1.The Experience of Concurrent Review of blood Transfusion Requests.

Blood utilization review has recently been used to minimize inappropriate transfusion, reduce post transfusion infections and improve the quality of transfusion practices. Ajou University Hospital is a 906 bed tertiary care teaching institution. Since March, 1995, the concurrent review of blood transfusion requests using institutional blood transfusion criteria has been initiated. To evaluate the effectiveness of the transfusion review, the units of blood components transfused per patient(total number of units of blood components transfused/total number of patients discharged) and inappropriate transfusion ratio of blood components(total number of units of blood components inappropriately transfused/total number of units of blood components transfused) were compared during the concurrent review for 9 months and pre-concurrent review for 9 months, respectively. The possible savings of blood components realized by the review were also extrapolated. The results were as follows : 1. The unit transfused per patient for pre-review and during review were 0.675 (9,080/ 13,422) and 0.654(12,123/18,546) for packed RBC(PRBC); 0.417 (5,602/ 13,422) and 0.296 (5,444/18,546) for fresh frozen plasma(FFP); 0.372(5,007/13,422) and 0.424(7,868/18,546) for platelet con-centrates(PC), respectively. 2. The inappropriate transfusion ratio of blood components for pre-review and during review were 0.039 and 0.009 for PRBC; 0.321 and 0.064 for FFP; 0.143 and 0.008 for PC, respectively. These differences were statistically significant(p=0.000). 3. The PRBC and FFP anticipated to have been saved by this review was 519 units and 2,992 units annually, respectively. 4. However, 2,621 units of PC were used additionaly during this review due to increased number of oncology patients and better hemostatic therapy practiced during the same period. In conclusion, the concurrent review of transfusion requests was very effective tool to educate residents in clinical departments the transfusion medicine and improve utilization of blood and its components.

Background and Objectives: This study aimed to examine factors of poor outcome by analyzing
the outcomes of bilateral subthalamic deep brain stimulation in Parkinson’s disease after 3 years.
Methods: We assumed that patients who could not manage independent life in their best stimulationon/medication-on
condition after a defi ned period might not have been a good surgical candidate.
A poor outcome is defi ned as a failure to maintain functional independence at three years during a
stimulation-on/medication-on state. Results: A total of 84 patients underwent bilateral subthalamic deep
brain stimulation and all were followed up for 3 years. We excluded one patient who had intracranial
hemorrhage. Twenty-one patients of the 83 patients could not keep up independent life even during
their best condition for the following reasons: freezing in 15 patients, dementia in 5, depression in 3,
musculoskeletal problems in 7, and cancer in one patient.
Conclusions: Many patients could not keep up independent life during their best condition as early as
three years after deep brain stimulation. Musculoskeletal problems were one major cause of disabilities,
as were freezing and dementia. We emphasize that musculoskeletal problems need more attention in
the preoperative screening of deep brain stimulation candidates and during the follow up.

8.Experimental Studies on the Influence of Testosterone into Pituitary Anterior Lobe, Thymus, Adrenal Gland and Testis.

White rabbits weighing about 2kg, divided in 2 groups: the first group is administered with 5 mg, of testosterone propionate and second group administered with 50mg. The animals are killed after one, two and three weeks of administration from each group. Pituitary gland, thymus, adrenal gland and testis are removed and cut into thin preparation followed by staining with homatoxylin-eosin and van Gieson. The slides are examined microscopically and the results are as follows: 1) The chief cells and basophilic cells of anterior lobe of pituitary gland showed slight degeneration alone with vascular dilatation and congestion. s a whole, the group of massive dose revealed more severe degeneration than in the other group. 2) Thymus manifested marked congestion and severe hemorrhage, interlobular connective tissue proliferation and atrophic change in the cortex and medulla with fatty degeneration. In the group administered with massive dose, the changes are more remarkable. 3) In the adrenal gland, whole layers of cortex revealed cloudy swelling and hyaline degeneration. The cells of zona reticularis showed marked congestion. In the massive dose group, these changes are slightly more emphasized. 4) In the testis, germ cells and Sertoli cells are undergoing cloudy swelling and thickening of the basement membrane, and decreased spermatogenesis. In the group of massive dose, the changes are more prominent.

9.A STUDY ON FRACTURE STRENGTH OF COLLARLESS METAL CERAMIC CROWN WITH DIFFERENT METAL COPING DESIGN.

The metal ceramic crown is currently the most popular complete veneer restoration in dentistry, but in many cases, the metal cervical collar at the facial margin is unesthetic and unacceptable. Facial porcelain margin has been used in place of it. But this dose not solve the problems, such as dark gingival discoloration and cervical opaque reflection of porcelain veneer. Recently, metal copings which were designed to terminate its labio-cervical end on the axial walls coronal to the shoulder have been clinically used to solve the esthetic problem of metal ceramic crown. But in this design, porcelain veneer of labio-cervical area which is not supported by metal may not be able to resist the stress during cementation and mastication. The purpose of this study was to evaluate fracture strength and fractured appearance of crowns according to different coping designs. A resin maxillary left central incisor analogue was prepared for a metal ceramic crown, and metal dies were made with duplication mold. Metal copings were made and assigned to one of four groups based on facial framework designs: group 1, coping with 0.5mm metal collar; group 2, metal extended to the shoulder; group 3, metal extended to 1 mm coronal to the shoulder; group 4, metal extended to 2mm coronal to the shoulder. Copings and crowns were adjusted to be same size and thickness, and cemented to metal dies with zinc phosphate cement by finger pressure. Fracture strength was measured with Instron Universal Testing Machine. Metaldies were anchored in Three-way-vice at 3mm below finish line and at 130degree inclined to the lone axis of the crown. Load was directed lingually at 2mm below midincisal edge. Load value at initial crack and at catastrophic fracture was recorded. The results obtained were as follows: 1. Fracture strength values at initial crack were higher in groups 1, 2 than in groups 3, 4 but this difference was not statistically significant(P<0.05). 2. Conventional metal collared crown had greater catastrophic fracture strength than any other collarless crowns. 3. The greater the labial metal coping reduction, the lower the catastrophic fracture strength of crowns but when more than 1mm of labial metal reduction was done, the difference in strengths was not statistically significant(p<0.05). 4. The strongest collarless coping design was group 2.
