1.Flexible Bronchoscopic Observation on Endobronchial Tuberculosis.

Endobronchial tuberculosis is frequently associated with lymphadenopathy, bronchial stenosis, atelectasis which needs to be differentiated with other pulmonary disease. Authors are presenting 25 cases of endobronchial tuberculosis which were confirmed boy bronchoscopic biopsy and AFB smear and/or culture. The study was done on patients who visited Yeungnam University Hospital from May 1983 to July 1985. The following results were obtained: 1. Of the 25 cases, 8 cases were male and 17 cased were female (male:female=1:2). 32 percent of the cases was older than 60 years of age. 2. Abnormal findings on chest x-ray were seen on 22 cases of 25 cases (88%) and its most predilective site was right upper lobe (24%). 3. Symptoms were coughing (56%), hemoptysis, hoarseness, chest pain, dyspnea and fever in orders. 4. The positive results were obtained in 73 percent of sputum AFB smear (11 cases of 15 cases). 60 percent of AFB culture (3 cases of 5 cases) and 58 percent of bronchoscopic biopsy (14 cases of 24 cases). 5. Complete pulmonary function test was done on 19 cases and showed normal result in 6 cases, restrictive pattern in 8 cases and small airway dysfunction in 5 cases. No case of obstructive airway disease was detected. 6. Associated disease were hypertension (2 cases), tuberculous meningitis (1 case), diabetes mellitus (1 case), and cataract (1 case). 7. The site of bronchoscopic lesion on bronchoscopic examination were as follow; 8. All 25 patients were treated with anti-tuberculosis therapy and 6 patients were treated with additional steroid therapy because of severe respiratory symptom and showed dramatic improvement of symptom as well as lung function.

3.The Incidence of Urinary Incontinence and influences on quality of life among elderly Women.

The purpose of this study was to identify the incidence of urinary incontinence of elderly women in Korea, to identify the life style for urinary incontinence. The sample consisted of 237 women of Seoul and 5 Provinces. Data for this study was collected from January 16 to June 23 by structured questionnaire. Urinary Symptom Questionnaire developed by Jackson and demographic questionnaire were used to collect the data. The data was analyzed by frequency, percentage, Chi-Square test, Chronbach alpha coefficient with SPSS/PC+ program. 1. It was reported that 64.5% of the subjects have experienced urinary incontinence. And stress, urge, and mixed type incontinence was 34.0%, 8.5%, 57.5% respectively. 2. Characteristics related to urinary incontinence are : Daily frequency 28.7%, nocturia 71.5%, urgency 74.0%, bladder pain 36.7%, unexplained incontinence 35.1%, noctural incontinence 23.5%, frequency of incontinence 23.3%. Wearing protection 42.3%, changing outer clothing 31.0%, hesitancy 17.1%, straining 24.7%, intermittent stream 27.2%, abnormal strength of stream 28.4%, retention 13.6%, burning 5.0%, incomplete emptying 13.6%, Inability to stop mid stream 32.5%. 3. The life style and incidence with urinary incontinence were as following. Fluid intake restriction 18.6%, interference in daily task 30.3%, avoidance of places and situation 28.0%, interference in physical activity 22.3%, interference in relationship with other people 16.6%, interference in relationship with husband/companion 4.5%, time after attack of urinary symptoms 81.5%. In terms of the feeling about the rest of life with urinary symptom, perfectly happy 5.0%, pleased 10.6%, mostly satisfied 18.7%, mixed feelings 28.6%, mostly dissatisfied 20.1%, very unhappy 16.8%, desperate 0.3%. 4. Significant relation between incidence of urinary incontinence and life style. Significant differences between Fluid intake restriction(X2=8.876, P=0.002), affected daily task(X2=32.113, P0.000), avoidance of places and situation(X2=30.155, P=0.000), interference in physical activity(X2=30.209, P=0.000), interference in relationship with other people(X2=26.091, P=0.000). In terms of the feeling about the rest of life with urinary symptom(X2=43.425, P=0.000) of life style and incidence of urinary incontinence. In conclusion, this study were preliminary study to provide nursing practices guidelines for elderly urinary incontinence. Nurses working with elderly should develop and provide adequate care for the incontinent elderly subjects.

