7g/dL to 7.1g/dL after leukapheresis(P<0.01). The mean number of leukocytes removed per procedure was 3.1x1011. Adverse reactions such as abdominal pain and dizziness were observed in two cases. The leukocyte count was decreased in all patients and improvement of symptoms was observed in four patients. In conclusion, therapeutic leukapheresis is relatively safe and can be used to relieve leukostatic symptoms in leukemic patients.

2.Effect of interleukin-12 on airway inflammation in mouse model of bronchial asthma.

BACKGROUND: Th2-like cells are thought to play a crucial role in the recruitment and activation of eosinophil in bronchial asthma. In contrast to Th2 cytokine, Thl cytokine IFN-y decreases eosinophil recruitment. Previous studies have shown that IL-12 promotes differentiation of Th0 into Thl and enhances production of Thl cytokine. IL-12 also prevents differentiation of Th0 into Th2 during primary immune response. Its effect on established Th2 cell, however, is well known. OBJECTIVE: The objective of aur study was focused on whether IL-12 prevents recruitment of eosinophil and expression of Th2 cytokine in murine model for bronchial asthma, and whether its effect differs according to timing of dosage. METHOD: Administration of IL-12 was tested in the 3 different time-frames; 1) allergic sensitization (early dosage) 2) allergic challenge (late doaage) or 3) both. The number of eosinophil in the bronchoalveolar lavage(BAL) fluid and tissue was examined for change of airway inflammation. The effect on cytokine expression was assessed by measuring cytokine in bronchoalveolar lavage fluid (ELISA) and mRNA in peribronchial lymph node (RT-PCR) RESULTS: Early dosage of IL-12, and the combination of early and late dosages, strikingly decreased the numbers of eosinophil in both BAL fluid and tissue(p<0.05). Late dosage of IL-12 decreased tissue eosinophilia, while the number of eosinophil in BAL fluid remained unchanged. IL-12 increased IL-4 and IL-5 levels, and decreased IL-2 and I~FN-r levels. There were no differences in Thl/Th2 cytokine regulation among the three dosage times. Early dosage of IL-12, and the combination of early and late dosages, increased IL-10 level, but late dosage had no effect on IL-10. CONCLUSION: These results demonstrate that depending upon whether IL-12 is administered during sensitization or during subsequent allergen exposure, Thl/Th2 cytokine regulation by IL -12 shows no difference because it seems that difference of inhibition of eosinophil recruitment by IL-12 might be related with the other factors, such as IL-10.

4.A Clinical Analysis of Fracture of the Tibial Condyle

The author reviewed 15 cases of fracture of tibial condyle treated at the orthopaedic department of Kyungpook National University hospital during the period from January 1963 to June 1970, and analysed as to cause of injury, type of fracture, average duration for treatment, complications and end results. The clinical results were obtained as follows: 1) The cause of injury were mostly traffic accident(46.7%), falling and stumbling(26.6%), coal miner accident(13.3%), direct blow(13.3%). 2) In classification of fracture type, Type I is most common type in this series and fracture of the lateral condyle is more common than medical condyle. 3) Associated ligaments tear is more common is Type III than other type, and medial collateral ligamant is most common. 4) In Type I with cast immobilization for 8 to 10 weeks, and then encouraged knee joint exercise. In Type II one of two cases treated with cast immobilization and with long leg brace after remove of the cast. Other case was treated with open reduction. In Type III 3 of 5 cases treated with bolting and immobilized for 16 weeks. 5) Undesirable sequelas are instability of joint, persistent pain and limitation of joint motion.

5.Gorham's Syndrom: A Case Report

Gorham's syndrome (Gorham's disease, Massive osteolysis, Disappearing bone disease, phantom bone) is a rare disorder characterized by histologically benign proliferation of thin-walled vascular channels originating in bone associated with extensive lysis of regional bone matrix. It starts in bone, but it may secondarily involve soft tissues and adjacent bones. Although the osteolysis usually arrests spontaneously, its biologic behavior is unpredictable. The etiolgy is still in dispute and the treatment presents many problems. The author's case was a 20-year old girl, who had pathologic fracture in neck and supracondylar area of left femur with osteolysis of left pelvis, femur, and proximal fibula and tibia. She also had lymphangioma involving bones and adjacent soft tissues. To our knowledge, there is no report on Gorham's syndrome in Korea. The authors report a case of Gorham's syndrome with review of literatures.

