2.Carcinosarcoma of the Esophagus: A report of case.

Carcinosarcoma of the esophagus is a rare neoplasm composed of both carcinoma and spindle sarcomatous area. Usually the carcinoma component is a squamous cell carcinoma but rarely adenocarcinoma or undifferentiated carcinoma is found. The histogenesis of the sarcomatous component is still unknown. A case of ulcerated polypoid lesion with a stalk in esophagus was reported. Microscopically it was composed of spindle shaped cells interminled with squamous cell carcinoma and small cell carcinoma nests. No distinct transition between spindle shaped cells and carcinoma are was observed. Immunoreactivity to cytokeratin was observed in both carcinomatous and spindle cell component, but electron microscopic examination failed to demonstrated desmosome or tonofilaments in spindle cells. Undifferentiated small cell nests were reactive to neuron specific enolase and contained membrane bounded secretory granule in electron microscopy.

3.A Study on Blood Glucose Level and Self Management among Community Dwelling Type II Diabetes Patients.

PURPOSE: The purpose of this study was to examine relationships among blood glucose, HbA1c, and self management comparing these with general and illness characteristics of subjects with Type II Diabetes living in the community. METHODS: Using a comprehensive survey developed for the study, data were collected from 82 type II diabetes patients who were registered at five community health departments. The variables of self management, blood glucose and HbA1c were assessed by nurses. Data were analyzed with descriptive statistics including t-test, ANOVA and Pearson's correlation coefficient to compare self management and level of HbA1c by subject's general and illness characteristics and to examine the relationships among variables. RESULTS: About 73.2% of the subjects' HbA1c were 7.0% or higher and 54.9% of subjects' blood glucose were 200 mg/dL or higher. The level of self management was moderate. Most frequently perceived reasons for failure of blood glucose control were dietary failure (32.9%). There was significant relationship between self management and HbA1c(r=-.223, p=.040). The mean score of self management were higher among female (t=-2.37, p=.021), who are not on diabetes medication (t=6.70, p=.011). CONCLUSION: Comprehensive intervention is needed to improve dietary self management, especially for male and those who is on diabetes medication.

5.Use of Renal Scan(DTPA) for Clinical Follow-up of Renal Function after Extracorporeal Shock Wave Lithotripsy of Renal Stones.

Patients treated by extracorporeal shock wave lithotripsy are usually evaluated by excretory urography within 1 month after treatment to determine the clearance of stone debris and rule out asymptomatic obstruction. In an attempt to obtain more precise functional information, we used 99m-Tc-DTPA renal scan as the initial follow-up study after ESWL in 10 patients. GFR checked by renal scan shows no difference in pre and post ESWL. Our experience suggests that follow up after ESWL by radionuclide renal imaging studies provides specific functional information and also particular value in the management of patients with obstructing stone debris and diminished renal function. Radionuclide renal imaging studies may also reveal unexpected obstruction and functional impairment after ESWL of stones, and is recommended as routine follow-up study after ESWL.

Polycystic ovary syndrome is characterized by symptoms of oligomenorrhea, amenorrhea, infertility, hirsutism and obesity. It is known that the women with such diseases would have higher risks to coronary heart diseases, hypertension, DM and endometrial cancer later on, Known risk factors of endometrial cancer are nullipara, late menopause, obesity, DM, unopposed estrogen, tamoxifen treatment, and atypical endometrial hyperplasia ect. 75% of endometrial cancer occurs in age over 50 and 4% of endometrial cancer occurs in age under 40. Particulary endometrial cancer concurrent polycystic ovary syndrome occurs usually in people under age 40 and is commonly well differentiated cell type and is related to good prognosis. We experienced a case of endometrial cancer with polycystic ovary syndrome, who was early dignosed, presented with infertility and got an operation in our hospital, and report this case with a brief review of literature.

9.Survival Rate and Prognostic Factors of Cancer Patients Diagnosed in a University Hospital.

