1.Self-care and Influencing Factors in Patients with Hypertension.

In chronic illness patients, promotion and self-care are important strategies for maintaining and improving health. Self-care behavior of hypertensive patient are affected by various variables. The purpose of this study was to investigate the relationships between three cognitive-perceptual resources-self efficacy, hardiness and perceived barriers and self-care in hypertensive patients. The subjects for the study were 102 patients who admitted to K medical center or visited to out-patient ward. The instruments used for this study included a survey of general characteristic. self-care, self efficacy, health related hardiness and perceived barriers. Analysis of data was done by use of mean, t-test, ANOVA, Person correlation coefficient and multiple regression The results of this study are summarized as follows: 1)The average score for the self-care was 56.18, for the hardiness was 120.90, for the self efficacy was 59.46 and for the perceived barriers was 34.60. 2)The relationships between the variables of self-care and cognitive-perceptual resource-self efficacy, hardiness, perceived barriers were significant. Self-care was significantly related to hardiness(r=.47, p<.001), suggesting that the person with high hardiness got higher self care scores. Self-care was significantly related to perceived barriers(r=-.41, p<.001), suggesting that the person with high perceived barriers got higher self care scores. A slight but significant correlation was also found between self-care and self efficacy(r=.26, p<.01). 3)In the relationships between general characteristics variables and self-care there were significant differences in gender(t=2.46, p<.05), job(t=3.72, p<.05), duration of illness(t=4.92, p<.05). 4)Stepwise multiple regression analysis was performed to examine the relationship between self-care and independent variables(self-efficacy, hardiness, perceived barriers) and selected general characteristics variables-job, duration of illness. These variables accounted for 36% of the variance in self-care score(F=13.34, p<.000). The hardiness was the most predictive factor(R2=19%) followed by job and perceived barriers. The finding of this study have implications for education of hypertensive patients and health care providers. For chronically ill patients adhere to self-care, we should focus on determining their personal resources such as hardiness and perceived barriers.

3.Cord Blood Leptin of Newborns with Normal Intrauterine Growth in 30-42 Weeks of Gestation.

PURPOSE: To evaluate leptin values in placental cord blood of newborns with normal intruterine growth after 30 weeks of gestation and to study the relationship of leptin with gestational age, birth size, ponderal index, BMI, placental weight, gender difference, Insulin-like Growth Factor-I(IGF-I) and Insulin-like Growth Factor binding protein-3(IGFBP-3). METHODS: Ninety healthy newborns(49 males and 41 females) with normal intrauterine growth were studied. They were classified into 4 groups according to the gestational age(GA):group I(30-33 weeks GA, n=17), group II(34-36 weeks GA, n= 24), group III(37-39 weeks GA, n=32) and group IV(40-42 weeks GA, n=17). At the time of delivery, newborn infant' weight, length, head circumference and placental weight were measured and BMI, ponderal index were calculated. A venous cord blood sample was collected at the time of delivery and serum leptin(RIA), IGF-I (IRMA) and IGFBP-3(IRMA) were measured. RESULTS: Cord blood leptin correlated positively with gestational age(r=0.730, P>0.01), body weight(r=0.686, P<0.01), length(r=0.419, P<0.01), head circumference(r= 0.498, P<0.01), BMI(r=0.750, P<0.01), ponderal index(r=0.704, P<0.01), placental weight(r=0.536, P<0.01), IGF-I(r=307, P<0.01) and IGFBP-3(r=0.736, P<0.01). Multiple linear regression analysis showed BMI, IGFBP-3 and sex(female) were independent predictors of leptin values. Leptin values increased progressively throughout gestation. Mean leptin values of group II(3.90+/-.28ng/mL) were significantly higher (P<0.01) than those of group I(1.92+/-.09ng/mL) and also values of group III(5.37+/-.52ng/mL) were higher than those of group II(P<0.05). But there was no significant difference between group III and group IV(6.45+/-.36ng/mL). Mean leptin values in females(5.07+/-.54ng/mL, n=41) were significantly higher than in males(4.07+/-.76 ng/mL, n=49) but there was no inter-gender difference in body weight, BMI, ponderal index, IGF-I, IGFBP-3. In addition, mean leptin values in females(7.45+/-.07 ng/mL) were significantly higher than in males(5.02+/-.09ng/mL) after 40 weeks of gestation. CONCLUSION: BMI, IGFBP-3 and sex(female) were independent predictors of leptin values. Leptin values increased significantly after 34 weeks of gestation and females had higher leptin values than males, especially in fullterm newborns. Leptin values in newborns had higher correlations with IGFBP-3 than with IGF-I as gestational age increased.

6.An Epidemiological Survey on Cholera in Pusan City 1970.

An epidemiological survey on cholera was made toward the 291 cases admitted in Pusan National University Hospital. The results were as follows. 1. The most predominant age groups among all cholera patients were form 30 to 39(21.3%). 2. 74.9% of all cases got under the educational background of primary school and 69.4% of all cases occurred at the low income group. 3. Daily average numbers of patient were 6.2 and maximum numbers were 15, August 19, 1970. 4. Among the food eaten prior to onset, drinking water was the most predominant with 16.2% and next, food and noodle groups were 14.8% and 10.3% respectively. 5. Regarding incubation period, it was found that "within 48 hours" was 83 5%. 6. Patients occurred mainly in Suh Ku area with 105(36.1%) of all closes. 7. Carrie rates among family members were 1.78%. 8. Carrier rates among residents of epidemic area were 0.l2%.

7.MRI of cervical carcinoma: before and after chemotherapy.

To evaluate usefulness of MR in assessment of tumor response to the chemotheraphy, we prospectively studied cases of cervical carcinoma with more then 2.5cm in diameter or stage IIb or more. Three courses of chemotherapy were performed with cisplatin and 5 F-U. MR images were obtained both before and after chemotherapy. Nine of 13 patients were undertaken radical hysterectomy after chemotherapy and MR examination. MR volumetry, stage and depth of stromal invasion were compared before and after chemotherapy. And in 9 patients who underwent radical hysterectomy, comparison of pathologic and MR imaging findings were also done. @ES The results were following. @EN 1) All tumors dectrased in volume (m=80.5%). 2) Five tumors (38.5%) reduced in stage, IB→CIS(1); IIA→CIS(1), IIA→IB(2), IIB→IB(1). 3) Depth of stromal invasion in MRI correlated well with that of histo-pathologic specimen in 7 of the 9 patients. Conclusively MR imaging is useful in assessement of tumor response to chemotherapy.

Human Papillomaviruses(HPVs) are regarded as sexually transmitable and closely associated with precancerous lesions or invasive cancers of the vulva, vagina, and cer V1X. In situ hybridization, besides showing higher sensitivity, allowed the identification of specific viral types. We studied 12 cases of condylomata accuminata for HPV typing by in situ hybridization techenique. Ten cases(83.3 % ) showed positive for HPV type 6/11, and 2 cases(16.7% ) were positive for HPV type 16/18.