4.Studies on the Determining Factors at Each Stage of Germinal Center B Cell Differentiation.

To delineate the individual role of follicular dendritic cells (FDC) and T cells at each stage of GC B cell differentiation at the clonal level and to analyze the signals required for the differentiation, we developed an experimental model using an FDC line, HK and a lymphoma cell line, L3055 for centroblasts. Phenotypic analysis of L 3055 revealed its origin of GC and the homogeneity. L3055 cells undergo spontaneous apoptosis when cultured in the absence of HK cells. L3055 cells proliferate continuously in the presence of HK cells, while they differentiate into a population with the phenotype of centrocytes after stimulation with CD40 ligand (CD 40L) plus IL-2, IL-4 and IL-10. L3055 undergo anti-Ig-mediated apoptosis, which is not protected by HK but by CD40L and the cytokines. These experimental results suggest that FDC provide signals for the survival and rapid proliferation of centroblasts and T cells trigger the differentiation of centroblasts into centrocytes.

8.Syringocystadenoma Papilliferum Giganteum: Report of A Case.

Syringocystadenoma papilliferum is s rare tumor of disputed origin that was first reorted by peterson in 1892. Theclinical features of syringocystadenoma papilliferum are not constant. It may be present at birth or develop later in life. Its appearance varies from an irregular, flat, grey or reddish area to a grey or dark brown. raised and cauliflower-like surface. Sometimes it is moist, flesh or pedunculated and occasionally a verrucous plaque, or cyst may be seen. The typical lesion occurs most frequently on the scalp, and is a hairless smooth plaque until puberty, when a nodular verrucous transformation occurs. A 39 years old male patient had egg sized (3*5cm) and yellowish red colored verrucous tumor mass on his epigastric area, which had grown up slowly for 7 years. The diagnosis of syringocysyadenoma papilliferum was confirmed by histological examination. We repoet a case of syringocystadenoma papilliferum in unusually large size on the epigastric area.

9.A Case of Epidermolytic Hyperkeratosis.

Epidermolytic hyperkeratosis, previously called bullous congenital ichthyosiform erythroderma is a dominantly inherited disorcler and usually manifested by blisters at or shortly after birth. We report a case of epidermolytic hyperkeratosis in 9 year old girl vrho developed generalized erythema &: easily eroded vesicles at birth. Later the skin lesions progresed to characteristic thick, grayish-dark brown verruform cais, affecting predominantly flexural and intertriginous areas. Fiistologic findings show typical granular degeneration.

10.IgE and In Vivo Delayed Hypersensitivity to the Recall Antigens in Atopic Dermatitis and Their Relationship.

We measured serum 1gE by using IgE 'Eiken' radioirnrnunoassay Kit and delayed cutaneous hypersensitivity responsiveness by the Multitest CMI Kit (Institut Merieux) that simultaneously administrates 7 standardized recall antigens(tetanus, diphtheria, streptococcus, tuberculin, candida, trichophyton, proteus) and 70% W/V glycerin diluent control by multiple prick puncture. We studied 40 cases comprising 20 patients with atopic dermatitis (AD) and 20 healthy children as control. The results were obtained as follows; 1) The mean serum level of IgE in AD(63.3+/-395.2 1U/ml) was higher than in control(155.6+/-67.1IU/ml) (p<0.0l), but the intensity of reaction (IR) & rate of reaction (RR) in AD (l.34+/-1.06, 0.38+/-0.23) was not significantly decreased than in control (1.70+/-0.87,0.44+/-0.16). 2) 1n both AD & control group, tuberculin showed highest IR R RR. 3) Anergic state was found in only one patient with disseminated cutaneous lesions Sc 4) serum IgE, but rate of hypoergy was significantly increased in AD(65%) than in control (30%) (p<0.05). 4) Serum IgE was not statistically correlated with IR and RR, but IR was statistically correlated with RR (p<0.001, r=0.9504). 5) IgE level in severe AD (generalized skin lesion) was significantly increased than in both mild AD 8c control group (p<0. 001). but IR & RR were not statistically correlated with disease severity.