PURPOSE: This study was conducted to investigate case-fatality rates and survival rates, prognostic factors of prevalent five cancers(stomach, lung, liver, cervix, colon) in a university hospital located in Taejon City. METHODS: 2,158 cancer patients who have admitted the hospital from Jan. 1, 1991 to Dec. 31, 1998 were analysed. The higher ratio of outbreak and growing were selected among the investigated cancer data for over 100 of subjects in Korean cancer patient was examined into two method. The one is medical record and the other is affirm a government office for existence or not. RESULTS: The lung cancer was discovered for the highest fatality rate. The crude 5-year survival rate of all cancer patients was 43.9% and that in male was higher than that in female. The 5-year survival rate of stomach cancer patients were 49.7%, that treated with combined(operation and chemotherapy) were 66.2%, that with metastasis to lymph node were 48.1%, and distant metastasis were 31.9%. Lung cancer patients were 25.9%, that treated with operation were 42.7%, and that with metastasis to lymph node were 29.3%. Hepatoma patients were 25.5%, that treated with operation were 37.8%. Uterine cervix cancer patients were 74.8%, that treated with operation were 95.0%, that with metastasis to lymph node were 83.3%, and distant metastasis 74.8%. Colon cancer patients were 41.8%, that treated with operation were 50.2%, that with metastasis to lymph node were 33.4%. Prognostic factors affecting survival rate among stomach cancer patients were age, operation, operation with cancer chemotherpy, and metastais to lymph node and distant matastasis. Prognostic factors of uterine cervix cancer was age, and that of colon cancer were operation with radiotherapy, metatasis to lymph node and distant metastasis. CONCLUSION: case-fatality rate in male were higher than that in female, and increased with age. The cancer survival rate of female is high, the high in over 40 years group, and operation is exposed in the highest survival rate, also significant difference in metastasis level.

10.Clinical value of pretreatment serum cyfra 21-1 and SCC Ag levels in cervical cancer patients.

OBJECTIVES: SCC Ag(Squamous cell carcinoma antigen) is so far the most useful tumor marker in assisting clinical diagnosis of cervical cancer and follow-up after therapy. Elevated levels of cytokeratin 19-fragments(CYFRA 21-1) have recently been detected in large proportion of patients with non small cell cancer of the lung, and in particular those with squamous cell carcinoma. This study is to assess the clinical efficacy of CYFRA 21-1 with SCC Ag as the clinicopathologic parameter in cervical cancer. METHOD: Retrospective analysis of the serum tumor markers CYFRA 21-1 & SCC Ag in eighty cervical cancer patients was performed. RESULTS: Cut off values for SCC Ag & CYFRA 21-1 were 1.94 ng/ml, 3.11 ng/ml respectively. Using the cut-off point, the sensitivity, specificity, positive predictive value(PPV), and negative predictive value(NPV) of serum SCC were 55, 95, 97, 46%, respectively. Serum CYFRA 21-1 showed a sensitivity of 45%, specificity of 91%, PPV of 87%, and NPV of 55%. The combination of SCC and CYFRA 21-1 increased the sensitivity to 62%, with a specificity, PPV, and NPV of 72, 75, 58%. Serum levels of both markers were compared with tumor stage, lesion size and were significantly related. In FIGO stage Ib-IIa, the serum levels of SCC Ag & CYFRA were 2.2+/-3.9, 2.5+/-3.6 ng/ml and in FIGO stage IIb-IV, 12.2+/-15.2, 10.8+/-11.2 ng/ml. In < or =4cm of lesion size the serum levels of SCC Ag & CYFRA were 3.3+/-9.0, 4.5+/-7.6 ng/ml and in >4cm of lesion size, 11.8+/-11.9, 7.7+/-9.3 ng/ml. CONCLUSION: These data seems to show that serum CYFRA 21-1 may be of additional value in assessing the state of disease in some patients with cervical cancer. The prediction of recurrent cervical cancer with SCC Ag were improved by the combination with CYFRA 21-1 but further investigation is needed.
