1.A Case of Ki-1 Positive Large-Cell Lymphoma Transformed from Mycosis Fungoides.

Mycosis fungoides(MF) is a cutaneous T-cell lymphoma of low-grade malignancy characterized by the proliferation of small to medium-sized cerebriform lymphoid cells confined to the skin. It usually shows favorable prognosis, but morphologic transformation occurs in some cases and is often associated with a more aggressive clinical course. Herein we report a case of Ki-1 positive large cell lyrnphoma (Ki-1 LCL) which developed in the plaque stage of mycosis fungoides. Although the lesions responded well to low-dose methotrexate therapy and there was no evidence of local or systemic recurrence until now, continuous follow-up is needed because the prognosis of transformed MF is known to be poor compared with primary cutaneous Ki-1 LCL. We also discussed the differential diagnosis of primary cutaneous Ki-1 positive lymphoproliferative disorders.

2.Two Cases of Extramammary Paget' s Disease Showing a Good Response to Radiotherapy.

We report the clinical evolution of two elderly male patients with extramammary Paget's disease who were treated with radiotherapy. Both cases primarily involved the scrotum and penile root, and were not associated with any underlying or visceral malignancy. The total doses administered were 6000 cGy and 3000 cGy, respectively. A good clinical response was achieved in both cases. No major side effects were observed. The patients are being followed for any signs of recurrence and have been completely asymptomatic to date

3.Oncogene interaction in basal cell carcinomas of human skin.

The expression of the p53 protein (p53) was compared with those of several oncogenes including c-fos (Fos), c-jun (Jun), and epidermal growth factor receptor (EGFR1) using immunohistochemistry in frozen and paraffin-embedded sections of 25 basal cell carcinomas (BCCs) to find out any correlation between p53 and oncogenes in the pathogenesis of human BCC. In normal skin, positive reactions were obtained for EGFR1 and Fos, while p53 and Jun were negative in all cases. In the lesions, EGFR1 was observed in all cases and p53 was positive in 9 of 25 (36%). Fos was expressed in 21 of 25 (84%) and four negative cases were all p53-positive; this negative correlation between p53 and Fos staining was statistically significant (P< 0.01). Jun was detected in 14 of 20 (70%) and no significant relationship was observed between the expression of Jun and Fos or p53. These data suggest the possibility of down regulation of Fos expression by high levels of p53 protein. Further work is necessary to determine the mechanism of this interaction.

4.A Comparative Study of Positive Culture Rates Between Urethral Discharge and Urinay Sediment for the Diagnosing of Neisseria gonorrhoeae.

BACKGROUND: Although the culture of urethral discharge using a cutton swab is regarded as one of the most sensitive and reliable diagnostic methods of gonorrhea, it may give pain or diagnostic methods of gonorrhea, it may give pain or discomfort to patients, and its detection rate may be influenced by the examiner's technical ability. OBJECTIVE: The purpose of this study was to compare the positive culture rates between ure4thral discharge and urinary sediment for the diagnosing of Neisseria gonorrheae. METHODS: We compared this technique with the culture of urethral discharge using the specimens from 357 male patients with gonococcal infection at VD clinic of Jungku Pvblic Health center in Seoul from Jan 1991 to Jun 1993. RESULTS: The culture of urinary sediment was positive in 339 of 357(94.95), whereas the culture of urethral discharge was positive in 346 of 357(96.95). However, these data have no statistical significance(P<0.05). The obe observed sensitivity and specificity of culture of urinary sediment comjpared to culture of urethral discharge were 97.75 and 90.9%, respectively. The predictive value of positive and negative test compared to the culture of urethral discharge were 99.7% and 55.65, respectively. As our data showed low negative predictive value, a negative culture result of urinary sediment does not always imply a negative culture result of urethral discharge. CONCLUSION: Therefore, at present, the previous method of the culture of urethral discharge is more highly recommended and further studies for this diagnostic and/or new method of gonorrhea based on larger group of patients is encouraged.

5.Immunolocalization of Desmoglein in Autoimmune Pemphigus.

BACKGROUND: Pemphigus foliaceus(PF) and pemphigus vulgaris(PV) have different target antigens which belong to the desmoglein(DG) subfamily of the desmosomal cadherins; DG in the case of PF and PV antigen(PVA) in the case of PV. OBJECTIVE: Because DG is also a normal major component of the intercellular adhesive core, we investigated the immunohistochemical distribution of DG in PF to compare and contrast the findings with those of PV. METHODS: Immunohistochemical analysis using streptavidin-biotin complex method with anti-DG monoclonal antibody was done in six cases of PF and six cases of PV, as well as two samples of normal skin as control. RESULTS: Both disorders showed abnormal intense diffuse cytoplasmic staining patterns in the lesional skin. Contrary to PF, showing complete loss of normal, rim-like, membranous staining, two of six cases of PV showed the relatively well preserved normal staining pat-terns in the upper epidermis. CONCLUSION: The differences in the expression of DG in the lesional skins between PF and PV suggest that PVA is distinct from DG, although an overlapping of antigens between PF and PV could exist.

7.Balanitis Circumscripta Plasmacellularis.

A 79-year-old man with balanitis circumscripta plasmacellularis(BCP), presenting as an erythematous constricting band of the inner surface of the prepuce encircling the penile shaft is described. The biopsy specimen of the lesion showed, in addition to the typical histologic findings of BCP, increased fibrosis and decreased amount of elastic fibers which correlate well with our clinical observations. Electron microscopic examination revelaed no viral particles or elastic fibers. Immunohistologically, IgG was found to be the major immunoglobulin class in the plasma cellular inf iltrate.

8.Erythema Multiforme due to Topical Application of Diphenylcyclopropenone(DPCP).

We report a case of erythema multiforme following topical application of diphenylcyclopropenone (DPCP), which occurred during the artificial induction of contact dermatitis in one of two volunteer patients of the objective structured clinical examination(OSCE). The characteristic polycyclic erythematous target-like lesions developed on both hands and forearms after fourth sensitization with 0.1% DPCP in a period of a week because of lack of response. In spite of systemic and topical corticosteroid treatment, the skin lesions lasted one month accompanied with dermographic urticaria. A minimal dose of DPCP can be used for initial sensitization to reduce the risk of a severe generalized eruption like erythema multiforme, although allergic reaction is less likely to be dose-dependent.

Trichofolliculoma is an adnexal tumor of hair follicle oriyin, considered to be of intermediate differentiation between a hair follicle nevus, a simple hyperolasia of the hair follicle, and trichoepithelioma, Which usually lacks mature hair follicles. Grossly the tumor usually appears on the head or neck as a 0.5 to 1.0cm solitary white to pearly papule, Many lesions show a cental pore, and occasionally woolly or cottony hairs may protrude from this pore. Histologically, trichofolliculomas are characterized by a central dilated follicle filled with keratin or hair. Radiating from this primary hair follicle are numerous secndary hair follicles of varying degrees of maturation. An organized fibro-vascular stroma surrounds these epithelial structure, giving it a well-defined nodular appearance. We presented here two case of trichofolliculoma with typical clnical and histopathological findings, one in a 65 year-old female and the other a 56 year-old males.
