2.Effects of the Aroma Massage on Shoulder Pain, Depression, Sleep Disturbance in Hemiparesis Patients.

PURPOSE: The purpose of this study is to find out the effects of the aroma massage on shoulder pain, depression, and sleep disturbance in hemiparesis patients. METHOD: The research design was a nonequivalent control group nonsynchronized design. The data was collected from July 1 to August 31, 2003 at D medical Center in Busan. The subjects were 37 patients that they were assigned to two groups, nineteen subjects in the experimental group and seventeen subjects in the control group. The oil made from a blend of lavendar, clarysage, bergamot at a ratio 3:1:2 used for relieving shoulder pain, depression, sleep disturbance. The instruments used for this study were Visual Analogue Scale(VAS) for shoulder pain, Zung' depression scale, 4 score's scale for sleep disturbance. The data were analyzed using SPSS and hypotheses were examined with ANCOVA. RESULTS: Shoulder pain, depression, and sleep disturbance were significantly lower in the experimental group compared than the control group. CONCLUSION: The aroma massage using selected essential oils is an effective intervention for relieving shoulder pain, depression, sleep disturbance in hemiparesis patients. However differences between aroma massage and massage were not clear, further study needs to be done.

3.An Analysis of Histopathologic Evaluation of Lung Carcinomas in Last Ten Years.

Lung cancer is one of the most common types of malignancy in western nations with serious health problem, and it has become the leading cause of cancer death of males, second only to stomach cancer, in Korea. A review of the histopathology of 1363 cases (1231 patients) of lung carcinoma, diagnosed at the Keimyung University Medical center from 1987 to 1996, was performed to reclassify the type of carcinomas and to investigate the change in the distribution of histologic types of lung carcinoma according to age, sex and year. Among the 1363 cases, 132 patients underwent a surgical operation after biopsy. The diagnosis of each case was proven by histopathologic analysis of surgical specimens (13.2%) and biopsy materials (86.8%). The histologic types in our study were basically based on modified WHO classification (1982) and on new WHO classification (1999). The classification of small cell carcinoma was based on International Association for the Small Cell Lung Cancer (IASLC, 1988). Of the 1231 patients with lung carcinoma, 1012 were male and 219 were female (male to female ratio was 3.6:1). According to the analysis of age distribution, the most prevalent age group was 60~69 years in both sex as (n=516, 42.0%). Changing trends in sex distribution of lung carcinoma patients showed that the proportion of men had decreased throughout the years, whereas the proportion of women had significantly increased. Histologically, squamous cell carcinoma was the most common (n=624, 50.7%), followed by small cell carcinoma (21.1%), adenocarcinoma (18.1%), large cell undifferentiated carcinoma (2.1%), adenosquamous carcinoma (0.4%), and large cell neuroendocrine carcinoma (0.4%), in order of frequency. In men, squamous cell carcinoma was the most frequent type (55.1%). In women, adenocarcinoma was the most frequent type (39.7%). In both sexes, adenocarcinoma was the most common type in patients under the age of 40 (n=12, 41.4%), while squamous cell carcinoma proved the most frequent type in patients over the age of 40 (n=617, 51.3%). Changing trends of histologic types of lung cancer showed that the incidences of squamous cell carcinoma had significantly decreased throughout the years, whereas those of adenocarcinoma and small cell carcinoma had increased. In conclusion, the results showing increases in the percentage of female patients and in the number of cases of adenocarcinoma were noteworthy, and well correlated with other related reports.

4.The Experience of Concurrent Review of blood Transfusion Requests.

Blood utilization review has recently been used to minimize inappropriate transfusion, reduce post transfusion infections and improve the quality of transfusion practices. Ajou University Hospital is a 906 bed tertiary care teaching institution. Since March, 1995, the concurrent review of blood transfusion requests using institutional blood transfusion criteria has been initiated. To evaluate the effectiveness of the transfusion review, the units of blood components transfused per patient(total number of units of blood components transfused/total number of patients discharged) and inappropriate transfusion ratio of blood components(total number of units of blood components inappropriately transfused/total number of units of blood components transfused) were compared during the concurrent review for 9 months and pre-concurrent review for 9 months, respectively. The possible savings of blood components realized by the review were also extrapolated. The results were as follows : 1. The unit transfused per patient for pre-review and during review were 0.675 (9,080/ 13,422) and 0.654(12,123/18,546) for packed RBC(PRBC); 0.417 (5,602/ 13,422) and 0.296 (5,444/18,546) for fresh frozen plasma(FFP); 0.372(5,007/13,422) and 0.424(7,868/18,546) for platelet con-centrates(PC), respectively. 2. The inappropriate transfusion ratio of blood components for pre-review and during review were 0.039 and 0.009 for PRBC; 0.321 and 0.064 for FFP; 0.143 and 0.008 for PC, respectively. These differences were statistically significant(p=0.000). 3. The PRBC and FFP anticipated to have been saved by this review was 519 units and 2,992 units annually, respectively. 4. However, 2,621 units of PC were used additionaly during this review due to increased number of oncology patients and better hemostatic therapy practiced during the same period. In conclusion, the concurrent review of transfusion requests was very effective tool to educate residents in clinical departments the transfusion medicine and improve utilization of blood and its components.

