1.Human Cysticercosis in Subcutaneous Soft Tissues

80 cases of Cysticercosis were studied and analysed, which were diagnoses by biopsy at Seoul National University in the past three years from January-1978 to March-1980. The results were summerized as follows. 1. The relative incidence ratio of cysticercosis among 18,941 surgical specimens was 0.42%. 2. The age group between 11 and 50 years old occupied 87.5%. 3. Of the 80 cases, there were 38 males and 42 female patients, without significant sex predominence. 4. The common sites of cysticercosis were upper extremities (35.0%) and anterior surface of the trunk (37.5%) 5. Epidemiologically, there was marked predominence of rural area over urban area. 6. There were 8 cases (10%) of definite cerebral cysticercosis and 4 cases (5%) of suspicious cerebral cysticercosis. 7. In the cases of nodular masses of extremities, the parasitic infection, especiaily cysticercosis celluiosae, must be included in differential diagnosis distinguished from other soft tissue tumors.

2.Two cases of aquamous cell carcinoma of the lower lip treated with liquid nitrogen.

Squamous cell carcinoma of the lower lip can be developec by a number of underlying etiologic factors, the most outstanding of which is solar irracdiation, and is considerably more aggresive and has a much higher incidence of metastatic spread. Its early detection and effective treatment afford a high cure rate. Cryosurgery is a simple, efficient and effective method of treatment of skin cancer and is now a widely accepted modality for the treatment of these lesions. Two patients(67-year-old male and 57-year-old female) were presented with solitary or two masses on the lower lip without regional lymphadenopathy. These two cases showed clinical and histopathologic findings of squamous cell carcinoma. They were successfully treated by cryotherapy with liquid nitrogen without any significant side effects.

3.A case of leukemia cutis.

Leukemia cutis is defined as infiltration of the skin by malignant leukocytes that have originated from the bone marrow. The cutaneous eruption of leukcmia may be divided into the specific and non-specific lesion (leukemid). These may occur separately or in combination. True leukemia infiltrations of the skin consist of nodular, diffuse, or plaque-like infiltrations with swelling, macules, papules, ecchymoses, palpable purpura, erythroderma, and ulceration. A 55-year-old man presented with multiple as mptomatic dark-brown nodules on the whole body with recurrent gingival bleeding. The patient developed seoticemia and died 6 months after the onset of the disease.

4.A Case of Histoid Leproma with Mycobacterium leprae Detected in the Peripheral Blood by polymerase Chain Reaction.

Histoid leproma usually occur in pat.ients with lepromatous leproy who have had relapsed after being undcr treatment(usually sulfones) for a long time and who have shown variable improvement. A 56 year old female patient who had been suffering from lepromatous leprosy and had been treated which clapsone relatively regularly for approximately 28 years had a history of several shiny rice grain t.o peanut sized nodules on the face and right flank. Histopathologic examination of a specimen taken from a nodular lesian revealed a well circirmscrihed nodule composed mainly of spindle-shaped cells and epithe:ioi 1 cells on the upper and mid-levmis, and granuloma composed of epithelioid cells, multinucleated wait cells and foamy histiocytes on the lower dermis. In AFB stain, numerous acid-fast bacilli were scatt red dermis and epidermis, especially stratum corneum. This study opplied a polymease chain reaction(PCR) on sections of biopsy tissues and peripheral blood of pat.ient. Positive amplification of a 530 bp fragment was deteated not only in the lesion tissues but also in the per ipher al blood.

6.Follow-up Study of total Hip Arthroplasty

Total hip replacement arthroplasty has become an established procedure in the management of painful arthritic hip in past two decades. However there is no many reports that concern follow-up study in total hip replacement arthropiasty. This is a follow-up study in 100 total hip replacement arthroplastics in 86 patients of 157 total hip replacement arthroplasties in 135 patients, performed at the Department of Orthopedic Surgery, Seoul National University Hospital, between November 1973 and December 1979. The longest fallow-up was 5 years and the shortest 6 months, the average being 2 years and 6 months. 1. There were 54 males (63%) and 32 females (37%). 2. The underlying diseases of 100 hips were avascular necrosis of 35 hips, septic hip residua of 31 hips, primary osteoarthritis of 9 hips, old hip fracture of 9 hips, Legg-Perthes disease of 7 hips, rheumatoid arthritis of 5 hips, congenital dislocation of 3 hips, and failed endoprosthesis of 1 hip. 3. The types of prosthesis used were Muller ones in 82 hips, T-28 in 17 hips, and Charniey and Harris type one case respectively. 4. In the half cases of a hundred total hip replacement arthroplasties, the anterolateral or posterolateral approach was used without trochanteric osteotomy. 5. Postoperative complications were as follows: 13 cases of wound problem including 3 early deep infections, 2 dislocations, 1 femoral nerve palsy, 3 peroneal nerve palsies, 1 acetabular perforation, 2 femoral shaft perforations, 14 trochanteric problems including 11 cases of wire breakage, 2 nonunions, and 3 cases of trochanteric bursitis 3 ectopic ossifications, 3 cases of radiological loosening including 2 femoral and 1 acetabular. 6. The range of motion were improved from 28 to 5 of flexion contracture, from 91 to 101 of further . flexion, and from 16 to 48 of abduction in the average. 7. The improved ranges of motion were varied with disease group rather than type of prosthesis and the length of neck of prosthesis. 8. The function of each hip was evaluated preoperatively and postoperatively by the method of Harris and d'Aubigne, and improved from 55.3 and to 88.2 by Harris score and from 11.2 to 15.8 d Aubigne respectively.

8.TGF - beta Type II Receptor Anomaly and of Its Functional Restoration in Osteosarcoma Cell Lines.

TGF-p receptor mutation is now considered as one of the carcinogenic process of many tumors. To evaluate whether there is an abnormality in TGF-p type II receptor in osteosarcoma cell lines, we performed Northern analysis, cross-linking assay, luciferase activity and TGF-p growth inhibition assay in four osteosarcoma cell lines: G292, U202, HOS and SaOS. We also transfected the tumor cells with normal TGF-p type II receptor sequence to find if there is a possibility of gene therapy in osteosarcoma. In Northern analysis, Type II receptor expressions were decreased at SaOS, U202 and HOS cell lines. In cross-linking assay, all four cell lines didnt show type II receptor at their cell surface. The growth of these tumor cells were not suppressed by TGF-p. From these findings, we concluded that the normal production of TGF-p type II receptor was impaired in osteosarcoma. The transfection of these tumor cells with normal type II receptor sequence restored growth inhibition by TGF-p. This means even though TGF-p type II receptor is abnormal in osteosarcoma, we can restore its function by transfection of normal sequence. We think that the TGF-p type Il receptor gene therapy can be one of the treatment method for osteosarcoma in the future.
