Tumor necrosis factor-n (TNF - alpha) involved in the pathogenesis of multiple sclerosis and contribute to the degeneration of oligodendrocytes as well as neurons. TNF - alpha is produced by miocroglia and astrocytes, which also produce hormones and cytokines that influence its biological activity. Astrocytes, the major glial cells in the CNS, are capable of producing TNF - alpha at both the mRNA and protein levels in response to interleukine-1 (IL-1) or TNF - alpha. Two immunosuppressive cytokines, transforming growth factor - beta (TGF - beta) and IL-10, have been shown to influence glial cell function. TGF - beta can modulate the activity of glial cells by inhibiting interferon-gamma (IFN - gamma) induced expression of class II major histocompatibility complex (MHC) molecules on astrocytes and microglia. To explore the role of astrocytes in the production of TNF - alpha, astrocytes were pretreated with IL-10 or TGF - beta and then stimulated with IL-1p to determine their effects on TNF - alpha production. The secretion of TNF - alpha by human fetal astrocytes was markedly inhibited by TGF - beta at a low concentration. In contrast IL-10 had no effect on TNF - alpha mRNA level. These results show that TGF - beta may regulate the expression of TNF - alpha in activated human fetal astrocytes.

6.35 Cases of Percutaneous Stone Extraction.

On a standpoint of commonly available ESWL for the treatment of urinary stone, percutaneous stone extraction had the role of cooperation to it, and sometimes it could be the first procedure of choice. We have performed percutaneous extraction of the renal and upper ureteral stones in 35 renal unite. The overall success rate was 78.4%, average operation time was 81 minutes, mean fluoroscopic exposure time was 11 minutes, average postoperative hospital stay was 7 days. Causes of failure were as followings ; five inadequate calyceal puncture, one ureteral perforation, one impacted stone, and one downward stone migration. The complications were inappropriate position of the nephrostomy tube, prolonged hematuria through nephrostomy tube, and partial ureteral avulsion. They were not serious but resolved by conservative management.

7.A Clinical Survey of Cesarean Hysterectomy.

Cesarean hysterectomy remains a necessary procedure for life saving during intractable obstetric hemorrhage. The procedure itself is usually associated with considerable perioperative morbidity, especially when performed under emergency conditions. In this study, the outcomes of 15 cases of cesarean hysterectomy performed at Catholic university of Taegu-Hyosung hospital for 5 years from January, 1993 to December, 1997 were reviewed. There were 22,251 deliveries during this period, cesarean hysterectomy was performed in 13 of 5,753 cesarean section(0.22%) and in 2 of 16,698 vaginal deliveries(0.12%), so more frequently after cesarean section than vaginal deliveries. The age of patients varied from 23 to 39 years old. The higher the age of patients, the higher incidence of cesarean hysterectomy was noted. The most common indication of emergency cesarean hysterectomy was in uterine atony(53.3%), followed by placental disorders(33.3%), uterine rupture(6.7%), and uterine myoma with pregnancy (6.7%). All patients who had cesarean hysterectomy received transfusion from 3 pints to 13 pints. In aspect of fetal outcomes, live birth were 12 cases(80%), one case was fetal death in utero(6.6%), 2 infants were dead during the early neonatal period(13.3%) due to prematurity and RDS (respiratory distress syndrome), so the perinatal mortality rate was 20.0%. The postoperative complications were wound disruption, hematoma, febrile morbidity and disseminated intravascular coagulopathy.

8.Confluent and Reticulated Papillomatosis Successfully Treated with Minocycline.

Confluent and reticulated papillomatosis (Gougerot and Carteaud) is a rare but clinically distinct dermatosis of which the etiology is unknown. We report 2 cases. One case is a 16-year-old boy who had a hyperpigmented reticulated eruption on the trunk and both arms and the other case is a 33-year-old man who had a similar eruption on the chest and back. The former, with previous treatment with oral and topical antifungal agent in another clinic didn't show any improvement and the latter case, with initial treatment of itraconazole did not improve his skin lesion. The rashes of two patients virtually disappeared with administration of minocycline. These observations indicate that the role of microorganisms sensitive to minocycline or anti- proliferative effect of minocycline could be presumed.