7.Semiquantitative Food Frequency Method as an Epidemiological tool in a Rural community, Korea.

This study was performed to evaluate the semiquantitative food frequency questionnaire used to obtain information about. dietary patterns of adults in a rural county, Yang-pyeung Gun. The study population was 1311 adults aged 17~85 years. And well-trained interviewers interviewed subjects in 1990, 1992, 1993. The results are as follows. 1) Nutrients intake calculated from each questionnaire showed significant difference, because the number of question was not same(fifty questions in 1990, eighty questions in 1992, 1993). 2) Estimated nutrients intake from separate food items was significantly different when food items are combined in one question. 3) The agreement of nutrients intake level except vit C were high when the subjects are cross-classified by their ranks on the nutrients intake level from items in both questionnaires and on the nutrient intake level from items in each questionnaire. 4) And analysing this study data by multiple stepwise regression, we selected twenty seven food items whic could explained over 85% of inter-individual variation of nutrients(energy, carbohydrate, fat, protein, vit A, vit B1, vit B2, vit C, Ca). From above results, we suggested that it is neccessary to standardize questionnaire and perform validity studies.

8.Musculoskeletal problems need more attention in deep brain stimulation for Parkinson’s disease

Background and Objectives: This study aimed to examine factors of poor outcome by analyzing
the outcomes of bilateral subthalamic deep brain stimulation in Parkinson’s disease after 3 years.
Methods: We assumed that patients who could not manage independent life in their best stimulationon/medication-on
condition after a defi ned period might not have been a good surgical candidate.
A poor outcome is defi ned as a failure to maintain functional independence at three years during a
stimulation-on/medication-on state. Results: A total of 84 patients underwent bilateral subthalamic deep
brain stimulation and all were followed up for 3 years. We excluded one patient who had intracranial
hemorrhage. Twenty-one patients of the 83 patients could not keep up independent life even during
their best condition for the following reasons: freezing in 15 patients, dementia in 5, depression in 3,
musculoskeletal problems in 7, and cancer in one patient.
Conclusions: Many patients could not keep up independent life during their best condition as early as
three years after deep brain stimulation. Musculoskeletal problems were one major cause of disabilities,
as were freezing and dementia. We emphasize that musculoskeletal problems need more attention in
the preoperative screening of deep brain stimulation candidates and during the follow up.

10.Primitive Neuroectodermal Tumor Associated with Mature Cystic Teratoma of the Ovary: A case report.

Malignant transformation of benign cystic teratomas of the ovary is rare, with an incidence rate of 1-3%. The most common malignancy associated with benign cystic teratoma of the ovary is squamous cell carcinoma, which represents 90% of malignant transformations, followed by adenocarcinoma and carcinoid. Primary malignant neuroectodermal tumor in a mature cystic teratoma is extremely rare and microscopically can be subclassified as differentiated, primitive, and anaplatic. We experienced a case of primitive neuroectodermal tumor (PNET) associated with mature cystic teratoma of the ovary in a 11-year-old girl. Grossly, the ovary was a unilocular cyst which contained sebum material and disclosed several large, yellow to gray-tan polypoid excresences attached to the inner cystic wall. Except a large, fish-flesh, soft mass, the other polypoid masses were covered with sebum and contained small pieces of cartilage and tooth, and microscopically represented as mature teratoma. The large, fish-flesh, soft mass disclosed sheets and lobules of closely packed small cells with thin fibrovascular septa, which frequently replacing and encroaching the mature cystic teratoma. The small tumor cells were uniform and made up of round to oval nuclei with coarse chromatin and prominent nucleoli, very similar to those of PNET of the central nervous system.
