4.Photographic Analysis of Lip Repair in the Bilateral complete cleft lip and palate.

Characteristic residual deformities are thought to be unavoidable in most bilateral complete cleft lip and palate patients. Newly-adopted designs and modified surgical strategies were applied to 40 consecutive patients of bilateral complete cleft lip and palate and some characteristic deformities could be overcome. The future midline on the lateral lip segment was marked on the white roll of thickest vermilion and the alar base was transposed medially without circumalar incision. The prolabial width was reduced to 4-5 mm in consideration of further widening. To evaluate the outcome of bilateral cleft lip repair, 15 patients with adequate photographic records were assessed separately and overall by a panel of judges. Gross facial attractiveness of the visual analog recording scale correlated well with the cleft impairment scale. Full-bodied vermilion with tubercle was achieved and upper lip tightening and tower lip pouting could be avoided. Adequate philtral width was maintained over a 3-year follow-up. Nasal tip projection and proper alar position could be accomplished without circumalar incision. Cleft impairment scale showed impaired characteristics os short columella, no nostril sill, transverse nostril axis, flat and undimpled philtrum, convex-profiled upper lip and a downward drawn nose. Columellar length and nostril axis were expected to improve with growth and development. Absence of the philtral dimple and convex-profilled upper lip still remain to be solved. A reliable, easy to perform, and inexpensive evaluation protocol was provided for bilateral cleft lip repair and selected surgical procedures and postoperative outcomes could be assessed with photographic records.

5.A Clinical Study of Twin Pregnancies with One Fetal Demise.

One fetal demise of twin pregnancy in the second or third trimester is an unusual and difficult problem in the managemcnt of pregnancy. It can be associated with an increased risk for mortality and morbidity in the remaining fetus and with maternal DIC. 255 twin gestations were delivered at our hospital between December 1984 and August 1998. 8 cases of twin pregnancies with one fetal demise were observed. We reviewed 8 cases of twin pregnancies with one fetal demise that had been managed conservatively. The incidence of twin pregnancies with one fetal demise was 3.14% in the study population. The incidence of preterm delivery was 87.5%. The perinatal mortality rate of surviving twins was 25%. There were no cases of maternal disseminated intravascular coagulation or infection. There were no cases of neurologic damages in the surviving twins. The main cause of neonatal death was prematurity. An expectant approach to the twin pregnancies with one fetal demise seems reasonable.

6.A Study on the Effectiveness of Clocortolone Pivalate (Purantix) Gream on Certain Common Skin Diseases.

In order to evaluate the effectiveness of clocortolone pivalate(Purantix) on corticosteroid responsive dermatoses, a symmetical paired. comparison technique was performed using 0. 1% clocortolone pivalate and 0. 025% triamcinolone acetonide creams. During the pericd of 7 months from September 1977 to March 1978, 37 patients with bilateral lesions of contact dermatitis, seborrheic dermatitis atopic dermatitis, lichen simplex chronicus and psoriasis were selected and treated with afore mentioned drugs, In the 15 psoriatic patients, clocrtolone pivalate was superior in 3, triamcinolone acetonide was superior in 2, 4oth drugs were equally effective in 10. In the evaluation of all 37 patients clocortolone pivalate was superior in 8, triamcinolone acetonide was sureior in 3, both drugs were equally effective in 26. The overall superiority of clocortolone pivalate to triamcinolone acetonide was non significant, There was no side effects with either drug. The simplicity of the vasoconstriction tcst has led to its general use in the study of the potency of new topical steroids. In the 6h occluded single-application vasocon-strictor test performed on 40 healthy students, there was no significant difference between clocortolone pivalate and triamcinolone acetonide, but they significantly d.ifferent from 1% hydrocortisone and the control; a simple ointment base(p<0.01).

9.Homocysteine and risk of cerebrovascular lesions: The PRESENT project

Objective: Cerebral white matter changes (WMCs) and silent brain infarcts (SBIs) are common
radiological findings in neurologically asymptomatic elderly people. Their presence is associated with
an increased risk of stroke or dementia. Homocysteine (Hcy) can cause atherosclerosis and may thus act
as a useful marker of stroke prior to symptom onset. This study aimed to evaluate the utility of Hcy as
a surrogate marker for early intervention for stroke in community-dwelling healthy elderly persons with
cerebrovascular lesions such as WMCs and SBIs. Methods: We assessed the relationship between the
Hcy level and presence of WMCs/SBIs on brain computed tomography (CT) in 554 elderly individuals
aged ≥65 years with no history of stroke or dementia. Results: The mean age of the participants was
74.43±7.32 years. Of the 554 patients, 106 (19.1%) had WMCs and/or SBI findings on brain CT. The
mean Hcy level was higher in participants with WMCs/SBIs (P=0.001). The WMCs/SBIs group had
a significantly greater number of participants with high Hcy levels than the non-WMCs/SBIs group
(P<0.001). High Hcy levels (≥12.3 μmol/L) was significantly associated with WMCs and SBIs even
after adjusting for age, Korean Mini Mental Status Examination score, education level, and vascular
risk factors such as hypertension, diabetes, hyperlipidemia and smoking status.
Conclusion: The result of our study suggests that regular monitoring of homocysteine level as a
modifiable risk factor may be helpful for predicting the association of cerebrovascular lesions such
as WMCs and SBIs for the prevention of future ischemic stroke and cognitive impairment.