6.Differential Regulation of Antioxidant Enzymes during Monocyte Differentiation.

Antioxidant enzymes levels were determined in monocytes during phorbol myristate acetate (PMA)-induced differentiation. PMA induced the differentiation of a human monocytic leukemia cell line THP-1 into macrophage-like cells as indicated by activity of acid phosphatase and morphological changes. The level of Mn-superoxide dismutase (SOD) was selectively increased in PMA-treated THP-1 cells after one day of culture, while the levels of Cu/Zn-SOD and catalase were progressively decreased by Western blot analysis. In contrast, levels of Cu/Zn-SOD and catalase protein and enzyme activitiy remained unchanged in THP-1 cells after transforming growth factor-p, treatment. Cu/Zn-SOD is oxidatively inactivated by exposure to H,O, which is produced by PMA-treated THP-1 cells, and then the inactivated enzyme undergoes proteolysis and fragmentation as analyzed by radiolabeled method. Thus monocytes have a coordinated system for synthesis and degradation of antioxidant enzymes during PMA-induced differentiation.

7.Evaluation of Bedside-use Leukocyte Removal Filter.

Removal of leukocytes from blood components may prevent alloimmunization to leukocyte antigens, febrile nonhemolytic transfusion reactions, transmission of leukotropic viruses or bacteria, and graft-versus-host disease. New surface-modified fibrous polyester filters were evaluated; they require no special processing of blood components and can be used at the patient's bed-side. Pall RC100TM(Pall Co., USA) was used to filter 2 units of RBCs at a time for a total of 8 filtrations: mean red cell recovery rate was 88.7%, mean WBC removal rate 99. 99%, and mean residual WBC count after filtration 2.77x 10 5. Twelve units of apheresis platelets were filtered with Pall PL50TM(Pall Co., USA): mean platelet recovery rate was 90.7%, mean WBC removal rate 99.95%, and mean residual WBC count after filtration 6.99 x 10 5. This study indicates that these filters efficiently remove WBCs without significant loss of red cells or platelets and that this kind of filters can be easily used at the patient's bed side.

8.A STATISTICAL STUDY ON FACIAL BONE FRACTURES OF KOREANS.

For the establishment of the basis of treatment and study in the patients of facial bone fracture, we performed a clinico-statistical study about 28 papers and 9564 cases reported as facial bone fractures in the journal of Korean association of oral and maxillofacial surgeons, the journal of the Korean academy of maxillofacial plastic and reconstructive surgery, and related journals. The results were as follows: 1. The ratio of men to women was 4.50:1. 2. The age frequency was highest in the third decade(37.6%), and fourth(21.5%), second(15.5%), fifth(10.3%) decade in orders. 3. The most common location of facial bone fractures was the mandible(62.7%), and zygoma complex (22.6%), nasal bone(15.0%), and maxilla(13.0%) were next in order of frequency. 4. The major etiologic factors were traffic accident(37.9% ), fisticuffs(26.4%), and falldown and slip down(23.4%). 5. The frequent fracture site of mandible is symphysis(39.3%), angle(24,4% ), and condyle(22.5%). The ratio of left to right was 1.31:1. Open reduction(69.1%) was the more frequently using method of treatment in mandibular fracture than colsed reduction (28.6%). 6. The sites of zygoinatic fractures were zygoma complex(48.0%), zygornatic arch(35.7%), and combined(16.3%). The left to right ratio was 1.37:1. 7. The most frequent maxillary fracture was Le Fort I (31.4% ), and Le Fort II (27.1%), unilateral(14.3%), Le Fort III (7.6%) were next in order of frequency.

9.A Case of Livedo Reticularis Associated with Decompression Sickness.

Livedo reticularis is a mottled bluish discoloration of the skin which occurs in a netlike pattern and is not a diagnosis in itself, but is a nonspecific reaction pattern. It may be classified as idiopathic and secondary livedo reticularis. Decompression sickness can occur during decompression after diving into deep sea water or during a rapiid ascent from sea level, and is one of the many causes of the secondary livedo reticularis. We report a case of livedo reticulris which developed in a patient with decompression sickness. A biopsy from the purpuric patch revealed an unusual histopathologic finding that resembles those of bullae and sweat gland necrosis in drug induced coma.
