2.A Clinical Study of Meniscal Lesions

We meniscectomized 27 knees with meniscal lesions for disabling symptoms after the diagnosis by clinical evaluation and arthrography from January, 1970 to May,1979. Statistical analyses were as follows: 1. Male was affected 2 times more than female. The most meniscal lesions occurred in the age group between 10 and 29 years old (67%). 2. The lateral menisci were affected 2 times than the medial. In the former, the middle segments were affected most frequently and in the latter, the posterior horns. 3. The general diagnosis of torn meniscus for the 27 knees was correct by clinical evaluation for 19(71%), by arthrography for 20 (74%) and by two methods for 24 (88%). 4. In the follow-up study, satisfactory results were obtained in. 79%.

3.Immunofluorescent Studies of Various Chronic Bullous Dermatoses.

The accuracy and sensitivity of both direct and indirect immunofluorescence microseopy in diagnosing chronic bullous dermatoses were evaluated and compared in 11 cases of six different disease entities(pemphigus vulgaris, pemphigus vegetans, bullous pemphigoid, linear IgA bullous dermatosis, familial benign chronic pemphigus), which had been diagnosed clinically and by routine histopa thological studies. And the results obtained were as follows: 1) In 4 cases of bullous pemphigod, the direct IF of perilesional skin showed linear deposition of IgG and C2 along basement membrane zone(BMZ), whereas the indirect IF revealed negetive findings. 2) In 2 cases of pemphigus vulgaris, the direct IF showed deposition of IgG and C2 in intercellular substance(ICS) of perilesional epidermal tissue, and the indirect IF revealed auto-antibody to ICS(1:320) in one case. 3) In 2 cases of linear IgA bullous dermatosis, which were diagnosed as dermatitis herpetiformis by clinical and routine hisopathological findings, the direct IF of perilesional and uninvolved skin manifested linear deposition of IgA, IgM, C3 and F along BMZ, However, no immunofluorescence was detected by the indirect IF. 4) In a case of chronic bullous dermatosis of childhood, the direct IF of perilesional skin showed linear deplosition of IgA and IgM along BMZ. 5) In a case of pemphigus vegetans, the direct IF revealed no specific findings, while the inderect IF disclosed auto antibody positive to ICS(1:40). 6) In a case of familial benign chronic permphigus, no immunofluorescence was found by direct IF. These results indicate that both direct and indirect immunofluorescence micros-copy are valuable in diagnosing chronic bullous dermatoses and in understanding their immune pathogenesis.

4.A Clinical Study of Zomepirac

A clinical study was performed to evaluate the effectiveness of Zomepirac in 27 patients who had undergone bone surgery, 13 patients who had soft tissue surgery and 7 non-surgical patients admitted to the department of orthopedic surgery of Hanyang University Hospital from Oct., 1981 to Nov., 1981. The conclusions were as follows: 1. To decrease pain by half, Zomepirac was determined to be much more effective in patients with severe pain than in patients with moderate pain. 2. The initial pain relief was also more marked in patients with severe pain than in patients with moderate pain. 3. The duration of analgesia with Zomepirac was longer in patients with severe pain than in patients with moderate pain. 4. The duration of pain relief after medication with Zomepirac was from 1 to 3 hours in 79.3% of patients. 5. The results of treatment were excellent in 5 patients (10.6%), very good in 5 patients (10.6%), good in 7 patients (14.9%), fair in 4 patients (8.5%), poor in 1 patient (2.2%), when the intensity of pain before treatment was severe; but fair in 13 patients (27.7%), poor in 11 patients (23.3%), when the pain was moderate. Thus the clinical results demonstrated that this medication was remarkably effective in patients with severe pain and much less so in patients with only inoderate pain. 6. Side effects were mild transient drowsiness in 3 patients (6.3%) and gastric colic in 2 patients (4.3%). In patients with drowsiness was continuously administered. The gastric colic subsided when the medication was combined with antacid.

5.A Case of Pemphigus Vegetans.

We experienced a 54-year-old female patient who had multiple vegetaing plaques and, peripherally, a few pustules in her both axillae. On the skin biopsy specimen of the vegetating plaque, it showed acanthosis, suprabasal acsntholysis, papillo-matosis, downward proliferation of the epidermis, and eosinophilic microroabscess in the epidermis. The indirect immunofluorescence study disclosed anto-antibody positive to intercellular substance of the lip of guinea pig, while direct. immuno-fluorescence of the perilesional skin reealed no specific findings. The skin lesion had improved with topical and systemic steroid therapy.

8.Adrenergic Effects on the Cardiac Damage by Oxygen Free Radicals.

BACKGROUND: It is now well known that oxygen free radicals act as mediators of cellular injury in many cardiovascular diseases. Although some evidences have been presented for an altered number of adrenergic receptors in heart disease, a detailed examination of the interaction of free radicals with cardiac adrenergic receptors has little been carried out. METHODS: This study is therefore undertaken to examine the influences of adrenergic blocking agents(propranolol, prazosin) and cyclooxygenase inhibitor(aspirin) on the cardiac damage by oxygen free radicals. Ischemic condition of the isolated rat heart was made by Langendorff preparation, and then measured superoxide dismutase(SOD), lactate dehydrogenase(LDH) released in the perfusate, and malondialdhyde(MDA) concentration in the cardiac tissue. RESULTS: We obtained the results that increased SOD activity and tendency of decreased LDH activity by the pretreatment of propranolol, prazosin, and aspirin in cardiac ischemic-reperfusion injury. Those effects were more predominant in adriamycin treated group comparing with not treated group. CONCLUSION: Adrenergic blocking agent, propranolol, prazosin, and aspirin seem to have some protective effect on the reperfusion injury of the heart.

10.Unusual Three Cases of Adult Coronary Arteriovenous Fistula.

We report unusual 3 cases of adult coronary arteriovenous fistula(CAVF) diagnosed by coronary arteriography in Soonchunhyang University Hospital. In one patient with mitral stenoinsufficiency, multiple CAVFs originated from left anterior descending and left circumflex coronary areries. In another patient with significant stenosis(90%) in distal right coronary artery, new CAVF was seen in mid right coronary artery during consecutive coronary arteriography. In the remaining one, it was arose from contus branch of right coronary artery. Surgical correction was performed in two cases.
